Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Key Information

Abstract

SSA uses Forms SSA-4814 and SSA-4815 to collect the information necessary to determine if an individual with human immunodeficiency virus (HIV) infection, who is applying for SSI disability payments, meets the requirements for PD. SSA mails the appropriate paper form to the claimant's medical source to complete and return to SSA. Respondents can complete this collection via the paper forms or telephone interview. When respondents complete these forms via telephone interview, which per our Management Information data, is most often, FO or DDS personnel enter the information from the respondents directly into the form, and document the medical source’s name, title, and date in the signature block. We then fax the form into the electronic case file. If SSA FO staff is unable to make a PD finding based on the information the applicant’s medical source provides, the DDS agency makes a determination at their discretion. The respondents are the medical sources of the applicants for SSI disability payments. We are updating all the fields where we request date on Form SSA-4814 and SSA-4815. We are adding an arrow on the side where the respondent can pull up a calendar and select the date (the respondent can still type in the date in any format they choose). We are also revising the date format from December 2, 2015 to 12/2/2015.

Key Information

Abstract

The U.S. Fish and Wildlife Service (Service or we) is changing to the permanent subsistence migratory bird harvest regulations in Alaska in response to recurrent requests from Alaska indigenous representatives to use inedible parts (mostly feathers) from birds taken for food during the subsistence hunt in handicrafts for sale. Federal regulations currently state that people may not sell, offer for sale, purchase, or offer to purchase migratory birds, their parts, or their egg(s) taken under the Alaska migratory bird subsistence harvest regulations (50 CFR part 92.6). These regulations would enable Alaska indigenous people eligible for the spring-summer subsistence harvest of migratory birds in Alaska to sell in a limited manner authentic native handicrafts or clothing containing inedible parts from migratory birds taken for food during the subsistence harvest season. The regulations were developed by a committee of the Alaska Migratory Bird Co-Management Council (AMBCC) including the Service, the Alaska Department of Fish and Game, and AMBCC native representatives from the Yukon-Kuskokwim Delta, Bering Strait-Norton Sound, North Slope, Kodiak Archipelago, Bristol Bay, Gulf of Alaska-Cook Inlet, Aleutian-Pribilof Islands, and Northwest Arctic regions. We propose to add a provision to 50 CFR part 92.6 to allow sale of handicrafts that contain the inedible parts of birds taken for food during the Alaska spring-summer migratory bird subsistence harvest. To limit sales of handcrafts including parts of migratory birds, we propose to add definitions for (a) “authorized migratory birds;” (b) “authentic native article of handicraft or clothing;” and (c) “sales by consignment.” This information collection is a recordkeeping requirement. This proposed rule requires that a certification (FWS Form 3-2484) or a Silver Hand insignia accompany each Alaska Native article of handicraft or clothing that contains inedible migratory bird parts. It also requires that all consignees, sellers, and purchasers retain this documentation with each item and produce it upon the request of a Law Enforcement Officer.

Environmental Education Local Grants Progress Report Form

Key Information

Abstract

EPA proposes to collect information from this programs grant recipients. Specifically, EPA proposes to have all EE grantees use the progress report form, detailed in the Supporting Statement, when drafting their Quarterly Progress Reports and Final Reports. By requiring all EE Grantees to use the EE Local Grant Progress Report Form, EPA's Office of Environmental Education will be equipped to gather data on this grant programs outputs, outcomes, the total number of individuals reached, and the total number of under served communities reached. This information will help EPA ensure projects are on schedule to meet their goals and produce high quality environmental outputs. Recordkeeping to Mitigate the Spread of COVID-19 in Head Start

Key Information

Abstract

The Administration for Children and Families (ACF) published an Interim Final Rule with comment (IFC) to establish new program standards, including recordkeeping requirements, to mitigate the spread of the coronavirus disease 2019 (COVID-19) in Head Start programs. ACF requested from Office of Management and Budget (OMB) emergency review and approval of the information collection requirements, as authorized under 44 U.S.C. 3507 (subsection j), which allowed for 6 months of approval under the Paperwork Reduction Act (PRA). This was followed by a full request, including two public comment periods, to extend approval of the recordkeeping requirements without changes. The OMB Control Number 0970-0583 for this information collection request (ICR) was established as a result. ACF issued two Final Rules following the IFC – one on January 6, 2023, and one on June 26 2023. • The final rule issued on January 6, 2023, added a new recordkeeping requirement that programs update their program policies and procedures to include an evidence-based COVID-19 mitigation policy to continue to promote the safety of Head Start program staff, volunteers, and enrollees and mitigate the spread of COVID-19. ACF requested another emergency review and received approval for the new recordkeeping requirement by OMB as authorized under 44 U.S.C. 3507 (subsection j). This was necessary to ensure programs were able to meet the requirements by the effective date of the final rule, 60 days following publication. • The final rule published on June 26, 2023, removed all previously established vaccination recordkeeping requirements and the requirement on maintaining a testing protocol. The only remaining recordkeeping requirement was the one established in the final rule issued on January 6, 2023. This information collection only contains the remaining recordkeeping requirement that is currently applicable to Head Start programs following the IFCs and Final Rules described above. This remaining recordkeeping requirement is essential to the mission of the agency to promote the safety of Head Start program participants.

