What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

SSA bases eligibility for Supplemental Security Income (SSI) on the needs of the recipient. In part, we assess need through determining the amount of income a recipient receives. This income includes in-kind support and maintenance in the form of food and shelter home owners provide. SSA uses Form SSA-8011-F3, Statement of Household Expenses and Contributions, to determine whether the claimant or recipient receives in-kind support and maintenance. This is necessary to determine: (1) The claimant’s or recipient’s eligibility for SSI, and (2) the SSI payment amount. SSA only uses this form in cases where SSA needs the householder’s (head of household) corroboration of in-kind support and maintenance. The SSA-8011-F3 provides information, which could affect SSI eligibility and payment amount. An SSA claims specialist collects the information on Form SSA-8011-F3 through telephone contact with the respondents, or through face-to-face interviews. The claims specialist records the information in our electronic SSI Claims System. When we use this procedure, we do not use a paper Form SSA-8011-F3, and we do not require a wet signature, rather we request verbal attestation. However, for those few instances when we use a paper form, we ensure the appropriate person, i.e., the householder, signs the form, and then the claims specialist documents the information in the SSI Claims System; faxes the form into the appropriate electronic folder; and shreds the form. Respondents are householders of homes in which an SSI applicant or recipient resides. We are submitting this Non-Substantive Change Request to remove the signature requirement and replacing the Penalty of Perjury Statement with a Penalty of Perjury Warning.

Abstract

The FERC-725L information collection contains six reliability standards: MOD-025-2, MOD-026-1, MOD-027-1, MOD-031-3, MOD-032-1, and MOD-033-2. Reliability Standards MOD-025-2, MOD-026-1, and MOD-027-1 serve the purpose of generator verification Reliability Standards. These three standards ensure: • generators remain in operation during specified voltage and frequency excursions; properly coordinate protective relays and generator voltage regulator controls; and • generator models accurately reflect the generator’s capabilities and equipment performance. Reliability Standard MOD-031-3 provides planners and operators access to actual and forecast demand and energy data as needed to perform resource adequacy studies. Reliability Standards MOD-032-1 and MOD-033-2 were designed to replace, consolidate and improve upon past MOD standards by addressing system-level modeling data and validation requirements necessary for developing planning models and the Interconnection-wide cases that are integral to analyzing the reliability of the Bulk-Power System.

Abstract

The Consolidated Appropriations Act, 2021, signed into law on December 27, 2020, added Section 104A of the Community Development Banking and Financial Institutions Act of 1994 (the “Act”). Section 104A authorizes the Secretary of the Treasury to establish the Program to support the efforts of low- and moderate-income community financial institutions to, among other things, provide loans, grants, and forbearance for small businesses, minority-owned businesses, and consumers, especially in low-income and underserved communities, including persistent poverty counties, that may be disproportionately impacted by the economic effects of the COVID-19 pandemic by providing direct and indirect capital investments in low-and moderate-income community financial institutions. As required by the Act, the interest and dividend rates payable on ECIP instruments are determined based on the increase in the amount of lending by an institution within minority, rural, and urban low-income and underserved communities and to low- and moderate-income borrowers during the preceding annual period compared to a baseline set from the annual period ending on September 30, 2020. To establish this baseline level of qualified lending, Treasury will be collecting an Initial Supplemental Report (ISR) from participating applicants. Applications and other forms for the ECIP program were previously approved under OMB No. 1505-0267. Following review of the applications, Treasury made awards and is now ready to begin scheduling closings.

