Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

The VA Form 28-1902w, Information for Veteran Readiness and Employment Entitlement Determination is used by the Vocational Rehabilitation Counselor (VRC) with the Veteran during the comprehensive initial evaluation after the VA receives an application for Veteran or Service member Readiness and Employment benefits and has determined the Veteran is eligible to apply for Chapter 31 under Title 38 U.S.C. 3104(a) and 38 CFR § 21.50. Use of the VA Form 28-1902w will allow the VA counselor to use the form to collect the information during the initial evaluation. Abstract

The CDC's Paul Coverdell National Acute Stroke Program (PCNASP) seeks to improve quality of care for acute stroke patients through systematic approaches to quality improvement. Certificate of Waivers under 14 CFR Part 91.903

Key Information

Abstract

Title 14, Part 91 of the Code of Federal Regulations prescribes the rules governing the operation of aircraft within the United States. Included in this is the operation of unmanned aircraft systems (UAS), commonly known as drones, by both civil and public aircraft operators. 14 CFR § 91.903 allows for operators of aircraft to apply for a certificate of waiver authorizing the operator to deviate from the rules listed in § 91.905 if the proposed operation can be conducted safely. The FAA has developed a web portal to allow UAS operators to apply for Part 91 certificate of waivers requests only. Prior to granting a certificate of wavier from any of the regulations listed in § 91.905, the FAA must evaluate the proposed operation and determine if the proposed operation may be conducted safely. The information requested is the minimum information necessary for the FAA to make this decision. Users are also required to fill out an Access Request Form when first signing up to use the web portal. This form asks for contact information, the type of user, and the type of operations that the user will perform. The FAA asks for this information to ensure that only UAS operators who will require use of the site have access to it.

This request is for a new OMB approval to collect data necessary to support the following: Charter School Programs (CSP) Data Collection, Risk Assessment, and Monitoring. This collection will replace previously approved 1855-0016 collection in order to obtain a new OMB number that is associated with the sub-agency this collection has moved to following a reorganization. The collection of information is still necessary in order to perform data collection, data analysis, risk assessment, and grantee monitoring for the CSP. However, the number of respondents is expected to decrease over the life of this collection, as new grantees (from FY 2022 onward) use the Charter Online Management and Performance System (COMPS) for their reporting (OMB Control Numbers 1810-0781 through 1810-0787). This collection will remain in place for the older grantees that still have active CSP grants. This data collection is coordinated with the EDFacts Initiative (EDFacts) to reduce respondent burden and fully utilize data submitted by States and available to the U.S. Department of Education (ED) through the EDFacts database. Specifically, under the current data collection, ED collects CSP grant award information from grantees (State agencies, charter management organizations, and some schools) to create a new database of current CSP-funded charter schools and award amounts. Once complete, ED merges performance information extracted from the EDFacts database with the database of CSP-funded charter schools. Together, these data allow ED to monitor CSP grant performance and analyze data related to accountability for academic performance, financial integrity, and program effectiveness.

Children’s Bureau Regional Partnership Grants Final Report Outline

Key Information

Abstract

The Regional Partnership Grants (RPG) program funds regional partnerships to provide, through interagency collaboration and integration of programs, services and activities that are designed to increase the well-being of, improve permanency outcomes for, and enhance the safety of children who are in an out-of-home placement or are at risk of being placed in an out-of-home placement as a result of a parent’s or caretaker’s substance abuse. The partnerships implement a range of activities and interventions, including peer recovery coaching, family-centered and prevention-focused services, parenting and family strengthening programs, specialized services to fathers, and pregnant and postpartum women, medication assisted treatment, in-home parenting and child safety support for families, and related evidence-based practices. Final Reports are required as part of the Administration for Children and Families Children’s Bureau’s (CB) reporting requirements. Each project is to submit Semi Annual Progress Reports to the CB during the project period and then a Final Report at the end of the project period. The Final Report is to be submitted 120 days after the project period ends. The RPG Final Report Outline is intended to assist RPG grant recipients with the development and submission of the Final Report. The RPG program is in its 7th Round of funding. Providing a consistent clear template for the project’s final reports will assist the CB to ensure program monitoring and to build the evidence of effective programing and practice for the RPG sites and other CB efforts to support families impacted by substance use.