Abstract

The Office of Research Integrity is requesting an extension on a currently approved collection. The purpose of the Institutional Assurance and Annual Report on Possible Research Misconduct form PHS–6349 is to provide data on the amount of research misconduct activity occurring in institutions conducting PHS-supported research. The purpose of the Assurance of Compliance by Sub-Award Recipients forms PHS–6315 is to establish an assurance of compliance for a sub-awardee institution. Forms PHS 6349 and PHS–6315 are also used to provide an annual assurance that the institution has established and will follow administrative policies and procedures for responding to allegations of research misconduct that comply with the Public Health Service (PHS) Policies on Research Misconduct (42 CFR part 93). Research misconduct is defined as receipt of an allegation of research misconduct and/or the conduct of an inquiry and/or investigation into such allegations. These data enable the ORI to monitor institutional compliance with the PHS regulation. There were minor revisions made on forms PHS-6349 and PHS-6315. The revisions will not alter the data collection. Need and Proposed Use: The information is needed to fulfill section 493 of the Public Health Service Act (42 U.S.C. 289b), which requires assurances from institutions that apply for financial assistance under the Public Health Service Act for any project or program that involves the conduct of biomedical or behavioral research. In addition, the information is also required to fulfill the assurance and annual reporting requirements of 42 CFR part 93. ORI uses the information to monitor institutional compliance with the regulation. Lastly, the information may be used to respond to congressional requests for information to prevent misuse of Federal funds and to protect the public interest.

Abstract

Railroads are classified by size into groups of large, medium, and small carriers. The Board must determine the classification of each rail carrier as a Class I, Class II, or Class III railroad. Railroads with a Class I classification (large railroads) must file Annual Reports (Form R-1) under 49 U.S.C. 11145. See OMB Control Number 2140-0009 (Class I Railroad Annual Reports). All other railroads are required to compute their adjusted revenues using the “railroad revenue deflator formula,” and, if the computation derived from that formula indicates a change in a carrier’s classification, that carrier must submit a “Classification Index Survey Form” to the Board pursuant to 49 C.F.R. 1201 General Instructions 1-1 and 1241.15.

Abstract

The Clean Air Act requires that on-highway vehicles and motorcycles, and nonroad vehicles, engines and equipment imported into the U.S. either comply with applicable emission requirements or qualify for an applicable exemption or exclusion. The Compliance Division (CD) in the EPAs Office of Air and Radiation maintains and makes available instruments to importers to help facilitate importation of products at U.S. Borders. EPA Form 3520-1 is used by importers of on-highway vehicles and motorcycles, and EPA Form 3520-21 is used by importers of nonroad vehicles, engines and equipment. In addition, this ICR covers the burden of EPA Form 3520-8 which is used to request final importation clearance for Independent Commercial Importers (ICIs) of on-highway vehicles who are required to bring the on-highway vehicles into compliance and provide emissions test results.

Abstract

This ICR collects information for the Food and Drug Administration's Medical Product Safety Network (MedSun) program from a subset of medical device user facilities that constitutes a representative profile of user reports for device-related deaths and serious injuries related to medical devices. FDA relies on reports of product problems from consumers and health care professionals, as well as from device manufacturers, in order to understand if signals of product problems are emerging in the postmarket use environment. MedSun is a significant, and very successful, strategy used by FDA to understand problems with medical devices in use in hospitals and outpatient services associated with hospitals. The MedSun program is unique to FDA and provides FDA with the ability to have a dialogue with the clinical community, so they may work together to learn about, understand, and solve problems with the use of medical devices.

Generic Clearance for Grant Program Monitoring Activities

Key Information

Abstract

To carry out OMB Circular A-102 and 2 CFR Part 215.51 , SAMHSA must collect grant program information necessary to ensure compliance with Federal and programmatic requirements. The Generic Clearance for Grant Program Monitoring Activities allows SAMHSA to collect standardized information from its grant recipients necessary to perform agency program oversight activities such as monitoring progress on recipient activities and determining and responding to recipient’s training and technical assistance (T/TA) needs. SAMHSA currently manages grant programs that provide prevention, treatment, recovery support services, and T/TA for substance use treatment and mental health providers along the continuum of care including prevention, harm reduction, treatment, and recovery.