Publicly Owned Treatment Works (POTW) Influent Per- and Polyfluoroalkyl Substances (PFAS) Study and National Sewage Sludge Survey

Abstract

This data collection is intended to gather data necessary to identify and quantify sources of PFAS discharges, prioritize industrial categories for potential regulation, and establish a current national data set of sewage sludge characteristics. This data collection will support two separate United States Environmental Protection Agency (EPA) studies. POTW Influent PFAS Study. As announced in the EPAs Effluent Guidelines Program Plan 15, published in January 2023, the EPA will conduct the POTW Influent PFAS Study to gather nationwide data on industrial and domestic wastewater discharges of PFAS into the environment. To accomplish this goal, the EPA will require a subset of POTWs across the United States to complete an electronic questionnaire and conduct wastewater sampling to obtain information on types and quantities of PFAS in discharges from industrial facilities, domestic wastewater or sewage, and POTW influent and effluent. The data will primarily be used to identify and prioritize industrial point source categories where additional study or regulations may be warranted to control PFAS discharges. National Sewage Sludge Survey (NSSS). The EPA also plans to conduct a NSSS to identify the presence of pollutants in sewage sludge using samples collected from POTWs. This NSSS will focus on obtaining a current national data set on PFAS concentrations and ancillary parameters found in sewage sludge. The data generated by the NSSS will inform future risk assessments and risk management options for sewage sludge. The data collection design represents the EPAs efforts to gather sufficient data to perform the analyses required to accurately review and determine whether Effluent Limitations Guidelines and Standards (ELGs) are warranted to address PFAS discharges from for specific industries and administer an ICR that limits the burden placed on respondents.

Ambulatory Surgery Center Survey on Patient Safety Culture Database

Abstract

Background on the Ambulatory Surgery Center Survey on Patient Safety Culture (ASC SOPS): The survey is designed to enable ASCs to assess provider and staff perspectives about patient safety issues, medical error, and error reporting. The survey includes 27 items that measure 8 composites of patient safety culture. In addition to the composite items, the survey includes one item measuring how often ASCs document near-misses; one item asking whether the respondent is in the room during surgeries, procedures, or treatments; and three items about communication before and after surgeries, procedures, or treatments. The survey also includes an overall rating item on patient safety, two items about respondent characteristics, and a section for open-ended comments. AHRQ made the survey publicly available along with a Survey User’s Guide and other toolkit materials in May 2015 on the AHRQ website. The AHRQ ASC SOPS Database consists of data from the AHRQ ASC Survey on Patient Safety Culture. Ambulatory surgery centers in the U.S. can voluntarily submit data from the survey to AHRQ, through its contractor, Westat. The ASC SOPS Database (OMB NO. 0935-0242; Approved September 10, 2018) was developed by AHRQ in 2019 in response to requests from ASCs interested in tracking their own survey results. Organizations submitting data receive a feedback report, as well as a report of the aggregated, de-identified findings of the other ASCs submitting data. These reports are used to assist ASC staff in their efforts to improve patient safety culture in their organizations. Rationale for the information collection. The ASC SOPS and the ASC SOPS Database support AHRQ’s goals of promoting improvements in the quality and safety of health care in ASCs. The survey, toolkit materials, and database results are all made publicly available on AHRQ’s website. Technical assistance is provided by AHRQ through its contractor at no charge to ASCs, to facilitate the use of these materials for ASC patient safety and quality improvement. This database will: 1. Present results from ASCs that voluntarily submit their data; 2. Provide data to ASCs to facilitate internal assessment and learning in the patient safety improvement process; and 3. Provide supplemental information to help ASCs identify their strengths and areas with potential for improvement in patient safety culture.

In 1976, the Department issued ERISA Procedure 76-1, the Procedure for ERISA Advisory Opinions (ERISA Procedure), in order to establish a public process for requesting guidance from the Employee Benefits Security Administration (EBSA) on the application of ERISA to particular circumstances. The ERISA Procedure sets forth specific administrative procedures for requesting either an advisory opinion or an information letter and describes the types of questions that may be submitted. As part of the ERISA Procedure, requesters are instructed to provide information to EBSA concerning the circumstances governing their request. Section 6 of ERISA Procedure 76-1 lists the information that must be supplied by the party requesting an advisory opinion. This information includes identifying information (name, type of plan, EIN Number, etc.), a detailed description of the act(s) or transaction(s) with respect to which an advisory opinion is being requested, a discussion of the issues presented by the act(s) or transaction(s), a statement of the party's views concerning the issues to be resolved and the legal basis for such views. The requesting party must also include copies of the relevant documents and may also request a conference with EBSA in the event that EBSA is considering issuing an adverse opinion.

