What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

The final rule "Patient Protection and Affordable Care Act; Health Insurance Market Rules; Rate Review" implements sections 2701, 2702, and 2703 of the Public Health Service Act (PHS Act), as added and amended by the Affordable Care Act, and sections 1302(e) and 1312(c) of the Affordable Care Act. The rule directs that states submit to CMS certain information about state rating and risk pooling requirements for their individual and small group markets, as applicable. Specifically, states will inform CMS of age rating ratios that are narrower than 3:1 for adults; tobacco use rating ratios that are narrower than 1.5:1; a state-established uniform age curve; geographic rating areas; whether premiums in the small group market are required to be based on average enrollee amounts; and, in states with pure community rating, uniform family tier structures and corresponding multipliers. In addition, states that elect to merge their individual and small group market risk pools into a combined pool will notify CMS of such election. This information will allow CMS to determine whether state-specific rules apply or Federal default rules apply. It will also support the accuracy of the federal risk adjustment methodology.

Abstract

This paperwork burden directly supports the Department of Transportation Strategic Goal on Safety. Specifically, the goal is to promote the public health and safety by working toward the elimination of transportation related deaths, injuries, and property damage. The reporting and recordkeeping requirements of Federal Aviation Regulation (FAR) Part 91, General Operating and Flight Rules, are authorized by Part A of Subtitle VII of the Revised Title 49 United States Code. FAR Part 91 prescribes rules governing the operation of aircraft (other than moored balloons, kites, rockets and unmanned free balloons) within the United States. The reporting and recordkeeping requirements prescribed by various sections of FAR Part 91 are necessary for FAA to assure compliance with these provisions. While the number of respondents and total hourly burden has not changed since the previous submission, the wage rates used in questions 12 and 14 have been updated for 2024. This results in a larger monetary cost for both the public and the FAA.

Advance Designation of Representative Payee

Key Information

Abstract

On April 13, 2018, the President signed into law The Strengthening Protections for Social Security Beneficiaries Act of 2018, also known as Pub.L.115-165. Section 201 of the law allows SSA beneficiaries and applicants under Title II, Title VIII and Title XVI, of the Act to designate individuals to serve as a representative payee should the need arise in the future. Section 201(j)(2) of the Act provides the requirements for selecting a qualified representative payee. SSA only offers the option to advance designate to capable adults and emancipated minors. Beneficiaries who have an assigned representative payee, or have a representative application in process, cannot advance designate. Form SSA-4547, Advance Designation of Representative Payee, allows beneficiaries or applicants the option to designate individuals in order of priority, to serve as a representative. Beneficiaries or applicants can update or change the advance designee order of priority at any time. SSA uses the information on Form SSA-4547 to select a qualified representative payee in order of priority. If the selected representative payee is unable or unwilling to serve, or meet SSA requirements. SSA will select another representative payee to serve in the beneficiaries and applicants best interest. SSA will notify beneficiaries annually of the individuals they chose in advance to be their representative payee. The respondents are SSA beneficiaries and claimants who want to choose an advance designate representative. We are making minor revisions to the SSA-4547 and the Advance Designation of Representative Payee Internet (MySSA) screens to revise the Privacy Act Statement.

Abstract

The U.S. Army Corps of Engineers is required to regulate construction related projects in U.S waters, which is accomplished through the review of applications for permits to conduct this work. Information required on Engineer Form 4345 is necessary to evaluate applicants' requests for a Corps permit for certain activities under Section 10 of 33 USC 403, Section 404 of 33 USC 1344, and Section 103 of 33 USC 1413. Engineer Form 6082 is an application form specific to the information needed for nationwide permits (NWPs). The information on the ENG Form 6082 is used to determine whether the proposed activity potentially qualifies for NWP authorization and to provide information needed for the evaluation of the applicant’s proposed project. ENG Form 4345 is used for other forms of DA permits, but ENG Form 6082 was created specifically for nationwide permits and is used when NWP authorization is sought.

Abstract

SSA requires payees to submit a written report accounting for the use of money paid to Social Security or Supplemental Security Income (SSI) recipients, and to establish and maintain a dedicated account for these payments. SSA uses Form SSA-6233-BK to: (1) Ensure the payee uses the payments for the recipient’s current maintenance and personal needs; and (2) confirm the expenditures of funds from the dedicated account remain in compliance with the law. Although we mail a paper Form SSA-6233-BK to the payee, the report can also be completed through telephone contact and then be mailed to the payee for signature, or during a face-to-face interview. The respondents are payees for SSI and Social Security recipients. We are making non-substantive changes to revise language on Form SSA-6233-BK, we are also reducing the Average Burden Per Response (minutes) from 20 minutes to 15 minutes, and we are revising the Privacy Act Statement.

