Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

Section 728 of the Dodd-Frank Act specifically requires the Commodity Futures Trading Commission (“CFTC” or “Commission”) to establish certain standards for the registration and governance of swap data repositories (“SDRs”), including regulations regarding SDRs reporting data to the Commission and to the public. In addition to implementing standards governing registration of and statutory duties applicable to SDRs, the CFTC is also required to adopt regulations that mitigate certain conflicts of interest in the operation of these entities and ensure appropriate levels of access by the Commission and certain other regulators to the data maintained by SDRs. The information from this collection is necessary for the Commission to perform its regulatory mandate to provide oversight of the markets and SDRs. Without accurate reporting of the data as set forth in the Commission’s regulations discussed in this statement, the Commission would be unable to fulfill its regulatory mandate of monitoring systemic risk.

Abstract

SSA requires payees to submit a written report accounting for the use of money paid to Social Security or Supplemental Security Income (SSI) recipients, and to establish and maintain a dedicated account for these payments. SSA uses Form SSA-6233-BK to: (1) Ensure the payee uses the payments for the recipient’s current maintenance and personal needs; and (2) confirm the expenditures of funds from the dedicated account remain in compliance with the law. Although we mail a paper Form SSA-6233-BK to the payee, the report can also be completed through telephone contact and then be mailed to the payee for signature, or during a face-to-face interview. Abstract

Parents or legal guardians seeking to obtain or retain SSI eligibility for their children use Form SSA-3881-BK to provide SSA with the addresses of non medical sources such as schools, counselors, agencies, organizations, or therapists who have information about a child’s function. The adjudicative team (a disability examiner and a medical or psychological consultant) of the State disability determination services (DDS) office collects the information to help determine a child’s claim or continuing eligibility for SSI. The respondents are the parents, guardians, and other caretakers of applicants who appeal SSI childhood disability decisions, or of recipients undergoing a continuing disability review.

Abstract

This request is for the revision and extension of a current information collection. The National Marine Fisheries Service operates a fee-for-service Seafood Inspection Program (SIP), available to all segments of the seafood industry. Participants request services such as product inspection, export health and legal harvest certification, and facility approval. Information is collected from participants to confirm the identity of products being inspected and certified, as well as to show compliance with Program requirements. The implementing regulations for this Program at 50 CFR 260 are being updated to bring the regulation more in line with current practices, remove outdated text, and streamline seafood inspection services. The last updated information collection (published January 19, 2022, with expiration date January 31, 2025) already included all of the changes proposed with this rulemaking, including the shift to online inspection and service requests and housing day-to-day procedures in the online SIP Manual versus in codified text. The information collection is being revised and extended to add a previously overlooked Surety form and adjust the burden figures.

Abstract

SSA uses Form SSA-821-BK to collect employment information to determine whether individuals have worked after becoming disabled and, if so, whether the work is SGA. We use the information to make a determination of eligibility for disability payments. Specifically, we use Form SSA-821-BK to obtain information from individuals during the initial SSI and SSDI claims process; the Continuing Disability Review (CDR) process; and appeals involving work issues. An SSA employee may fill out the form during an in office or telephone interview, or the applicants or recipients may fill it out themselves. SSA reviews and evaluates the data to determine if the applicant or recipient meets the disability requirements of the law. If respondents work when they apply for benefits, they must complete this collection to obtain SSI or SSDI. The respondents are SSDI applicants or recipients, and SSI applicants.

This revision adds requirements for the purchase and validation of charter halibut stamps, adds a new field to the ADF&G saltwater charter logbook to validate stamps, and adds appeals for an Initial Administrative Determination received for a validated charter halibut stamp deficit. This revision does not add respondents but does increase the logbook burden by 1 minute for the new field. This revision is due to the final rule (RIN 0648-BN18) that requires a charter halibut stamp for charter vessel anglers in International Pacific Halibut Commission Regulatory Areas 2C and 3A. This rule is necessary to promote stability and economic viability in the charter halibut fishery, and is intended to promote the goals and objectives of the Magnuson-Stevens Act, the Northern Pacific Halibut Act of 1982, and other applicable laws. This supporting statement was updated from the proposed rule to the final rule to be consistent with regulatory and editorial changes to the rule. These updates did not change respondents, responses, or burden. This information collection contains the logbook, landing reports, permit and transfer applications, and administrative appeals for the Pacific halibut charter fishery off Alaska. This collection is necessary for NMFS to manage and administer the charter halibut fishery under the Charter Halibut Limited Access Program and the Pacific Halibut Catch Sharing Plan.