The Government Paperwork Elimination Act (GPEA), Public Law 105-277, 44 U.S.C. 3504 note (1998) requires federal agencies to use and accept electronic signatures, where practicable, when conducting official business with the public. In addition, Congress passed the E-Government Act of 2002, Public Law 107-347, 44 U.S.C. §§ 101, 3501, 3541, and 3601 notes (2002), to promote the use of the Internet and emerging technologies within and across Government agencies, provide citizen-centric Government information and services, reduce costs and burdens for businesses and other Government entities, promote access to high quality Government information and services across multiple channels, and transform agency operations by utilizing, where appropriate, best practices from public and private sector organizations. As part of its ongoing effort to comply with GPEA and the E-Government Act of 2002, USCIS is transforming its business processes. Abstract

The Veterans Child Care Assistance Program (VCAP) was established via Public Law 116-315, Section 5107a, Johnny Isakson and David P. Roe, M.D. Veterans Health Care and Benefits Improvement Act of 2020. Section 5107a provided permanent authority in 38 U.S.C. Sec1709C for VA to provide child care assistance to certain veterans receiving care at VA Medical Centers (VAMC) no later than January 2026. The Department of Veterans Affairs intends to collect information from veterans in order to establish eligibility and intent to utilize the VCAP. Information will include Veteran Name, Social Security Number, Address, Phone Numbers, Emergency Contact Information as well as various details on the child(ren) who will receive child care services. Currently, VA does not capture or maintain information on children of veterans, or those under their care. In order to protect the safety of the children and VA, appropriate information must be obtained for those being cared for on VA premises including emergency contact information for someone other than the veteran. VA facilities with on-site drop-in child care centers will be able to access the current VA systems to verify veteran name and SSN for eligibility but must maintain their contact information and the child’s information for use in the child care setting for emergency and safety precautions. This information will also be used for auditing and validation purposes in the reimbursement system for those veterans submitting claims for financial reimbursement of child care expenses. This is necessary to pay claims, as well as monitor and prevent fraudulent claims.

Abstract

SSA uses the information we obtain on Forms HA-55, HA-504, HA 504 OP1, HA-510, and HA-510-OP1 to manage the means by which we conduct hearings before an administrative law judge (judge) or administrative appeals judges (judge), and the scheduling of hearings with an judge. We use the HA-55, Objection to Appearing by Video Teleconferencing, and its accompanying cover letter, HA-L2, to allow claimants to opt-out of an appearance via video teleconferencing (VTC) for their hearing with a judge. The HA-L2 explains the good cause stipulation for opting out of VTC if the claimant misses the window to submit the HA-55, and for verifying a new residence address if the claimant moved since submitting their initial hearing request. SSA uses the HA-504 and HA-504-OP1, Acknowledgement of Receipt (Notice of Hearing), and accompanying cover letter, HA-L83, to: (1) acknowledge the claimants will appear for their hearing with a judge; (2) establish the time and place of the hearing; and (3) remind claimants to gather evidence in support of their claims. The only difference between the two versions of the HA-504 is the language used for the selection check boxes as determined by the type of appearance for the hearing (in person, phone teleconference, or VTC). In addition, the cover letter, HA-L83, explains: (1) the claimants’ need to notify SSA of their wish to object to the time and place set for the hearing; (2) the good cause stipulation for missing the deadline for objecting to the time and place of the hearing; and (3) how the claimants can submit, in writing, any additional evidence they would like the judge to consider, or any objections they have on their claims. The HA-510 and HA-510-OP1, Waiver of Written Notice of Hearing, allow the claimants to waive their right to receive the Notice of Hearing as specified in the HA-L83. We typically use these forms when there is a last minute available opening on an judge’s schedule, so the claimants can fill in the available time slot. If the claimants agree to fill the time slot, we ask them to waive their right to receive the Notice of Hearing. We use the HA-510-OP1 at the beginning of our process for representatives and claimants who wish to waive the 20 day (for amended or continued hearing notices) or 75 day (for all other hearing notices) requirement earlier in the process, and the HA-510 later in the process for those representatives and claimants who want the full 20 or 75 days before the scheduled hearing. The respondents are applicants for Social Security disability payments who request a hearing to appeal an unfavorable entitlement or eligibility determination or their representative payees. We are revising this Information Collection due to our Final Rule for Setting the Manner of Appearance of Parties and Witnesses at Hearings, RIN 0960-AI71.