FERC-725F, Mandatory Reliability Standard for Nuclear Plant Interface Coordination

Abstract

Reliability Standard NUC-001-4 requires nuclear plant generator operators and entities that provide generation, transmission and distribution services relating to off-site power (entities defined as "transmission entities") to enter into interface agreements with nuclear plant generator operators that will govern certain communication, training, operational and planning elements for use in addressing generation and transmission system limits and nuclear licensing requirements. The Commission understands that most entities subject to this Reliability Standard already have such agreements in place. The responsible entities are also required to retain evidence that they executed such an agreement and incorporated its terms into systems planning and operations. Further, each nuclear plant generator operator and transmission entity must self-certify its compliance to the compliance monitor once every three years. The NUC-001 Reliability Standard requires the Nuclear Plant Generator Operators and Transmission Entities to which they interconnect to execute and implement interface agreements for coordinating operations to meet nuclear licensing requirements. These agreements must include NPIRs that incorporate NPIRs into their operating analyses of the BES and operate the Transmission system to comply with the NPIRs. The lack of these agreements or compliance to these agreements can bring about lack of coordination of operations between a nuclear plant and its transmission entities. This lack of communication between entities can lead to an unanticipated separation from the Bulk Power System (BPS) placing the BPS at risk and defeating the goal of the Commission as mandated by Title XII, Subtitle A, of EPACT 2005. Reliability Standard NUC-001-4 does not require responsible entities to file information with the Commission. Nor, with the exception of a three year self-certification of compliance, does the Reliability Standard require responsible entities to file information with the ERO or Regional Entities. However, the Reliability Standard does require responsible entities to develop and maintain certain information for a specified period of time, subject to inspection by the ERO or Regional Entities.

National Assessment of Educational Progress (NAEP) 2025 Long-Term Trend (LTT) Revision

Key Information

Abstract

The National Assessment of Educational Progress (NAEP), conducted by the National Center for Education Statistics (NCES), is a federally authorized survey of student achievement at grades 4, 8, and 12 in various subject areas, such as mathematics, reading, writing, science, U.S. history, civics, geography, economics, technology and engineering literacy (TEL), and the arts. The National Assessment of Educational Progress Authorization Act (Public Law 107-279 Title III, section 303) requires the assessment to collect data on specified student groups and characteristics, including information organized by race/ethnicity, gender, socio-economic status, disability, and limited English proficiency. It requires fair and accurate presentation of achievement data and permits the collection of background, noncognitive, or descriptive information that is related to academic achievement and aids in fair reporting of results. The intent of the law is to provide representative sample data on student achievement for the nation, the states, and subpopulations of students and to monitor progress over time. NAEP consists of two assessment programs: the NAEP long-term trend (LTT) assessment and the main NAEP assessment. The LTT assessments are given at the national level only and are administered to students at ages 9, 13, and 17 in a manner that is very different from that used for the main NAEP assessments. LTT reports mathematics and reading results that present trend data since the 1970s. In addition to the operational assessments, NAEP uses two other kinds of assessment activities: pilot assessments and special studies. Pilot assessments test items and procedures for future administrations of NAEP, while special studies (including the National Indian Education Study (NIES), the Middle School Transcript Study (MSTS), and the High School Transcript Study (HSTS)) are opportunities for NAEP to investigate particular aspects of the assessment without impacting the reporting of the NAEP results. This Amendment to the NAEP 2025 Long-Term Trend Clearance package updates several documents since the approval of the primary package for NAEP 2025 LTT (OMB# 1850-0928 v.32) in April 2024: 1) updated committee members in Appendix A, 2) updated sampling design memo in Appendix C, 3) final communication materials in Appendix D, 4) updated Assessment Management Screens (AMS) in Appendix F1, and 5) small updates to Parts A and B. These changes required no changes to approved respondent burden, but does require a small increase in the cost to the Federal Government.

