What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Key Information

Abstract

As part of SSA’s disability determination process, we use Forms SSA-3375-BK through SSA-3379-BK to request information from a child’s parent or guardian for children applying for Supplemental Security Income (SSI). The five different versions of the form contain questions about the child’s day-to-day functioning appropriate to a particular age group; thus, respondents use only one version of the form for each child. The adjudicative team (disability examiners and medical or psychological consultants) of State disability determination services offices collect the information on the appropriate version of this form (in conjunction with medical and other evidence) to form a complete picture of the children’s ability to function and their impairment-related limitations. The adjudicative team uses the completed profile to determine: (1) if each child’s impairment(s) results in marked and severe functional limitations; and (2) whether each child is disabled. The respondents are parents and guardians of child applicants for SSI.

An evaluation of the IA and PDI grantees specifically provides an opportunity to examine implementation successes and challenges within each area of the certification criteria among a larger and more diverse group of clinics operating in different state and community contexts. Community Residential Care (CRC) Program - Recordkeeping, Incident Reporting, Applications

Key Information

Abstract

VA is authorized under 38 U.S.C. 1730 to assist veterans by referring them for placement, and aiding veterans in obtaining placement, in Community Residential Care (CRC) facilities, which include Medical Foster Homes (MFHs). One of the standards a CRC facility must meet is the requirement to maintain records on each resident in a secure place. Under 38 CFR 17.63(i), facility records must include emergency notification procedures and a copy of all signed agreements with the resident. These records must be maintained by the CRC facility, and the CRC facility must make those records available for VA inspection upon request. An MFH is a subtype of CRC and, under 38 CFR 17.74(q), is required to comply with the recordkeeping requirements of 38 CFR 17.63(i). In addition, the CRC facility must maintain and make available, upon request of the approving official, records related to CRC staff requirements and provide that the CRC facility has sufficient, qualified staff on duty and available to care for the resident and ensure the health and safety of each resident. VA is adding to this collection the requirement under 38 CFR 17.63(j)(3) that CRC facilities report alleged violations involving mistreatment, neglect, or abuse of a veteran to the approving official. VA requires the CRC facility to document and investigate evidence of an alleged violation, including misappropriation of resident property. VA views the reporting, documenting, and investigating of an alleged incident and the subsequent report of the results of the investigation to be one collection of information. VA Forms 10-2407 and 10-387 also are being included in this collection. The 10-2407 form is the CRC application used by residential care home sponsors seeking VA approval to provide care to veterans. The 10-387 form is the MFH application used by medical foster home caregivers seeking VA approval to provide care to veterans. The applicants supply references and agree to a VA inspection of the home, a monthly rate, and compliance with other VA standards for residential care. This information is collected under the authority of 38 U.S.C. 1720 and 38 U.S.C.1730.

