Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

Pursuant to 38 U.S.C. 111 and 38 C.F.R. Part 70, Subpart A, the Veterans Health Administration (VHA) Beneficiary Travel (BT) Program provides payments for authorized travel expenses to help Veterans and other beneficiaries obtain care or services from VHA and VA authorized non-VA providers in the community. VHA must administer payments according to statutory mandates, including the Payment Integrity Information Act of 2019 (PIIA) (Public Law 116-117). In compliance with the PIIA and other program requirements, VHA must gather certain information to determine whether BT eligibility and other criteria for approval have been met, and the amount of payment or reimbursement that is authorized under the BT program. Claimants may include Veterans and other BT beneficiaries, and entities or individuals who provided or paid for travel. Claimants may apply for BT reimbursement orally or in writing through VA Form 10-3542 or the Beneficiary Travel Self Service System (BTSSS). This standard collection of information is necessary to enable VHA to provide this benefit and appropriately ensure that funds are being paid to the correct claimant. VA Form 10-3542 has been revised to add intake fields for VHA use only. In the upper right area on page 1, two fields have been added for “Facility ID” and “Claim Received Date.” There are no changes to the estimated numbers of respondents or burden hours

Generic Clearance for NWS Customer Surveys

Key Information

Abstract

This request is for a revision and extension of a currently approved generic information collection. The question bank has been revised to ensure compliance and alignment with current Administration priorities and Executive Orders. This collection follows the guidelines contained in the OMB Resource Manual for Customer Surveys. In accordance with Executive Order 12862, the National Performance Review, and good management practices, National Oceanic and Atmospheric Administration (NOAA) offices seek approval to continue to gather customer feedback on services and/or products which can be used in planning for service/product modification and prioritization. Under this generic clearance, individual offices would use approved questionnaires and develop new questionnaires, as needed, by selecting subsets of the approved set of collection questions and tailoring those specific questions to be meaningful for their particular programs. These proposed questionnaires would then be submitted to OMB using a fast-track request for approval process, for which separate Federal Register notices are not required. This generic clearance will not be used to survey any bodies NOAA regulates unless precautions are taken to ensure that the respondents believe that they are not under any risk for not responding or for the contents of their responses, e.g., in no survey to such a population will the names and addresses of respondents be required. Two sets of survey questions are used for generation of program-level questionnaires. Quantitative questions seek to obtain numerical ratings from respondents on their satisfaction with various aspects of the product or service they obtained – satisfaction with the quality of the product, the courtesy of the staff, the format of and documentation for data received, and similar standard types of questions. The offices using such questions are able to determine which aspects of their program need improvement or have improved. Qualitative questions are more focused on who is using the product and service, how it is being used, and the medium or format in which the respondent would like to see data provided.

VHA Homeless Programs - Project CHALENG for Veterans

Key Information

Abstract

The Department of Veterans Affairs (VA) launched Project CHALENG (Community Homelessness Assessment, Local Education and Networking Groups) for Veterans in 1993 in response to Public Law 102-405, which required VA to make an assessment of the needs of homeless Veterans in coordination with other Federal departments, state and local government agencies, and nongovernmental agencies with experience working with homeless persons. Since 1993, VA has administered a needs assessment in accordance with guidance in Public Law 103-446 and Public Law 105-114. This collection of information is necessary to ensure that VA and community partners are developing services that are responsive to the needs of local homeless Veterans, in order to end homelessness and prevent new Veterans from experiencing homelessness. Over the years data from CHALENG has assisted VA in developing new services for Veterans such as the Homeless Veteran Dental Program (HVDP), the expansion of the Department of Housing and Urban Development-VA Supportive Housing (HUD-VASH) Program, the Veterans Justice Programs and Supportive Services for Veteran Families (SSVF). In addition community organizations use CHALENG data in grant applications to support services for homeless Veterans; grant applications are for VA, other Federal, local government, and community foundation dollars, which maximizes community participation in serving homeless Veterans. This collection will be a renewal of the collection approved in 2021. No revisions have been made to the questions; however, revisions to the burden hours are based upon the number of respondents in the previous year. The collection consists of two survey forms: one for Veterans (VA Form 10-10161) and one for VA staff members, community homeless providers, and interested community members (VA Form 10-10162). The only difference between the two survey forms are the introductory demographic questions; otherwise the survey is the same for both groups. The CHALENG Survey for Veterans (VA Form 10-10161) has been updated pursuant to Executive Order 14168: Defending Women from Gender Ideology Extremism and Restoring Biological Truth to the Federal Government. There are no longer gender options, and the “Gender” field in Question 3 has been changed to “Sex” with only “Female or Male” as choices.