In order to work continuously to ensure that our programs are effective and meet our customers’ needs, and consistent to the extent applicable with Executive Order 12862 and other Executive Order(s), the Institute of Museum and Library Services (IMLS) (hereafter “the Agency”) seeks to obtain OMB approval of a generic clearance to collect qualitative feedback on our service delivery. By “qualitative feedback,” we mean information that provides useful insights into perceptions and opinions but not statistical surveys yielding quantitative results that can be generalized to the population of study. This collection of information is necessary to enable the Agency to garner customer and stakeholder feedback in an efficient, timely manner, in accordance with our commitment to improving service delivery. Certification Procedures for Products and Parts

Key Information

Abstract

All responses to this collection of information are required to obtain or retain benefits per 14 CFR Part 21. Any identity may initiate a certification. This information is used to (1) collect the name of the party, corporation, or organization to whom the Type Certificate, Amended Type Certificate, Production Certificate, Supplemental Type Certificate, or Amended Supplemental Type Certificate will be issued. This is necessary because the same name will appear on the certificate exactly as it is entered in this form. Also, to obtain the signature of the certifying official which must be the holder or the person duly authorized to sign for the holder, company, or corporation. FAA Airworthiness safety inspectors, designated inspectors, engineers, and designated engineering representatives review the required data submittals to determine whether the aviation products and /or articles, or their manufacturing facilities, comply with the applicable regulatory requirements, and that the products and articles have no unsafe features. Those products and articles, and their facilities that do comply with the minimum regulatory requirements, are issued an appropriate approval or certificate. A probable unsafe condition could exist for the owner /operator of the product or article, and /or the public, if the certification procedures for products and articles required by Title 14 CFR Part 21 were not followed. The collection of the information referenced herein is based on the applicants need, not a set time or frequency. Information collected is required as needed, to be used by the Aircraft Certification Office Branch in the geographical area of the applicant in order to initiate the certification project and gather resources to work on the particular certification project.

Authorization Request Form and Certification /Letter of Medical Necessity for Compounded Drugs (OWCP-26)

Abstract

OWCP will require the claimant’s treating physician to complete a Form OWCP-26 online before payment will be made for a compounded drug. The physician will be required to specify the ingredients in the compounded drug, indicate whether the compounded drug and each of its ingredients are medically necessary, and explain why the claimant cannot use an FDA-approved drug instead of the compounded drug being prescribed. The form will permit the OWCP to more easily track the volume, type, and characteristics of compounded drugs prescribed for claimants. It will allow additional oversight of the pharmacy benefit—improving patient safety, decreasing cost for stakeholders, and decreasing the risk for fraud, waste, and abuse. Completed forms will be reviewed by a clinical pharmacist or physician before being reviewed for decision by the individual program staff.

Annual Report on Children in Foster Homes and Children Receiving Payments (ACF-4125)

Key Information

Abstract

The Elementary and Secondary Education Act of 1965 (ESEA), Section 1124 of Title I, as amended by P.L. 114-95, requires the Secretary of Health and Human Services (HHS) to determine the number of children aged five to seventeen, inclusive, that (1) are being supported in foster homes with public funds; or (2) are from families receiving assistance payments in excess of the current poverty income level for a family of four. The Administration for Children and Families (ACF) within HHS passes this information to the Secretary of Education for purposes of allocating grants authorized under this law. The statute requires that the formula to allocate these grants and distribute funds be based, in part, on October caseload data of the number of children in foster care or in families receiving payments from state programs funded under Title IV-A of the Social Security Act [Temporary Assistance for Needy Families (TANF)]. Both the TANF and foster care programs fall under the purview of ACF within HHS. The purpose of this annual data report is to provide annually updated data so that funds may be allocated in accordance with the ESEA.

Abstract

The information collection activity is necessary to determine the eligibility and quality of potential awardees for traineeships and mentorships in the National Institute of Diabetes and Digestive and Kidney Diseases’ Office of Minority Health Research Coordination programs. This will be a revision of an existing clearance submission that has a current expiration date of 08/31/2023. Since the last revision occurred there have been no major changes made to the program, but the following activities have occurred: For the Network of Minority Health Research Investigators (NMRI) survey an internal data report was created but no changes were proposed based on its findings; For the NMRI Evaluation form this was split into three documents, one for each day of workshop evaluation, but the questions remain the same from the last revision; For the National Medical Association (NMA) and National Hispanic Medical Association (NHMA) feedback forms some participant suggestions are incorporated into the NMA Career development workshop.

Generic Clearance for Program Progress Performance Reports

Key Information

Abstract

This is a request for a new generic clearance as a means of promoting efficiency and innovative solutions. DOC provides grants that promote the economic and social well-being of individuals, small businesses, and communities with partnerships, funding, guidance, training, and technical assistance. DOC grant funding is intended to expand access to capital, promote economic resiliency, create new jobs, and increase economic opportunity. The Department of Commerce Bureaus manage grant programs that vary in purpose, target population, and activities. Therefore, a critical need for program offices to customize performance measurements has been identified, which led to the request for this umbrella generic program specific Progress Performance Reports (PPR).
