Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

Sections 106 and 202 of the Federal Credit Union Act require federally insured credit unions to make financial reports to the NCUA. Section 741.6 requires insured credit unions to submit a Credit Union Profile (NCUA Form 4501A) and update the Profile within 10 days of election or appointment of senior management or volunteer officials or 30 days of other changes in Program information. The NCUA website further directs credit unions to review and certify their Profiles every Call Report cycle. Credit union information collected through the Profile is essential to the NCUA supervision of federal credit unions and also facilitates the NCUA monitoring of other credit unions with share accounts insured by the National Credit Union Share Insurance Fund (NCUSIF).

Key Information

Abstract

Diamondback terrapins are the only brackish water turtles in the United States. These turtles inhabit coastal marshes and mangroves from Massachusetts to Texas. Most terrapin populations are in decline due to commercial harvest, incidental capture in crab traps, and loss of nesting habitat due to coastal development. Today, the illegal pet trade has added extra pressure to these already declining populations. While much is known about terrapins along the US Atlantic coast, very little information is available about terrapin populations that inhabit Florida’s northwest coast. In fact, the International Union for the Conservation of Nature (IUCN) does not even include Northwest Florida within the terrapins’ range however recent studies confirm that terrapins do inhabit the region. This project would use citizen scientists to conduct surveys in potential terrapin habitat throughout the Northwest Florida region to document terrapins and terrapin signs, including hatched eggs and crawls. Volunteers collect detailed information on when and where terrapins or their signs are observed, environmental parameters such as air temperature and weather during the observation, habitat information such as vegetation type at the observation site, level and type of human activities observed at the site, and data on the terrapins or their signs such as number of tracks, nests, eggs or individual terrapins observed. Managers, including State and National Parks, need this information to document species’ status and inform conservation decisions, including whether to list or de-list species.

Abstract

We are proposing to require hospitals to deliver, as soon as possible after certain conditions are met and prior to release from the hospital, a new standardized beneficiary notice, informing eligible beneficiaries of the change in their status, the resulting effect on Medicare coverage of their stay, and their appeal rights if they wish to challenge that change. This new notice will be called the Medicare Change of Status Notice (MCSN). This is as required by the order from the Federal district court for the District of Connecticut in the case Alexander v. Azar, 613 F. Supp. 3d 559 (D. Conn. 2020)), aff’d sub nom., Barrows v. Becerra, 24 F.4th 116 (2d Cir. 2022).

Abstract

SSA uses Form SSA-L5061 to obtain rental subsidy information, which enables the agency to determine and verify an income value for these subsidies. We use this income value to determine eligibility for SSI, and to calculate the correct amount of SSI payable to the claimant. SSA bases an individual’s eligibility for SSI payments, in part, on the amount of countable income the individual receives. Income includes in-kind support and maintenance in the form of room or rent, such as a subsidized rental arrangement. SSA requires claimants to assist in obtaining this information to prevent a delay or overpayment with their SSI payments. We collect this information only if the SSI applicant or recipient is the parent or child of the landlords (respondent). For most respondents, we collect this information once per year or less, via telephone or face-to-face personal interview. The Claim Specialist (CS) records the information in our SSI Claims System, and we require verbal attestation in lieu of a wet signature. However, if the CS is unable to contact the respondent via the telephone or face to face interview, we print and mail a paper form to the respondent for completion. The respondent completes, signs, and returns the form to the CS. Upon receipt, the CS documents the information in the SSI Claims System or for non-SSI Claims System cases, faxes the form into the appropriate electronic folder and shreds the paper form. The respondents are landlords related to the SSI beneficiaries as a parent or child. We are submitting this non-substantive change request to remove the signature requirement from this form.

Abstract

HRSA conducts gatherings for various purposes, including conferences, meetings, workshops, webinars, trainings, communities of practice, focus groups, and other in-person or virtual gatherings for individuals and organizations that are interested in HRSA programs. To ensure that HRSA has sufficient information to plan, convene, administer, and evaluate the effectiveness of these gatherings, HRSA must collect information from potential attendees including contact information, organizational information, logistical information (e.g., preferred delivery methods, need for specific aids or services pursuant to the Americans with Disabilities Act), and feedback about the gathering’s content. Furthermore, HRSA may conduct a test of knowledge to see what attendees know about the subject matter before or during the meeting or focus group. In some instances, attendees may also apply and/or submit an abstract for prescreening to be selected for attendance. After the gathering concludes, attendees may be asked to complete an evaluation form and/or a test of knowledge to measure the gathering’s effectiveness.