VA Suicide Prevention 2.0 Program - Community Opinion Survey

Abstract

The information VA is proposing to collect does not currently exist. It will be used in order to accomplish three aims: 1) collect baseline data on the knowledge and attitudes of adult US citizens living in specified communities about Veterans, Veteran suicide, and resources available to Veterans to reduce suicide, prior to the implementation of suicide prevention programs; 2) collect follow-up data in the same communities to assess whether those knowledge and attitudes have changed over time; and 3) determine whether the programs and policies implemented by a community resulted in positive change in knowledge and attitudes. The data will be utilized by the Office of Mental Health and Suicide Prevention in VA Central Office to measure the return on investment of significant resources that have been invested to support communities in their (that is, the communities’) efforts to reduce Veteran suicide. Specifically, the Community-Based Interventions (CBI) arm of VA’s “Suicide Prevention 2.0” initiative has launched two different initiatives whose goals are to increase the successful implementation of best practices to prevent Veteran suicide in local communities. The data will allow VA to measure a baseline level of expected outcomes, follow-up levels, and to explore the role of new programs in those changes. The data will be used to perform program planning and evaluation. The data will also be used by the State teams that are engaged in the Governor’s Challenge initiative. GC is one of the initiatives supported by SP2.0 and is structured so that State teams are provided training and technical assistance by VA to expand their efforts to implement suicide prevention programs in their State. This data collection will assist the State teams to assess the effects of their new programming or policies. The Suicide Prevention 2.0 Program - Community Opinion Survey has been updated pursuant to Executive Order 14168: Defending Women from Gender Ideology Extremism and Restoring Biological Truth to the Federal Government. The gender question has been changed to “sex” and gender options have been deleted from the Survey.

Abstract

Section 313 of the Joseph Maxwell Cleland and Robert Joseph Dole Memorial Veterans Benefits and Health Care Improvement (Cleland-Dole) Act of 2022 (P.L. 117-328) requires the VA to conduct a study on financial and credit counseling by querying financial and credit counselors, homeless programs providers, and subject matters experts regarding the use of financial and credit counseling services. The intent of this survey is to identify financial and credit counseling needs of Veterans experiencing homelessness or at risk of experiencing homelessness, the financial and credit counseling services offered to these Veterans, the specific barriers that these Veterans have in accessing these financial and credit counseling services, and the effects of financial and credit counseling services on such outcomes as employment, housing status, income, and credit score. Abstract

