Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Key Information

Abstract

This information collection supports a web-based tracking network (the tracking network) to allow Federal, State, and Territorial regulatory and public health agencies to share safety information about animal food. Information is submitted to the tracking network by regulatory and public health agency employees with membership rights. The efficient exchange of safety information is necessary because it improves early identification and evaluation of a risk associated with an animal food product. We use the information to assist regulatory agencies to quickly identify and evaluate a risk and take whatever action is necessary to mitigate or eliminate exposure to the risk.

Sections 668.53, 668.54, 668.55, 668.56, 668.57, 668.59 and 668.61 contain information collection requirements (OMB control number 1845-0041). This subpart governs the verification and updating of information provided on the Free Application for Federal Student Aid (FAFSA) which is used to calculate an applicant’s Expected Family Contribution (EFC) for purposes of determining an applicant’s need for student financial assistance under Title IV of Higher Education Act of 1965, as amended (HEA). The collection of this documentation helps ensure that students (and parents in the case of PLUS loans) receive the correct amount of Title IV program assistance by providing accurate information to calculate an applicant’s expected family contribution. Rather, the information is provided to state authorities as a method of implementing the Crime Control Act of 1990.

Abstract

The U.S. Army Corps of Engineers (USACE) Galveston District, (SWG) seeks to conduct a survey of commercial shipping companies that use the Gulf Intracoastal Waterway (GIWW) to transport commodities along the Texas Coast. SWG will incorporate survey information into a General Investigation Feasibility Study Update of long term solutions to shoaling and allisions near the intersections of the GIWW and the Brazos and Colorado rivers that could result in a potential loss of the navigation pool at the flood gates and locks. As part of the study, we are surveying shippers that use the GIWW to help us better understand the potential economic effects of a long-term disruption in navigation through the area. Part of the study requires an examination of how shippers would respond if navigation crossing the Brazos River and Colorado River was restricted for an extended period, or if the flood gates and locks are widened. The survey will provide information to determine if tentative project costs would be justified by reducing risks of losing the navigation pool along the GIWW.

Rehabilitation Services Administration (RSA) Payback Information Management System

Abstract

P.L. 114-95, section 302 (b) of the Rehabilitation Act of 1973, as amended by the Workforce Innovation and Opportunity Act (WIOA), the Rehabilitation Services Administration (RSA) provides Long-Term Training grants to academic institutions to support scholarship assistance to students. Students who receive scholarships under this program are required to work within the public rehabilitation program, such as with a state vocational rehabilitation agency, or an agency or organization that has a service arrangement with a state vocational rehabilitation agency. The student is expected to work two years in such settings for every year of full-time scholarship support. The program regulations at 34 CFR 386.33-386.35 and 386.40-386.43 detail the payback provisions and the RSA scholars' requirements to comply with them. Section 302 (b)(2)(C) of the Act requires tracking of scholars’ employment status and location of former scholars supported under the grants in order to ensure that students are meeting the payback requirement. Scholars must provide requested information necessary to meet the exit certification requirements. In addition to meeting the requirement that all scholars be tracked, the information collected will provide performance data relevant to the rehabilitation fields and degrees pursued by RSA scholars, as well as the funds owed and the rehabilitation work completed by them. In summary, RSA is requesting a revision of the currently approved collection by adding a section titled State Vocational Rehabilitation (VR) Agency: RSA Job Board Posting, where RSA envisions State VR agencies may initiate posting of an open position on the RSA Job Board on a rolling basis as positions become available. This addition to the current collection will be a helpful resource to students who receive RSA support, graduate, and seek qualifying employment to fulfill their required service obligation. This collection package adds the participation of State VR agencies to the existing approved ICR. Job leads will be submitted electronically through the online RSA Payback Information Management System (PIMS). There is minimal estimated burden required to report job information using the PIMS system.

Abstract

The Defense Contract Audit Agency is mostly comprised of Auditors (85% of our total workforce). Due to our 10-12% attrition rate each year; the Human Capital team is responsible for hiring between 400-500 entry-level auditors. DCAA utilizes a single DOD Direct Hire Authority (DHA) for our GS-511 Auditors. This allows us to recruit on-campus, at any time, via a multiple of methods (college career fairs, our own hiring events, in-classroom briefings, accountant club engagements, etc.). Human Resource Specialists partnered with a number of Audit-Recruiters are utilized for these outreach efforts which engage potential applicants. DCAA found a solution to the aforementioned problem: the acquisition of a Customer Management Relationship (CRM) tool. The CRM will enable DCAA to: Know how many candidates were engaged at each event, Know how many applied and ultimately how many were hired at DCAA (from that event), Focus resources on events with proven recruiting value based on data (vs this is where always gone to recruit), Focus our advertising dollars in a more direct manner, and Identify trends in successfully hired candidates enabling better strategies for finding more individuals like those hired. DCAA needs to collect the data so as to conduct marketing, advertising, outreach, and recruitment activities with potential leads and applicants for employment. DCAA needs to manage all tracking and communications associated with potential leads so as to retain potential applicants during and through the recruitment and hiring process. DCAA needs the data to continuously engage with potential leads who may not initially meet minimum qualification requirements; who may have applied/submitted a resume but were not selected; or who meet all requirements except having graduated from college. DCAA needs the data to conduct data analytics for outreach and recruitment return on investment assessments; to better refine outreach strategies, and to measure effectiveness of marketing and advertising efforts/campaigns.

