Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Key Information

Abstract

This information collection is required to administer and operate this program in accordance with the Child Nutrition Act. The program is administered at the State agency level. Reports submitted and records maintained include agreements, financial reports, and the maintenance of accounting records for State Administrative Funds (SAE). The USDA Food and Nutrition Service administers the Child Nutrition Programs (CNP). In this revision, FNS is revising the FNS-525 form to eliminate data fields which collect data that FNS can obtain through its financial data systems. FNS is submitting a non-substantive change request in order to upload the correct version of FNS-525 into ROCIS. FNS discovered that the version of the form that was uploaded as part of the last renewal was not the current version of the form. FNS is taking this opportunity to upload the correct version into ROCIS.

Key Information

Abstract

CBP Form 7523, Entry and Manifest of Merchandise Free of Duty, Carrier’s Certificate of Release, is used by carriers and importers as a manifest for the entry of merchandise free of duty under certain conditions, such as when a shipment is valued at $2,500 or less. CBP Form 7523 is also used by carriers to show that articles being imported are to be released to the importer or consignee, and as an inward foreign manifest for vehicles or vessels, weighing less than five tons, arriving from Canada or Mexico, otherwise than by sea, with merchandise conditionally free of duty. CBP uses this form to authorize the entry of such merchandise. CBP Form 7523 is authorized by 19 USC 1433, 1484 and 1498. It is provided for by 19 CFR 123.4 and 19 CFR 143.23. Abstract

Through the National Program of Cancer Registries (NPCR), CDC supports central cancer registries in 45 states, the District of Columbia, Puerto Rico and the Pacific Island jurisdictions. NPCR-funded registries report information electronically to CDC twice a year. Information is used to monitor trends in cancer incidence and to plan and evaluate public health programs. Abstract

This request is for the continued clearance of data collection and reporting requirements to enable the U.S. Department of Housing and Urban Development (HUD) to better assess the effectiveness of discretionary-funded programs included in this information collection request (ICR). The discretionary-funded programs included in this ICR are the Multifamily Housing Service Coordinator Grant Program, the Multifamily Housing Budget-Based Service Coordinator Program, and the Resident Opportunity and Self Sufficiency Service Coordinator Grant Program (ROSS). This proposed collection, titled Standards for Success Reporting, has three key components that improve data collection and reporting for participating programs. First is the standardization of data collection and reporting requirements across programs which increases data comparability and utilization. Second is the ability to report on measurable outcomes and to align them with higher-level agency objectives. And third is the collection of record-level data, instead of aggregate data. Collecting de-identified data at the level of the service recipient allows for more meaningful analysis, improved management, and the ability to demonstrate the progress and achievements of the funding recipients and the programs. Standards for Success accepts data submission by direct data input through the HUD-funded GrantSolutions online data collection and reporting tool (OLDC) and by data file upload, accommodating file formats in Microsoft Excel or Extensible Markup Language (XML).

Key Information

Abstract

The Harbor Maintenance Fee (HMF) and Trust Fund is used for the operation and maintenance of certain U.S. channels and harbors by the Army Corps of Engineers. U.S. Customs and Border Protection (CBP) is required to collect the HMF from importers, domestic shippers, and passenger vessel operators using federal navigation projects. Commercial cargo loaded on or unloaded from a commercial vessel is subject to a port use fee of 0.125 percent of its value if the loading or unloading occurs at a port that has been designated by the Army Corps of Engineers. The HMF also applies to the total ticket value of embarking and disembarking passengers and on cargo admissions into a Foreign Trade Zone (FTZ). CBP Form 349, Harbor Maintenance Fee Quarterly Summary Report, and CBP Form 350, Harbor Maintenance Fee Amended Quarterly Summary Report are completed by domestic shippers, foreign trade zone applicants, and passenger vessel operators and submitted with payment to CBP. CBP uses the information collected on CBP Forms 349 and 350 to verify that the fee collected is timely and accurately submitted. These forms are authorized by the Water Resources Development Act of 1986 (26 U.S.C. 4461, et seq.) and provided for by 19 CFR 24.24, which also includes the list of designated ports. CBP Forms 349 and 350 are accessible at http://www.cbp.gov/newsroom/publications/forms or they may be completed and filed electronically at www.pay.gov.

This information collection is mandatory for any person or entity releasing a security conveyance. When an aircraft owner satisfies their debt, 14 CFR Part 49.17, “Conveyances recorded”, mandates that the security holder report the satisfaction of the security conveyance to the FAA Civil Aviation Registry (Aircraft Registry). When the Aircraft Registry records a security conveyance, an AC Form 8050-41 Notice of Recordation, is prepared by an Examiner to display the collateral, parties, and recording information. The form is mailed to the security holder. When the debt is satisfied the security holder completes Part II, Release, at the bottom of the AC Form 8050-41; a security holder signature and date is required to signify release of the debt. The security holder mails the form to the Aircraft Registry so that the interest in the aircraft shows to be released. The security holder may submit the same information without using the form, if desired. This form is not available for download since an FAA Examiner has to fill in the information.

If the individual capable of providing the information does not accompany the child claimant, we mail the SSA-783 to the individual for completion, or if the person has access to a computer, we will refer them to SSA's website. The Bald and Golden Eagle Protection Act (Eagle Act; 16 U.S.C. 668-668d) prohibits take of bald eagles and golden eagles except pursuant to Federal regulations. SSA is requesting to make revisions to the CBSV User Agreement. This collection instrument defines the conditions, terms, and safeguards under which SSA provides participating businesses with the verification of Social Security numbers. This action does not affect the public reporting burden. SSA developed CBSV in response to multiple requests from the business community. CBSV is a fee-for-service process which allows SSA to process high volumes of SSN verifications that companies need for business purposes. Since this is not required by law but is being provided as a service, businesses incur the costs of CBSV. The respondents are companies requesting SSN verifications from SSA as part of their business process.

The U.S. Consumer Product Safety Commission (CPSC or Commission) has promulgated a final rule that sets forth requirements for neck floats. 90 FR 58096 (Dec. 15, 2025). This rule contains information collection requirements that are subject to public comment and review by the Office of Management and Budget (OMB) under the Paperwork Reduction Act of 1995 (PRA; 44 U.S.C. 3501–3521).