The Impact Aid Discretionary Construction Program provides grants to eligible Impact Aid school districts to assist in addressing their school facility emergency and modernization needs. The eligible Impact Aid school districts have a limited ability to raise revenues for capital improvements because they have large areas of Federal land within their boundaries. As a result, these districts find it difficult to respond when their school facilities are in need of emergency repairs or modernization. Abstract

In this clearance submission (3137-0074), the Institute of Museum and Library Services (IMLS) is requesting approval from the Office of Management and Budget (OMB) for the collection of the Public Libraries Survey (PLS): Fiscal Year (FY) 2024, FY 2025, and FY 2026. This survey was first conducted in 1989 by the National Center for Education Statistics (NCES) to collect library statistics for all public libraries in the 50 states, the District of Columbia, and five territories (the Commonwealth of Puerto Rico, the United States Virgin Islands, Guam, American Samoa, and the Commonwealth of the Northern Mariana Islands). The Public Libraries Survey has been conducted by the Institute of Museum and Library Services under the clearance number 3137-0074. Pursuant to P.L. 107-279, the Public Libraries Survey collects annual descriptive data on the universe of public libraries in the United States, the District of Columbia, and the Outlying Areas. Information such as public service hours per year, circulation of library books, number of librarians, population of legal service area, expenditures for library collection, programs for children and young adults, staff salary data, and access to technology, etc., would be collected. Additional information: The FY 2020 PLS aims to capture Coronavirus-related practices from the entire universe of approximately 9,300 public libraries in the United States, the District of Columbia, and outlying areas. Compared to the FY 2020 survey, the FY 2021 PLS will include changes to 35 questions, as follows: • Addition of 16 questions and modification of six questions about the number of library programs and attendance at those programs with more granularity by targeted age group and by format • Addition of three questions about other physical collection items and circulation to capture availability and usage of non-traditional library materials (e.g., wireless hotspots, other technology, household items) • Addition of two questions and modification of two questions related to the number of public access computers, the number of public access computer user sessions, and the number of wireless internet sessions; these changes will collect indicators of the precision of related questions • Revision of one question and deletion of five questions related to library services during the COVID-19 pandemic

Abstract

The Patient Safety and Quality Improvement Act of 2005 (hereafter the Patient Safety Act), 42 U.S.C. 299b-21 to 299b-26, was enacted in response to growing concern about patient safety in the United States and the Institute of Medicine's 1999 report, To Err is Human: Building a Safer Health System. The goal of the statute is to improve patient safety by providing an incentive for health care providers to work voluntarily with experts in patient safety to reduce risks and hazards to the safety and quality of patient care. The Patient Safety Act signifies the Federal Government's commitment to fostering a culture of patient safety among health care providers; it offers a mechanism for creating an environment in which the causes of risks and hazards to patient safety can be thoroughly and honestly examined and discussed without fear of penalties and liabilities. It provides for the voluntary formation of Patient Safety Organizations (PSOs) that can collect, aggregate, and analyze confidential information reported voluntarily by health care providers. By analyzing substantial amounts of patient safety event information across multiple institutions, PSOs will be able to identify patterns of failures and propose measures to eliminate or reduce patient safety risks and hazards.
