Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Key Information

Abstract

Section 7331 of title 38, United States Code (U.S.C.), requires, in relevant part, that the Secretary of Veterans Affairs, upon the recommendation of the Under Secretary for Health, prescribe regulations to ensure, to the maximum extent practicable, that all VA patient care be carried out only with the full and informed consent of the patient, or in appropriate cases, a representative thereof. Based on VA’s interpretation of this statute and our mandate in 38 U.S.C. 7301(b) to provide a complete medical and hospital service, we recognize that patients with decision-making capacity have the right to state their treatment preferences in a VA or other valid advance directive. The VA advance directive form (both English and Spanish language versions) is VA Form 10-0137. These forms have a current OMB Paperwork Reduction Act (PRA) clearance under OMB Control Number 2900-0556. In addition, 2900-0556 includes the collection of a “Close Personal Friend Statement” for incapacitated Veterans who have not completed an Advance Directive and are in need of health care. When a Veteran is incapacitated and does not have an Advance Directive, the VA regulations allow a statement to be submitted from a “Close Personal Friend” who will be responsible for making health care decisions on behalf of the Veteran. It is estimated that 300 such statements will be collected annually.

Pipeline Safety: Integrity Management Program for Gas Distribution Pipelines

Abstract

The Federal Pipeline Safety Regulations require operators of gas distribution pipelines to develop and implement integrity management (IM) programs. The purpose of these programs is to enhance safety by identifying and reducing pipeline integrity risks. This mandatory record keeping requirement supports the U.S. Department of Transportation’s SAFETY GOAL, which targets three main strategic initiatives: managing risk and integrity, sharing responsibility, and providing effective stewardship. This goal enhances public health and safety by working toward the elimination of transportation-related deaths and injuries. PHMSA requires operators to maintain records demonstrating compliance with this information collection for 10 years. PHMSA uses the information to evaluate the overall effectiveness of gas distribution Integrity Management requirements.

Abstract

The Institute of Education Sciences within the U.S. Department of Education requests an extension of the Comprehensive Literacy Program Evaluation: Comprehensive Literacy State Development Grant Program (1850-0945, approved on February 26, 2021). The extension is to complete the collection of state administrative data. The extension is needed because the state administrative data for the 2022-23 school year will not be ready to collect prior to the February 29, 2024 expiration date in all 13 CLSD grantee states. No material change in the collection instrument, instructions, frequency of collection, or use of information is being requested. The Comprehensive Literacy State Development (CLSD) Program Evaluation was mandated by Congress. The CLSD evaluation includes an examination of implementation, a randomized trial to estimate the impact of CLSD funding on teacher and student outcomes, and a longitudinal comparison of trends in achievement in CLSD and similar, non-CLSD schools. With the exception of the state administrative data collection--for which this extension is being requested--all other data collection for the study previously approved (including state interviews, district, school leader, and teacher surveys) have been completed.

Abstract

The OCC's codified guidelines (12 CFR part 30, appendix D) establish minimum standards for the design and implementation of a risk governance framework for insured national banks, insured Federal savings associations, and insured Federal branches of a foreign bank (bank). The guidelines apply to a bank with average total consolidated assets: (i) equal to or greater than $50 billion; (ii) less than $50 billion if that institution's parent company controls at least one insured national bank or insured Federal savings association that has average total consolidated assets of $50 billion or greater; or (iii) less than $50 billion, if the OCC determines such bank's operations are highly complex or otherwise present a heightened risk as to warrant the application of the guidelines (covered banks). The guidelines also establish minimum standards for a board of directors in overseeing the framework's design and implementation. The OCC is now seeking to renew the information collection associated with these guidelines.

This collection is necessary for NMFS to manage and administer the charter halibut fishery under the Charter Halibut Limited Access Program and the Pacific Halibut Catch Sharing Plan. This collection is an essential part of the sustainable management of the Pacific halibut fishery off Alaska, and is an integral element of ensuring regulatory compliance in the charter halibut fishing sector. With this extension, a form for the manual (non-electronic) guided angler fish (GAF) landing report has been created. Previously there was no form for a permit holder to use if they could not submit this report electronically. This form will improve the efficiency of submitting non-electronic GAF landing information. Minor editorial changes have been made to existing forms to increase clarity and consistency with other NMFS Alaska Regional Office forms. The Application for Annual Registration of Charter Halibut Permit has been updated to show that it can be submitted online through eFISH. This supporting statement also incorporates the 2021 change request that removed the notary certification requirement for the Application for Transfer of Charter Halibut Permit.

Key Information

Abstract

The federal government provides states with funding to offer financial assistance to low-income families to pay for childcare. This funding is provided through the Child Care and Development Fund (CCDF) childcare subsidy program. Subsidy payment rates are determined by states, but payment rates are required to be informed by childcare market prices. States are encouraged, but not required, to set payment rates at the 75th percentile of the market price. To obtain market prices, states are required to conduct a Market Rate Survey (MRS) no more than two years prior to the submission of states’ Child Care and Development Fund (CCDF) Plan. The CCDF Plan serves as the mandatory state application to receive Child Care and Development Block Grant (CCDBG) funds. The National Database of Childcare Prices fills an important gap in data and was first approved for collection by the Office of Management and Budget in August 2019 and made available to the public in September 2022.

Abstract

The Centers for Medicare & Medicaid Services (CMS) awarded 31 grants to States to participate in the Money Follows the Person Rebalancing Demonstration (MFP) from January 1, 2007 through September 30, 2011. This demonstration, created by section 6071 of the Deficit Reduction Act of 2005 (P.L. 109-171), supports State efforts to "rebalance" their long-term support systems by offering $1.75 billion over 5 years in competitive grants to States. Specifically, the demonstration will support State efforts to: a) Rebalance their long-term support system so that individuals have a choice of where they live and receive services; b) Transition individuals from institutions who want to live in the community; and c) Promote a strategic approach to implement a system that provides person centered, appropriate, needs based, quality of care and quality of life services and a quality management strategy that ensures the provision of, and improvement of such services in both home and community-based settings and institutions. The demonstration provides enhanced federal medical assistance percentage (FMAP) for 12 months for qualified home and community-based services for each person transitioned from an institution to the community during the demonstration period.

NRC Form 748, National Source Tracking Transaction Report

Key Information

Abstract

In 2006, the NRC amended its regulations to implement a National Source Tracking System (NSTS) for certain sealed sources. The amendments require licensees to report certain transactions involving nationally tracked sources to the NSTS. These transactions include manufacture, transfer, receipt, disassembly, or disposal of the nationally tracked source. This information collection is mandatory and is used to populate the NSTS. National source tracking is part of a comprehensive radioactive source control program for radioactive materials of greatest concern. The NRC and Agreement States use the information provided by licensees in the NSTS to track the life cycle of the nationally tracked source from manufacture until disposal. NSTS enhances the ability of NRC and Agreement States to conduct inspections and investigations, communicate information to other government agencies, and verify legitimate ownership and use of nationally tracked sources.

Key Information

Abstract

The CureTB program works to prevent the spread of tuberculosis (TB) among people who cross international borders. Information will be collected from the referring entities, which are state and local health departments and federal immigration and detention agencies. Whenever the referring entities provide clinical services to an individual with TB who has imminent plans to relocate, and an individual needs continuity of care in their new location, CDC CureTB is contacted to assist with coordinating that care.