This revision increases the respondents for Longline Pot Gear Marker Buoys, which increases the total annual responses, burden hours, and costs for this collection. This rule does not change the burden or cost estimates because they allow for differences in the time and cost to mark buoys. This rule also affects information collection requirements approved under OMB Control Number 0648-0515. NMFS is submitting a separate request to revise -0515. This information collection for -0353 contains the gear identification requirements for groundfish fisheries in the Exclusive Economic Zone off Alaska and is necessary to facilitate enforcement of fishery regulations.

This rule also affects information collection requirements approved under OMB Control Number 0648-0353. NMFS is submitting a separate request to revise -0353. One change not associated with the rule was made from the proposed rule to the final rule. The form for the IFQ/CDQ landing report has been revised to increase clarity and improve consistency with other forms for the NMFS Alaska Region. This does not change the number of respondents and responses or the burden for this form.

School Pulse Panel 2024-25 Data Collection

Abstract

The School Pulse Panel is an ongoing study conducted by the National Center for Education Statistics (NCES), part of the Institute of Education Sciences (IES), within the United States Department of Education, to collect extensive data on current and emerging issues concerning students and staff in U.S. public primary, middle, high, and combined-grade schools. Specifically, the survey will ask school leaders about topics such as staffing, college and career readiness, artificial intelligence, tutoring, summer and after-school programs, facilities, and learning strategies and recovery, among other topics. The SPP has become one of the nation's main sources of timely and reliable data on issues concerning the education environment, as reported by principals in U.S. public schools. The preliminary activities for the 2024-25 data collection was formally cleared in January 2024 (OMB# 1850-0969 v.12) and the SPP monthly data collection package was formally cleared in June 2024 (OMB# 1850-0969 v.13). A change request (v.14) was cleared in June 2024 to make changes to the 2024-25 preliminary activities package in order to modify the July 2024 communication materials and screener survey. A change request (v.15) was cleared in August 2024 to make changes to the August 2024, September 2024, and October 2024 instruments, informed by cognitive testing results with school staff. A 30-day public comment period package (v.16) was cleared in October 2024 that included new items (within the scope of the research domains previously established) to be collected on the November 2024, December 2024, and January 2025 instruments, as well as planned items for the rest of the study year, through June 2025. A change request (v.17) was cleared in November 2024 to make changes to the December 2024, January 2025, and February 2025 instruments, informed by cognitive testing results with school staff. The new revision (v.18) is focused on a 30-day public comment period on new items (within the scope of the research domains previously established) to be collected on the March 2025-June 2025 School Pulse Panels. These items are considered very close to final and will go through minimal testing with school personnel to examine any comprehension concerns with item wording. This testing will occur simultaneously with when the package is out for the 30-day public comment period. Feedback from this testing, as well as additional input from SPP stakeholders, will result in modifications and additions that will be reflected in future change requests. The costs to the government have not changed as a result of this amendment, nor has the projected respondent burden. As of January 31st, 2025, cognitive testing for March and April questionnaires have been completed. March and April changes to Appendix B1 and Appendix C are based on the results of cognitive testing to improve the quality of the data or to better capture the intent of the question. Additionally, to ensure compliance with President Donald Trump's executive orders (E.O.) titled Ending Radical and Wasteful Government DEI Programs and Preferencing on and Initial Rescissions of Harmful Executive Orders and Actions on January 20, 2025, we request removal of two sub-questions from the May 2025 questionnaire (OMB# 1850-0969). Lastly, we have updated Appendix A - Communication Materials to include a notice for respondents should changes occur to our monthly data collection plan, such as a government shutdown.

Abstract

SAMHSA is requesting clearance for eleven data collection instruments and forms related to the implementation and impact studies to be conducted as part of the evaluation. The evaluation will provide new information about how grants support adoption of the CCBHC model, how grants influence sustainable expansion of CCBHC capacities beyond the grant period, and how implementation of the model influences client-level outcomes. An evaluation of the IA and PDI grantees specifically provides an opportunity to examine implementation successes and challenges within each area of the certification criteria among a larger and more diverse group of clinics operating in different state and community contexts. By carefully measuring that context and how it changes over time, the evaluation can provide new information about how factors such as alternative sources of funding and state/community support for the model can influence implementation and outcomes. Finally, this evaluation has an opportunity to assess changes in quality of care and longitudinal client-level health, behavioral health, and functional outcomes.

NCUA Application to Request Consent Pursuant to Section 205(d) of the Federal Credit Union Act

Key Information

Abstract

This information collection is required under Section 205(d) of the FCU Act, as amended by the FHBA (Section 205(d)), to allow the NCUA Board to make an informed decision whether to grant a waiver of the prohibition imposed by law under Section 205(d). Section 205(d) prohibits a person who has been convicted of any criminal offense involving dishonesty or breach of trust, or who has entered into a pretrial diversion or similar program in connection with a prosecution for such offense, from participating in the affairs of a federally-insured credit union except with the prior written consent of the NCUA Board.