EDFacts is a U.S. Department of Education (ED) initiative, conducted by the National Center for Education Statistics (NCES), to collect, analyze, report on, and promote the use of high-quality, pre-kindergarten through grade 12 (pre-K-12) performance data. By centralizing data provided by state education agencies about state level data, local education agencies, and schools, NCES uses the EDFacts data to report on students, schools, staff, services, and education outcomes at the state, district, and school levels. The centralized approach provides ED users with the ability to efficiently analyze and report on submitted data and has reduced the reporting burden for state and local data producers through the use of streamlined data collection, analysis, and reporting tools. EDFacts collects information on behalf of ED grant and program offices for approximately 170 data groups for all 50 states, Washington DC, Puerto Rico, and seven outlying areas and freely associated states (American Samoa, Federated States of Micronesia, Guam, Marshall Islands, Commonwealth of the Northern Mariana Islands, Republic of Palau, and the U.S. Virgin Islands), the Department of Defense Education Activity (DoDEA), and the Bureau of Indian Education (BIE). This request is to collect EDFacts data for the 2025-26, 2026-27, and 2027-28 school years. This collection package will be available for public comment during two open periods, a 60-day and a 30-day, after which revisions will be made accordingly. As part of the public comment period review, ED requests that SEAs and other stakeholders respond to the directed questions found in Attachment D. Due to overlap in the timing of data collection activities between consecutive years of the EDFacts collection, we are carrying over in this submission the approved SY 2024-25 data collection, which is scheduled to end in February 2026.

The Uniform Administrative Requirements in 2 CFR Part 200 require recipients of Federal financial assistance awards to monitor activities under Federal awards to ensure compliance with applicable Federal requirements and that performance expectations are achieved. Recipients of Federal Highway Administration (FHWA) administered grant awards and cooperative agreements include but are not limited to: State DOTs as defined in 23 U.S.C. 101, U.S. Territories, federally recognized tribes, local governments, and other entities identified by statute as eligible to receive Federal financial assistance from FHWA. The FHWA Performance Progress Report (FHWA PPR) form will be used by FHWA grant and cooperative agreement award recipients (1) to report key accomplishments; any scope, schedule, and budget concerns; and special events held during the reporting period and (2) to provide any updates on National Environmental Policy Act (NEPA) approval dates and grant project completion dates. Abstract

This ICR pertains to the renewal of the Custom Work surveys. The Custom Work program will survey farmers who have potentially paid for custom services during a specified reference period to gather information on the costs of these services. These services include land tillage, application of fertilizers and chemicals, planting, harvesting, hauling, various livestock tasks, and more. The program aims to provide farm operators with estimates of the average prices paid for different custom services in their state and/or local area. All questionnaires included in this information collection will be voluntary. This project is a cooperative effort between NASS and state agricultural departments and/or universities. Funding will be provided by the cooperating institutions under full cost recovery. The time between securing funding for an individual survey and the desired start of data collection is often too short to allow for a separate OMB approval for each survey. With this request, NASS will be able to provide services in a more timely manner.

Abstract

The Internal Revenue Code (IRC) at 26 U.S.C. 6061 provides that any return, statement, or other document submitted under the IRC’s provisions must be signed in accordance with the forms or regulations prescribed by the Secretary of the Treasury (the Secretary). Also, the Federal Alcohol Administration Act (FAA Act) at 27 U.S.C. 204(c) authorizes the Secretary to prescribe the manner and form of applications for basic permits issued under the Act. Under those authorities, the Alcohol and Tobacco Tax and Trade Bureau (TTB) regulations require individuals signing documents and forms filed with TTB on behalf of an applicant or principal to have specific authority to do so. As such, applicants and principals use form TTB F 5000.8, Power of Attorney, to delegate such authority to a designated individual and to report that delegation to TTB. Many documents and forms submitted to TTB are legally binding and have penalties for omissions or falsification, and TTB uses the collected information to determine who legally represents an applicant or permittee doing business with the agency.

Abstract

SSA is requesting to make revisions to the CBSV User Agreement. This collection instrument defines the conditions, terms, and safeguards under which SSA provides participating businesses with the verification of Social Security numbers. This action does not affect the public reporting burden. SSA developed CBSV in response to multiple requests from the business community. CBSV is a fee-for-service process which allows SSA to process high volumes of SSN verifications that companies need for business purposes. Since this is not required by law but is being provided as a service, businesses incur the costs of CBSV. The respondents are companies requesting SSN verifications from SSA as part of their business process. We are submitting a request for non-substantive changes to update the User Agreement to show current SSA personnel and contact information.

