Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Multistate Financial Institution Data Match and Federally Assisted State Transmitted Levy (MSFIDM/FAST Levy)

Key Information

Abstract

To satisfy any current and past-due support payments by an obligor, state child support enforcement agencies are statutorily required to attach and seize an obligor’s assets that are held in financial institutions. State child support enforcement agencies are required to enter into data matching agreements with financial institutions doing business in their state to locate obligors’ accounts. State child support enforcement agencies use information from the data match to help enforce the support obligation. The Office of Child Support Services (OCSS) operates the multistate financial institution data match program through the OCSS’s Federal Parent Locator Service (FPLS). OCSS operates Federally Assisted State Transmitted (FAST) Levy. FAST Levy is a secure and automated method of collecting and disseminating electronic levy notices between state child support enforcement agencies and financial institutions. FAST Levy helps child support agencies secure financial assets more quickly and efficiently. OMB approved the MSFIDM/FAST Levy information collection on January 19, 2024, for three years. OCSS changed a contact email in the Multistate Financial Institution Data Match (MSFIDM) record specifications and requests OMB approval of these minor revisions as a non-substantive change.

The Centers for Disease Control and Prevention (CDC) is currently approved to collect information needed to determine the annual pregnancy success rate of each clinic that provides assisted reproductive technology (ART) services (“Assisted Reproductive Technology Program Reporting System,” OMB No. 0920-0556, exp. 7/31/2018). This data collection is required by The Fertility Clinic Success Rate and Certification Act (FCSRCA), Section 2(a) of P.L. 102-493(42 USC 263 (a)-1) which mandates ART clinics to submit information to CDC and requires CDC to publish pregnancy success rates. CDC seeks OMB approval on a revision request to revise burden estimates, modify data elements to align with current clinical practice, and extend information collection for an additional three years. Compared to the burden previously approved, the total annualized burden requested is higher due to an increase in the average number of ART cycles performed per clinic, due to an overall increase in the utilization of ART in the United States.

Abstract

Claimants for Social Security disability payments or their representatives can use Form SSA-773-U4 to waive their right to appear at a disability hearing. Respondents complete the SSA-773-U4 through a personal interview with field office (FO) staff; Disability Determinations Service (DDS) staff; or Disability Hearing Unit (DHU) staff to insure the individual understands the due process rights associated with the decision to waive personal appearance at a disability hearing by a DHO. We conduct these personal interviews either in person or via telephone with the claimants or their representatives. Once we obtain the information on the SSA-733-U4, the DHO uses the signed form as a basis for not holding a hearing, and for preparing a written decision on the claimant’s request for disability payments based solely on the evidence of record. The respondents are disability claimants for Social Security benefits or SSI payments, or their representatives, who wish to waive their right to appear at a disability hearing.

Abstract

The Federal Trade Commission has issued a Final Rule amending the Ophthalmic Practice Rules (“Eyeglass Rule” or “Rule”) to add new information collection and recordkeeping requirements for ophthalmologists and optometrists, and is requesting approval for the new collection. The FTC has determined after numerous proceedings that prescribers’ failure to follow the longstanding requirement to release eyeglass prescriptions to consumers is an ongoing, prevalent problem, and that this failure causes harm to consumers and competition. The new information requirements are intended to ensure that consumers automatically receive a copy of their eyeglass prescription at the end of their eye exam so that they can comparison-shop and purchase eyeglasses from the seller of their choice. The requirements are further intended to ensure that prescribers retain records of electronic prescription release and of patients’ confirmations of prescription receipt sufficient to demonstrate prescribers’ compliance with the Rule and its disclosure requirements. The FTC has structured the amendments to achieve consumer protection objectives while also minimizing burden on prescribers.

Diaper Distribution Demonstration and Research Pilot Beneficiary Information

Key Information

Abstract

The Office of Community Services (OCS), Administration for Children and Families (ACF), U.S. Department of Health and Human Services (HHS), is requesting approval of a collection to provide information on baseline diaper need for participants served by the Diaper Distribution Demonstration and Research Pilot (DDDRP), as well as on changes in diaper need and family financial security and self-sufficiency after receipt of DDDRP services. The DDDRP is the first-ever federal diaper pilot, and research on diaper need and how diaper programs address that need is nascent. This data collection will supply critical information on diaper need and participant outcomes to provide timely information to program administrators at the federal level in addition to program administrators of DDDRP grant recipients. OCS has been collecting this information under the Formative Data Collections for ACF Program Support generic information collection (OMB #0970-0531). The formative collection supported the development of two virtual surveys hosted by OCS. The surveys included an enrollment version that was administered before participants took home their first pack of diapers and a follow-up version administered after participants had received diapers for several months. This information collection will build upon what OCS has learned from the formative collection and feedback received during the 60-day public comment period to continue collecting beneficiary data while transitioning to a new approach led by grant recipients. With this information collection, current grant recipients that are already using the virtual surveys hosted by OCS will continue to use the OCS-hosted enrollment survey for newly enrolling beneficiaries. Grant recipients awarded in fiscal year (FY) 2024 and later will use information they collect from individuals through their enrollment and tracking processes to complete an aggregate report of beneficiary characteristics and outcomes. The aggregate report will serve as a supplement to the Performance Progress Report that is approved under the Generic Performance Progress Reports collection (OMB #0970-0490).