Abstract

Healthcare Common Procedure Coding System (HCPCS) Level II Code Modification Application Form (CMS-10224) to allow CMS to collect HCPCS Level II Code Modification requests in an electronic application intake system on the CMS-10224 form. All information is received and distributed to CMS' HCPCS workgroup and is reviewed and discussed at monthly workgroup meetings. In turn, CMS' HCPCS workgroup reaches a decision as to whether a change should be made to codes in the HCPCS code set. The respondent who submits the application form can be anyone who has an interest in obtaining a code or modifying an exiting code. However, respondents are usually manufacturers of products, or consultants on behalf of the manufacturer. Applicants will be able to access the HCPCS Level II application, via the Medicare Electronic Application Request Information SystemTM (MEARISTM), on a designated website through CMS.gov. The electronic version of the HCPCS Level II application will be the same as the paper version except for a few minor non-substantive changes as described in question one above and in our crosswalk document. This secure online application maintained by CMS enables applicants to submit their responses to our application questions directly to CMS as opposed to downloading the application from CMS.gov, completing the application, and attaching the completed application and sending via email.

Abstract

The Department of Labor’s Veterans’ Employment and Training Service (VETS) administers funds for the Jobs for Veterans State Grant (JVSG) to each state, the District of Columbia, Puerto Rico, Guam, and the U.S. Virgin Islands on an annual fiscal year basis. These non-competitive, formula-driven grants are codified under Title 38, United States Code, (38 U.S.C.) Section 4102A(b)(5). This ICR collects the required information for the submission of JVSG State Plans and Modifications. The information covered includes the state's plan for furnishing employment, training, and placement services under 38 U.S.C. Chapter 41, including their performance goals for Disabled Veterans Outreach Program staff services to eligible veterans and other eligible persons.

National Survey on Drug Use and Health: Methodological Field Tests

Key Information

Abstract

Field test will be designed to examine the feasibility, quality, and efficiency of new prodedures or revisions to existing survey protocol. Specifically, the tests will measure the reliability and validity of certain questionnaire sections and items through multiple measurements on a set of respondents; assess new methods for gaining cooperation and participation of respondents with the goal of increasing response and decreasing potential bias in the survey estimates; and assess the impact of new sampling techniques and technologies on respondent behavior and reporting. Research will involve household survey interviews and cognitive interviewing.

Key Information

Abstract

Because it is very difficult to obtain a beneficiary's signature (or the signature of a person authorized to sign on behalf of the beneficiary) on a claim when the beneficiary is being transported by ambulance in emergency situations, we are proposing that, for emergency ambulance transport services, an ambulance provider or supplier may submit the claim without a beneficiary's signature, as long as certain documentation requirements are met.

Information Collection Request Ryan White HIV/AIDS Program (RWHAP) Parts A and B Unobligated Balances and Rebate Addendum Tables.

Key Information

Abstract

HRSA’s Ryan White HIV/AIDS Program (RWHAP) funds and coordinates with cities, states and territories, and local clinics/community-based organizations to deliver efficient and effective HIV care, treatment, and support to low-income people diagnosed with HIV. Grant recipients funded under Parts A and B of the RWHAP (codified under Title XXVI of the Public Health Service Act) are required to report financial data to HRSA annually in their Federal Financial Report (FFR SF-425). In addition to the FFR, the RWHAP Parts A and B grant recipients are required to identify and report the unobligated balance (UOB) by itemized subprogram/funding stream source (Formula, Minority AIDS Initiative [MAI], AIDS Drug Assistance Program [ADAP], etc.). As of April 22, 2021, the grant recipients must submit the subprogram breakdown of the UOB on their FFR in the Payment Management System (PMS). Grant recipients are also required to specify RWHAP Rebate Funding received in the fiscal year in the UOB table. HRSA uses the UOB and rebate addendum financial information to determine formula funding as directed by the RWHAP statute.