Reporting of Sea Turtle Incidental Takes in Virginia Chesapeake Bay Pound Net Operations

Abstract

This request is for extension of a current information collection. This action would continue the reporting measure requiring all Virginia Chesapeake Bay pound net fishermen to report interactions with endangered and threatened sea turtles, found both live and dead, in their pound net operations. This reporting is required under Endangered Species Act (ESA) regulations, and is necessary to ensure sea turtles are being conserved and protected, as mandated by the ESA. When a live or dead sea turtle is discovered during a pound net trip, the Virginia pound net fisherman is required to report the incidental take to National Marine Fisheries Service (NMFS) and, if necessary, the appropriate rehabilitation and stranding network. This information will be used to monitor the level of incidental take in the state-managed Virginia pound net fishery and ensure that the seasonal pound net leader restrictions (50 CFR 223.206(d)(10)) are adequately protecting listed sea turtles. Based on the number of sea turtle takes anticipated in the Virginia pound net fishery and the available number of Virginia pound net fishermen and pound nets, the number of responses anticipated on an annual basis is 1,003.

Abstract

This collection enables the submission of respirators for NIOSH evaluation under 42 C.F.R. 84 “Approval of Respiratory Devices” requirements. The updated ICR accounts for an increase of 6,218 estimated total annual burden hours (from 123,702 total annual burden hours per year in 2017 to 129,920 total annual burden hours in 2020). This occurred because NIOSH issued an interim final rulemaking entitled “Approval Tests and Standards for Air-Purifying Particulate Respirators” to allow for the approval of respirators in a new class, PAPR100, better suited to the needs of workers in the healthcare and public safety sectors currently experiencing a shortage of air-purifying particulate respirators, due to Coronavirus Disease 2019 (COVID-19), the disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2).• The resulting data allows NIOSH to certify that qualifying respirators meet published criteria, allowing proper selection of respiratory protection This Non-Substantive Change Request is submitted to revise the incentive structure based on activities of the contractor. Consent Forms and the Supporting Statement A will also be slightly modified. There is no anticipated change associated with currently approved burden.

Abstract

Department of Defense (DoD) Form 2064, “Certificate of Death,” is used by family members of deceased DoD employees and contractors to settle estates, insurance claims, and other post death related matters. This form is also used by the DoD to track the cause and manner of death of service members, federal employees, their family members and dependents when a death occurs outside the continental United States (OCONUS). DD Form 2064 generates a death certificate that is accessible to next-of-kin. The form is also used by OCONUS hospitals and allows for the transportation of human remains.

Abstract

This is a revision of an approved information collection associated with a Final Rulemaking published on October 18, 2024. The Notice of Proposed Rulemaking (NPRM) was published on February 15, 2024, and OIRA provided comments on the proposed rule on May 2, 2024. The NPRM included a change in notification criteria and a small increase in the respondent universe. Although FTA received 27 comments after the NPRM was published, none pertained to the information collection or the related burden hours or costs. Consequently, there are no changes from the NPRM to this Final Rulemaking submission. The purpose of the SSO program is to oversee safety throughout the Nation’s rail transit systems. The entities responding to this information collection (IC) are States and rail transit agencies (RTAs). States must establish a safety oversight agency to oversee the safety of their rail transit agencies in their jurisdiction. The States and RTAs are required to provide FTA specific information to obtain and retain funding that will assist them in carrying out SSO activities. The information provided is reporting in nature and includes an annual report summarizing their program activity from the previous calendar year; pre-audit documents; periodic requests in the form of safety advisories issued by FTA due to a safety/accident related event; financial and milestone reports as part of the grant management process and the collection of incident reports as a result of a major safety event. This information is submitted electronically to FTA’s SSO program managers and incident investigators on an annual and as needed basis.

The NRC is requesting OMB approval of a plan for a generic collection of information. The need and practicality of the collection can be evaluated, but the details of the specific individual collections will not be known until a later time. The Agreement States and non-Agreement States will be asked on a one-time or as needed basis to respond to a specific incident, to gather information on licensing and inspection practices or other technical information, or to provide comments on proposed policy and program updates. The results of such information requests, which are authorized under Section 274(b) of the Atomic Energy Act, will be utilized in part by the NRC in preparing responses to Congressional inquiries. In addition, the information can assist the Commission in its considerations and decisions involving Atomic Energy Act materials programs in an effort to make the national nuclear materials program more uniform and consistent.
