Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Semi-Annual Progress Report for the Tribal Sexual Assault Services Program

Key Information

Abstract

Authorized by 34 U.S.C. § 12511(e), the Tribal Sexual Assault Services Program (TSASP) supports projects to create, maintain, and expand sustainable sexual assault services provided by Tribes, tribal organizations, and nonprofit tribal organizations within Indian country and Alaska Native villages. TSASP supported projects provide intervention, advocacy, accompaniment (e.g., accompanying victims to court, medical facilities, or police departments), support services, and related assistance for adult, youth, and child victims of sexual assault, non-offending family and household members of victims, and those collaterally affected by the sexual assault. This information collection will enable the Office on Violence Against Women would require semi-annual progress reports on grant-funded activities from all Tribal Sexual Assault Services Program grantees in order to meet its Congressionally-mandated reporting requirements as well as to monitor effectively the award of federal funds.

Abstract

The semi-annual progress report will be completed by all technical assistance providers receiving funds under the Office on Violence Against Women's (OVW)Technical Assistance Program. The primary purpose of the OVW Technical Assistance Program is to provide direct assistance to grantees and their subgrantees to enhance the success of local projects they are implementing with Violence Against Women Act grant funds. Key Information

Abstract

Title III of the Foundations for Evidence-Based Policymaking Act of 2018 (hereafter referred to as the Evidence Act) mandates that OMB establish a Standard Application Process (SAP) for requesting access to certain confidential data assets. Specifically, the Evidence Act requires OMB to establish a common application process through which agencies, the Congressional Budget Office, State, local, and Tribal governments, researchers, and other individuals, as appropriate, may apply for access to confidential data assets collected, accessed, or acquired by a statistical agency or unit. This new process will be implemented while maintaining stringent controls to protect confidentiality and privacy, as required by law. The fulfillment of NCSES’s data security requirements places a degree of burden on individuals, which is outlined below.

Abstract

Borrowers who have defaulted on their Direct Loan or FFEL Program loans may remove those loans from default through a process called rehabilitation. Loan rehabilitation requires the borrower to make 9 payments within 10 months. The payment amount is set according to one of two formulas. The second of the two formulas use the information that is collected in this form. The form makes it easier for borrowers to complete through simplified language, and easier for loan holders through a uniform, common format.

Abstract

The National Defense Authorization Act for Fiscal Year 2008, Public Law 110-181, was enacted on January 28, 2008. Section 1105 of P.L. 110-181 amended the Federal Employees' Compensation Act (FECA) creating a new section, 5 U.S.C. § 8102a effective upon enactment. This section establishes a FECA death gratuity benefit of up to $100,000 for eligible beneficiaries of federal employees and Non-Appropriated Fund Instrumentality employees who die from injuries incurred in connection with service with an Armed Force in a contingency operation which also permits agencies to authorize retroactive payment of the death gratuity for employees who died on or after October 7, 2001 in service with an Armed Force in the theater of operations of Operation Enduring Freedom and Operation Iraqi Freedom. It also allows federal employees to vary the order of precedence of beneficiaries or to name alternate beneficiaries and provides that the OWCP Forms CA-40, CA-41, and CA-42 are to be used to designate beneficiaries and initiate the payment process for death gratuity benefits. Form CA-40 is an optional form that requests the information necessary from the employee to accomplish this variance and to name alternate beneficiaries only if the employee wishes to do so. Form CA-41 provides the means for those named beneficiaries and possible recipients to file claims for those benefits and requests information from such claimants so that OWCP may determine their eligibility for payment. Further, the statute and regulations require agencies to notify OWCP immediately upon the death of a covered employee. CA-42 provides the means to accomplish this notification and requests information necessary to administer any claim for benefits resulting from such a death. Form CA-40 is an optional form that requests the information necessary from the employee to accomplish this variance and to name alternate beneficiaries only if the employee wishes to do so. Form CA-41 provides the means for those named beneficiaries and possible recipients to file claims for those benefits and requests information from such claimants so that OWCP may determine their eligibility for payment. Further, the statute and regulations require agencies to notify OWCP immediately upon the death of a covered employee. CA-42 provides the means to accomplish this notification and requests information necessary to administer any claim for benefits resulting from such a death.

Abstract

The primary function of the Electronic Funds Transfer Authorization Agreement (CMS 588) is to gather information from a provider/supplier to establish an electronic payment process. The legal authority to collect this information is found in Section 1815(a) of the Social Security Act. This section provides authority for the Secretary of Health and Human Services to pay providers/suppliers of Medicare services. Under 31 U.S.C. 3332(f)(1), all Federal payments, including Medicare payments to providers and suppliers, shall be made by electronic funds transfer. 31 U.S.C. 7701 (c) requires that any person or entity doing business with the Federal Government must provide their Tax Identification Number (TIN). Goal of the Provider/Supplier Enrollment Application Revisions The goal of evaluating and revising the CMS 588 agreement is to renew the data collection. Due to previous revisions (2006 and 2009), this form is user friendly and concise. Only two minor revisions for systems requirements will be made at this time, specifically adding a street address line for the location of the financial institution and adding an additional National Provider Identification (NPI) number collection field for those providers/suppliers who have more than one NPI.