Key Information

Abstract

The data collected serves as the only source of data for charter and nonscheduled( on-demand) passenger data by Part 135 operator (air taxis). FAA request that on-demand operators voluntarily report the number of revenue passengers that boarded their flights annually by airport. FAA and contractor staff review the responses (either signed electronic copy or via paper) to ensure that the operator submitting data is valid and the response is complete. Valid submissions are incorporated into an FAA database which also contains enplanement data collected from certificated, commuters, and foreign air carriers. Enplanement data are consolidated into a report for each airport that lists each air carrier and their annual enplanements. This is used to determine whether an airport is eligible for Airport Improvement Program funds and calculates primary airport sponsor apportionment as specified by title 49 United Stated Code (U.S.C.), section 47114. The data collected serves as the only source of data for charter and nonscheduled passenger data by Part 135 operator (air taxis).

The NASA Office of Science, Technology, Engineering, and Math (STEM) Engagement (OSTEM) administers the agency's national stem engagement and education activities in support of the Space Act, including the performance assessment and evaluation of OSTEM projects, programs and NASA STEM engagement investments. Abstract

The Department of Labor's Employee Benefits Security Administration (EBSA) maintains a program designed to provide education and technical assistance to participants and beneficiaries as well as to employers, plan sponsors, and service providers related to their health and retirement plan benefits. EBSA assists participants in understanding their rights, responsibilities, and benefits under employee benefit law and intervenes informally on their behalf with the plan sponsor in order to assist them in obtaining the health and retirement benefits to which they may have been inappropriately denied, which can avert the necessity for a formal investigation or a civil action. EBSA maintains a toll-free telephone number through which inquirers can reach Benefits Advisors in ten Regional Offices. EBSA has also made a request for assistance form available on its website for those wishing to obtain assistance in this manner. Contact with EBSA is entirely voluntary. The collection of information is an intake form for assistance requests from the public. This information includes the plan type, broad categories of problem type, contact information for responsible parties, and a mechanism for the inquirer to attach relevant documents. Summary data from the existing intake form has also been used, in accordance with section 513 of ERISA, to respond to requests for information regarding employee benefit plans from members of Congress and governmental oversight entities, and to inform the policy formulation process.

Individual Fishing Quotas for Pacific Halibut and Sablefish in the Alaska Fisheries

Abstract

The National Marine Fisheries Service (NMFS), Alaska Regional Office (AKRO), requests extension and revision of this currently approved information. Minor editorial changes were made to the forms to improve clarity and consistency. One was revised to remove an unnecessary checkbox. Four were revised to collect additional information required by regulations, remove the need to submit a copy of the permit, and to no longer require a separate form for each transfer. The revisions do not change the burden hours or costs of these forms. One form that was inadvertently omitted has been added and does not significantly increase the burden or cost for this information collection. This information collection contains the forms used by IFQ Program participants to apply for, renew, or replace permits; transfer or lease IFQ and quota share (QS); determine compliance with IFQ Program requirements; and designate a beneficiary. Some are also used by participants in the Western Alaska Community Development Quota (CDQ) Program. Also included are annual reports and other collections submitted by methods that do not use a form. This information collection is necessary to manage the IFQ Program and is required to manage commercial halibut and sablefish fishing under the Magnuson-Stevens Act, the Halibut Act, and regulations at 50 CFR parts 300 and 679. NMFS uses the information to identify and authorize participants in the halibut and sablefish fisheries, to track and transfer quota share, to limit transfers to authorized participants, and to monitor quota share balances and harvest in these fisheries.