Abstract

This information collection request (ICR) relates to three new forms necessary for the efficient operation of the National Highway Traffic Safety Administration’s (NHTSA) whistleblower program, as established by 49 U.S.C. § 30172 (the Whistleblower Act). The Whistleblower Act authorizes the Secretary of Transportation (the Secretary) to pay an award to eligible whistleblowers who voluntarily provide original information, if the information provided leads to the successful resolution of a covered action. The three forms are the WB-INFO form, the WB-AWARD form, and the WB-RELEASE form. All of the information collections are collected on an as-needed basis. One of the information collections is completely voluntary and the other two are voluntary, but required to receive a benefit. Under the final rule, a potential whistleblower is required to submit a WB-INFO form and to submit a WB-AWARD form in order to qualify for a whistleblower award. A potential whistleblower may choose to voluntarily submit the WB-RELEASE form. Only minor changes, for clarity, were made to the forms in the final rule. The WB-INFO form will be used to permit NHTSA and its staff to collect information from potential whistleblowers about themselves, their legal representatives, and their allegations relating to any potential violation of the Safety Act or regulation thereunder. If a potential whistleblower proceeds anonymously, the individual must have a legal representative. That legal representative will retain their client’s signed WB-INFO form. The WB-AWARD form will permit NHTSA to collect information relating to a claimant’s eligibility for an award. This will allow NHTSA’s Administrator to determine claims for whistleblower awards. The WB-RELEASE form will provide a means for the potential whistleblower to provide consent for NHTSA to disclose information which could reasonably be expected to reveal the identity of the potential whistleblower. Being able to disclose this information may allow NHTSA to open a public investigation or proceed more efficiently with an investigation into the potential whistleblower’s allegations. The likely respondents to the forms (the WB-INFO, the WB-AWARD, and the WB-RELEASE form) are those employees or contractors of motor vehicle manufacturers, part suppliers, and dealerships who wish to provide NHTSA with information relating to any potential motor vehicle defect, potential noncompliance, or any violation or alleged violation of any notification or reporting requirement of the Safety Act or regulation thereunder. The anticipated recipients of the WB-INFO, WB-AWARD and WB-RELEASE forms are those individuals at NHTSA who have a need to know the information. This may include attorneys in NHTSA’s Office of the Chief Counsel and staff in NHTSA’s Office of Enforcement.

Abstract

The MEP Centers’ primary mission is to strengthen the global competitiveness of U.S.-based manufacturing by providing information, decision support, and implementation assistance to smaller manufacturing firms in adopting new, more advanced manufacturing technologies, techniques, and business best practices. Almost 1,700 knowledgeable specialists provide technical and business assistance. NIST MEP provides funding to the Centers through Cooperative Agreements. To ensure that Cooperative Agreement recipients are effectively disseminating the most current technical and business solutions to small and medium-sized manufacturers in the United States, MEP will collect and analyze information from the MEP Centers. MEP is mandated to collect this information by the regulations governing the operation of the MEP program (15 CFR 290, 291, and 292) and the Government Performance and Results Act of 1993 (GPRA, 5 CFR 1320). The advent of H.R. 1274 – Section 2 (Manufacturing Extension Partnership Program Center Extension) effectively removes the “Sunset Provisions” on the original MEP regulations (15 CFR 290, 291, and 292), and has made MEP a permanent investor in the Centers with the need to collect evaluative and informative data.

NIST Center for Neutron Research (NCNR) Information Management System (IMS) and Summer School Application

Key Information

Abstract

The National Institute for Standards and Technology (NIST) houses the NCNR which is a National User Facility, chartered to serve the nation’s scientific community by providing unique experimental apparatus for scientific studies using neutron scattering. Since the demand for such measurements exceeds the available resources, NCNR designed an Information Management System (IMS) that implements a peer-reviewed proposal based system to allocate the instrumentation to the best scientific proposals, collect information from the selected external participants to enable their scientific visits to NIST/NCNR, and track the results of those visits. This information is saved in a Privacy Act System of Records and has an applicable Statement and SORN uploaded in the supplementary section.

Key Information

Abstract

The collection of information associated with CFR parts 215, Railroad Freight Car Safety Standards (FCSS) requires that any railroad carrier that has a freight car assigned to dedicated service be stenciled to indicate special service. FRA reviews this information to determine if the equipment is safe to operate and if the operation qualifies for dedicated service. The required information is also used by the railroads to provide identification and control for carriers so that dedicated cars remain in the prescribed service. FRA is amending the FCSS to implement the SAFE TRAINS Act mandate that FRA issue a regulation to monitor and enforce industry's compliance with the standards of the Act. FRA is requiring freight car manufacturers to certify annually by electronic submission that each freight car complies with the Act before it operates on the U.S. general railroad system of transportation. This information collection is mandatory, submitted on an as needed basis, and there is a requirement for record keeping.

Military Severely Injured Joint Support Operations Center (MSIJSOC) and Travel Protocol Office (TPO) Programs

Abstract

Under the Aviation and Transportation Security Act (ATSA), TSA is responsible for security in all modes of transportation including screening operations for passenger air transportation and for carrying out such other duties relating to the transportation security as it considers appropriate. See sec. 101(a) of ATSA (Pub. L. 107-71, 115 Stat. 597 (November 19, 2001) (codified at 49 U.S.C. 114). The Helping Heroes Fly Act directs TSA to develop and implement a process to support and facilitate the ease of travel and, to the extent possible, provide expedited passenger screening services for severely injured or disabled members of the Armed Forces and severely injured or disabled veterans through passenger screening. See sec. 2 of the Helping Heroes Fly Act (Pub. L. 113–27, 127 Stat. 503 (Aug. 9, 2013) (codified at 49 U.S.C. 44927). Based on these requirements, TSA established the MSIJSOC and TPO programs to support and facilitate the movement of wounded warriors, severely injured military personnel, veterans, and other travelers requiring an escort through the airport security screening process.