Abstract

This information collection supports the National Aeronautics and Space Act of 1958, as amended. The NASA Astronaut Selection Office, in collaboration with the NASA Human Capital Management Office, recruits astronaut candidates for various USA Space Exploration programs. The applicant evaluation process includes the collection of references from individuals who have been directly associated with the applicant over the course of the applicant's career. The information is used to gain insight into the candidate's work ethics and professionalism. When evaluating an applicant for the Astronaut Candidate Program, it is important that the ASO have the benefit of qualitative and quantitative information and recommendations from persons who have been directly associated with the applicant over the course of their career. On average, over 8,000 applications are received for each Astronaut vacancy announcement (released via USA Jobs.). After a thorough application review process, which includes NASA Astronauts participating as subject matter experts, approximately 450 applicants are placed in the ‘highly qualified’ rating category. Only then is information obtained from external references identified by the applicant. Each applicant provides the ASO with a list of two (2) key references (current and most recent previous supervisor), totaling 900 responses for 300 burden hours (20 minutes per response). The reference pool includes previous employers/direct reporting managers, as well as co-workers. The information is collected electronically, however respondents have an option to submit references through the mail if they do not have access to a computer.

Abstract

The Bureau of Justice Statistics (BJS) requests clearance to conduct the Annual Survey of Jails in Indian Country (SJIC) for a three-year period under OMB Control Number 1121-0364. BJS started the SJIC in 1998 to track changes in the demographic characteristics of the Indian country jail population as well as changes in the size of jail population, jail capacity and crowding, and the flow of inmates moving into and out of Indian country jails. These statistics are part of BJS’s core corrections statistics, as they contribute fundamentally to BJS’s mission of describing movements of persons through the criminal justice system. Indian country jails are administered by tribal authorities or the Bureau of Indian Affairs, while the county and city jails covered by the Annual Survey of Jails (ASJ) are administered by local law enforcement authorities such as a sheriff or jail administrator. Together, the SJIC and ASJ produce annual national estimates on persons held in jails nationwide.

Domestic Abuse Victim Reporting Option Statement

Abstract

The information collected will be used for purposes of filing an official report for a victim of domestic abuse. When a restricted report is filed, the victim is able to receive advocacy and counseling services without a report being made to command or law enforcement. In cases of an unrestricted report, command and law enforcement will be notified, and the victim is eligible to receive advocacy and counseling services from the Family Advocacy Program. The information collected for the form in unrestricted report cases may be used to initiate an investigation, and subsequently make an incident status determination following the Incident Determination Committee procedures and processes outlined in DoD Manual 6400.01, Volume 3. Abstract

The Office of the National Coordinator for Health Information Technology is seeking the approval for a revision of a currently approved collection request titled the “United States Core Data for Interoperability New Data Element Submission Form.” The USCDI is a standardized set of health data classes and constituent data elements used to support nationwide, interoperable health information exchange. When published, the USCDI will become the required standard data elements set to which all health IT developers must conform to obtain ONC certification. The ONC certification is required for participation in some federal healthcare payment plans. To insure the USCDI remains current and reflects the needs of the health IT community, ONC established a predictable, transparent, and collaborative process to solicit broad stakeholder input to expand the USCDI. Anyone, including ONC staff, staff from other federal agencies, and other stakeholders may submit proposals for new data elements. ONC will evaluate each submission and provide feedback to the submitter. ONC will draft a new version of the USCDI based on these submissions and this draft will undergo review by ONC's federal advisory committee, the Health Information Technology Advisory Committee (HITAC), as well as by the general public. Upon approval by the National Coordinator for Health Information Technology, new data classes and data elements from these submissions will be added to the newest version of the USCDI standard for integration into health information technology products such as electronic health records. Based on feedback and policy changes since this data collection began in 2020, ONC proposes to make some revisions to this data collection instrument. These changes do not change the overall data collection burden on respondents. The information collected from this submission system is needed as it will comprise the sum total of the items ONC will evaluate for addition to the next version of the USCDI. The requested data will provide supporting documentation to justify addition of the data elements to the USCDI, and, if the documentation does justify addition to the USCDI, to one of several levels of candidate data elements for future development and consideration. The requested data and ONC's evaluation of the data will be publicly available for review at any time to provide transparency and predictability in the USCDI expansion process. It will contain information about the submitter to allow ONC to provide direct feedback to submitters on ONC's evaluation of such submission.

Abstract

This project seeks to answer several research questions specific to health outcomes for dialysis patients as measured by the Centers for Medicare & Medicaid Services (CMS) Quality Incentive Program (QIP). In addition to these QIP-specific research questions, CMS seeks to understand several contextual factors that have the potential to affect dialysis facility performance on the QIP To do so, the study team, using primary data collection, will answer the following research questions related to dialysis quality, access to care and health equity, and quality of life: 1) What aspects of patient dialysis care do patients report as a priority? 2) How, if at all, do dialysis facilities evaluate the quality of care they provide? 3) What strategies do providers and dialysis facilities use to improve access to care for underserved populations? 4) What do patients, providers, and stakeholder organizations believe contributes to high quality of life for patients with ESRD? Do perceptions vary by respondent type or respondent characteristics? 5) How do dialysis facilities measure patient satisfaction and quality of life? 6) How do dialysis providers and stakeholder organizations think quality of life for dialysis patients has changed over time? What was the impetus for that change? To answer these research questions, the study team will conduct in-depth interviews with individuals with ESRD, dialysis facility administrators, dialysis facility social workers, nephrologists, corporate representatives from dialysis organizations, representatives from patient advocacy organizations, and transplant center administrators. The findings from this data collection will allow CMS to understand the contextual factors that impact many of the QIP measures that dialysis facilities are held accountable for, thus making policy adjustments to account for these contextual factors.
