Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

Section 208.63 of the Board’s Regulation H - Membership of State Banking Institutions in the Federal Reserve System (12 CFR Part 208) requires state member banks to establish and maintain in writing procedures reasonably designed to ensure and monitor compliance with the provisions of the Bank Secrecy Act (BSA) and its implementing regulations. Sections 211.5(m)(1) and 211.24(j)(1) of the Board’s Regulation K - International Banking Operations (12 CFR Part 211) impose those same requirements on Edge and agreement corporations and, except for a federal branch or a federal agency or a state branch that is insured by the Federal Deposit Insurance Corporation (FDIC), the U.S. branches, agencies, and representative offices of foreign banks operating in the United States.

A banking organization is required to notify its primary Federal banking regulator of any “computer-security incident” that rises to the level of a “notification incident,” as soon as possible and no later than 36 hours after the banking organization determines that a notification incident has occurred. A bank service provider is required to notify each affected banking organization customer as soon as possible when the bank service provider determines that it has experienced a computer-security incident, that has caused, or is reasonably likely to cause, a material service disruption or degradation for four or more hours.

Hospice Information for Medicare Part D Plans (CMS-10538)

Key Information

Abstract

The Social Security Act in section 1861(dd) and Federal regulations in 42 CFR §418.106 and § 418.202(f) require hospice programs to provide individuals under hospice care with drugs and biologicals related to the palliation and management of the terminal illness as defined in the hospice plan of care. Medicare payment is made to the hospice for each day an eligible beneficiary is under the hospice's care, regardless of the amount of services provided on any given day. Because hospice care is a Medicare Part A benefit, drugs provided by the hospice and covered under the Medicare payment to the hospice program are not covered under Part D. The industry in conjunction with the National Council for Prescription Drug Programs, (NCPDP) developed a draft Medicare Part D Hospice Form to collect information necessary for the Part D sponsor to override a hospice prior authorization reject for beneficiaries enrolled in hospice. CMS made minor revisions to the NCPDP form to create this Standardized Form entitled Hospice Information for Medicare Part D Plans. This PRA submission is a request for approval of the standardized form available for use by Part D sponsors.

High Frequency Surveys Program Household Trends and Outlook Pulse Survey (June, August)

Key Information

Abstract

The High-Frequency Surveys Program was established as a natural progression from the creation of the Household Pulse Survey. The Census Bureau developed the Household Pulse Survey to produce near real-time data in a time of urgent and acute need to inform federal and state action in response to the Covid-19 pandemic. Changes in the measures over time provided insight into individuals’ experiences on social and economic dimensions during the period of the pandemic. It has evolved to include content on other emergent social and economic issues facing households and is designed to supplement the federal statistical system’s traditional benchmark data products with a new data source that provides relevant and timely information based on a high-quality sample frame, data integration, and cooperative expertise. In January 2025, the HPS and CHP will be relaunched as the Household Trends and Outlook Pulse Survey (HTOPS). The HTOPS will continue the strong tradition of the HPS by rapidly providing insight into national events that have social and economic impacts on U.S. households. It will complement more traditional federal surveys by producing data much closer to real time as the events develop and with the new longitudinal design, have the ability to track changes over time. Along with HPS content, the survey will enable the Census Bureau to address research and content development needs for its census and survey programs. The HTOPS will be one more tool among the Census Bureau's data collection efforts to provide the nation with quality, up-to-date information that informs our future.

COPS Community Policing Advancement Performance Report

Key Information

Abstract

The COPS Office established program goals and objectives during program planning and design (see § 200.202). The performance report questions were created to gather information necessary to measure a recipient's progress towards achieving award goals, identify promising practices of recipients, and build evidence upon which the COPS Office makes program and performance decisions. (see § 200.301). Performance reporting is a requirement of funding. Award recipients provide regular narrative and numerical updates on award activities bi-annually as a means for the awarding agency to assess whether the goals, objectives, and desired results of the program are being achieved. It is a method to determine how an award is progressing and whether any additional assistance is indicated (see § 200.329). The current performance report question set was used unilaterally for all programs but lacked specificity and provided inadequate data for determining effectiveness and relevancy of the individual programs. Revised performance report question sets specific to each program have been created for the following CPA programs: the Law Enforcement Mental Health and Wellness Act (LEMHWA); Community Policing Development Microgrants; Promoting Access to Crisis Teams (PACT); Safer Outcomes; Tribal Resources Grant Program – Technical Assistance (TRGP-TA); Collaborative Reform Initiative (CRI); Accreditation, and Collaborative Agreements.

The main purpose of Rule 19h-1 is to give the Commission a chance to review whether a person who is statutorily disqualified should be allowed to enter or to continue in the securities industry. Abstract

The NSF Directorate for STEM Education (EDU) Division of Undergraduate Education (DUE) requires evaluation and technical services for the Noyce program. Noyce has a 20-year history of responding to the critical need for highly effective K-12 STEM teachers in high-need school districts by recruiting and preparing talented undergraduate STEM majors and STEM professionals to pursue teaching careers in elementary and secondary schools. Since its inception, the program has commissioned three program evaluations, the most recent in 2017.

Abstract

The Federal Communications Commission (“Commission”) is requesting that the Office of Management and Budget (OMB) approve a revision of the information collection titled, “Application for Voluntary Assignment of Transfers and Controls, 47 CFR 73.3540” under OMB Control Number 3060-1290. There are no changes in the number of responses, respondents, annual burden hours for respondents. The Commission has updated the International Bureau Filing System (IBFS) to allow for filing of electronic forms directly into the system through an integrated web-based program with fillable fields. The integrated web-based program requires the use of an FCC Registration Number (FRN) will include support for Forms 314-IBFS, 315-IBFS, and 316-IBFS. Applicants will be required to submit these forms through the integrated web-based program. The forms were previously in development but are now completed and included as part of this collection. Please see the non-substantive change justification for the reason behind this change request submission to the Office of Management and Budget for review and approval.

Abstract

The development and maintenance of a cumulative database of all CMAQ projects is required by 23 U.S.C. 149(i)(1). This database must include specific information about each project such as name, location, sponsor, cost, and to the extent already measured by the project sponsor, cost-effectiveness based on reductions in emissions and congestion. The database must be readily available for public review either by publication or in electronically accessible format and means. States provide annual reports on all CMAQ obligations and deobligations taking place in each fiscal year. These reports include obligations of program funds; descriptions of individual projects; and potential impacts on air quality and congestion. The data provided in the annual reports and the CMAQ Public Access System allows for transparency, showing that the program continues to provide incremental benefits through enhanced regional and local air quality improvement, and through contributions to congestion relief. In order to populate the database, FHWA division offices, State DOTs, and MPOs develop a process for entering and approving project data in a timely manner. State DOTs submit the annual report to be approved by the FHWA Division by March 1 and FHWA HQ approves it by July 1 following the end of the previous Federal fiscal year (September 30). The annual report covers all CMAQ obligations for that previous fiscal year. Thus, State DOTs and MPOs report the data early enough that FHWA has time to review and comment on the report.