Moving to Work, Landlord Incentives Cohort Evaluation

Key Information

Abstract

The Office of Policy Development and Research (PD&R) at the U. S. Department of Housing and Urban Development (HUD) contracted with Abt Associates (in partnership with the University of Hawai’i) to conduct an evaluation of the Moving to Work, Landlord Incentives cohort. As required under the Paperwork Reduction Act, HUD seeks approval from the Office of Management and Budget (OMB) for data collection instruments associated with the evaluation. The Housing Choice Voucher (HCV) program is tenant-based housing assistance program and is the Department of Housing and Urban Development’s (HUD’s) largest rental assistance program. Rather than limiting households to rental units located in specific housing projects, the HCV program lets households use vouchers to find their own housing in the private rental market. For this reason, the HCV program works well only if private market landlords are willing to make their units available to voucher holders. Many public housing agencies (PHAs) struggle to attract and retain landlords in the program, leading to low rates for voucher holders successfully leasing units and other program inefficiencies. Moving to Work (MTW) is a HUD demonstration program that encourages PHAs to test ways to increase the cost effectiveness of federal housing programs, encourage greater self-sufficiency of households receiving housing assistance, and increase housing choice for low-income families. MTW designation gives PHAs relief from many of the regulations and statutory provisions that apply to HCV and public housing programs. MTW was initially authorized by Congress in 1996, and in 2016, HUD was authorized to expand the Demonstration to an additional 100 PHAs. In January 2021, HUD published a Request for Applications for the Landlord Incentives Cohort of the MTW Expansion. In this cohort, PHAs will implement incentives for landlords to encourage their participation in the HCV program. In January 2022, HUD announced that twenty-nine (29) PHAs were selected to participate in the Landlord Incentives Cohort. PHAs participating in the Landlord Incentives Cohort must adopt at least two incentives from a menu of nine incentive options. Incentive options include payment standards flexibility, one-time financial incentives (such as signing bonuses and damage reimbursements), and simplified inspections processes. The goals of these incentives are: (1) to increase landlord participation in the HCV program and (2) to increase voucher holder success rates in leasing units in the program. The Moving to Work Landlord Incentives Evaluation (“Landlord Incentives Evaluation”), led by Abt Associates, will examine how PHAs implement these incentives and how these incentives affect program outcomes. This request relates to primary data to be collected for the Landlord Incentives Evaluation. This supporting statement is the first in a series of OMB submissions that correspond to an array of data collection activities for the evaluation of the Moving to Work, Landlord Incentives cohort. HUD seeks clearance in this submission for: • Semi-structured interview guides for site visits and telephone interviews with staff from treatment and a subset of comparison PHAs; • Online surveys to treatment and comparison PHAs; and • Semi-structured interview guides for site visits with landlords within treatment and a subset of comparison PHA jurisdictions.

Abstract

The UCFE law (5 U.S.C. 8501, et seq.) requires state workforce agencies to administer the UCFE program in accordance with the same terms and provisions of the paying state's unemployment insurance law, which apply to unemployed claimants who worked in the private sector. Each state agency must be able to obtain certain information (wage, separation data) about each claimant filing claims for UCFE benefits to enable them to determine his/her eligibility for benefits. The Department of Labor (DOL) has prescribed forms to enable the state agencies to obtain this necessary information from the individual's Federal employing agency. Each of these forms is essential to the UCFE claims process and the frequency of use varies depending upon the circumstances involved. These are model forms which states may customize as needed to collect the necessary information required to operate the UCFE program.

Abstract

This information collection supports implementation of the Secure Federal LEASEs Act (Pub. L. 116-276) to reduce security risks in high-security leased space. Section 3 of the bill requires agencies, before entering into a lease agreement for high-security leased space, to require the contractor to identify the immediate or highest-level owner of the space, including any financing entity, and disclose whether that owner is a foreign person or entity, including the country associated with the ownership entity. The contractor shall (1) provide such identification and disclosure when first submitting a proposal in response to a solicitation; and (2) update such information annually. This information is partially implemented in the Federal Acquisition Regulation (FAR) through the provisions at FAR 52.204-3, Taxpayer Identification, FAR 52.204-7, System for Award Management, FAR 52.204-17, Ownership and Control of Offeror, and the clause at FAR 52.204-13, System for Award Management Maintenance. OMB Control Numbers 9000-0097 and 9000-0185 covers the FAR provisions and clause. However, as the FAR does not account for foreign financing as required by the Act, GSA issued an interim rule for lease acquisitions of high-security space to identify the required information through GSAR 552.270-33.

Title VIII of the Act requires SSA to pay a monthly benefit to qualified World War II veterans who reside outside the United States. When an SVB overpayment occurs, the beneficiary can request a waiver of recovery of the overpayment, or a change in the repayment rate, using Form SSA-2032-BK. Respondents are beneficiaries who have overpayments on their Title VIII record and wish to file a claim for waiver of recovery or change in repayment rate. We are submitting this non-substantive change request for removal of the signature requirement from this form.