The FDIC National Survey of Unbanked and Underbanked Households is a key component of the FDIC’s efforts to comply with a Congressional mandate that the FDIC conduct biennial surveys to provide “a fair estimate of the size and worth of the unbanked market in the United States” and to answer additional related questions. The Financial Services Household Survey is the only nationally- and state-representative survey of this type for the United States. The Household Survey is scheduled to be conducted in partnership with the U.S. Census Bureau as a supplement to its June 2023 Current Population Survey (CPS). The survey collects information on U.S. households’ use of bank accounts, prepaid cards, nonbank online payment services and nonbank financial transaction services, and bank and nonbank credit. The results of this biennial survey will be published by the FDIC, and help inform policymakers, bankers, and researchers about bank account ownership and how households use the banking system and nonbank products and services to meet their financial needs.

Abstract

Under 30 CFR 7.4 and 15, MSHA tests and approves blasting units and explosive materials as permissible for use in mines, respectively. Under 30 CFR 57.22606(a), mine operators of Category III metal and nonmetal mines must notify the appropriate MSHA District Manager of all nonapproved explosive materials and blasting units prior to their use. Explosive materials used for blasting must be approved by MSHA under 30 CFR part 15, or nonapproved explosive materials must be evaluated and determined by the District Manager to be safe for blasting in a potentially gassy environment. The notice must also include the millisecond-delay interval between successive shots and between the first and last shot in a round.

Abstract

This request is to establish a new umbrella generic clearance for the Administration for Children and Families (ACF) to request information from potential participants at ACF gatherings, such as meetings or conferences. This may include large scale conferences, meetings for grantees or contractors, workshops, trainings, poster sessions, and other in-person and virtual gatherings for individuals with interest in ACF programs (clients, researchers, policymakers, etc.), among others. To ensure ACF has adequate information to plan these activities, the Agency must often collect information from potential participants such as basic contact information, preferences for attendance (mode, special requests, etc.), organizational affiliation, feedback about meeting content, etc. Additionally, some activities require ACF to have additional information to have the means to select the most appropriate participants for attendance according to the type or purpose of a given activity, or to group participants into the most appropriate category or activity during an event. This may include information about poster presentations, speaking panels, training courses, professional perspectives or experiences, etc. In addition, attendees may be asked to submit an application or abstract for prescreening to be selected for attendance. The planning for meetings is most often on a quick timeline and the standard timeline to comply with a full request under the Paperwork Reduction Act would inhibit the ability to collect information to inform these activities. Therefore, an umbrella generic is necessary to allow for quick turnaround requests for similar information collections related to these activities. As part of this generic, ACF requests OMB provide a response on individual generic information collections within 5 business days. If no response is received and the request meets the purposes, uses and scope outlined in this document, ACF may move forward with the proposed information collection.

Abstract

This is a request for an extension of a currently approved information collection. The National Oceanic and Atmospheric Administration’s (NOAA’s) National Marine Fisheries Service (NMFS) is responsible for conserving and recovering marine and anadromous species listed as threatened or endangered under the Endangered Species Act of 1973 (ESA; 16 U.S.C. 1531 et seq.). With some exception, the ESA prohibits “take” of listed species. Take is defined by the ESA as “to harass, harm, pursue, hunt, shoot, wound, kill, trap, capture, or collect, or to attempt to engage in any such conduct." Section 10(a)(1)(A) of the ESA allows NMFS to issue permits that authorize take of listed species for scientific purposes or to enhance their propagation or survival. The regulations implementing the authority to issue permits for scientific research or enhancement are found at 50 CFR §222.308. The information collection discussed here relates to the issuance and continued exercise of scientific research permits under the ESA. The collection has not changed materially since 2021: we have always required prospective permittees to fill out applications and always required permit holders to annually report on their activities. The current collection effort is essentially the same as it was four years ago.
