What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

The Administration for Community Living’s (ACL’s) Office of Healthcare Information and Counseling (OHIC) seeks to collect data for grantees to develop standardized grantee profiles for three programs managed by OHIC: State Health Insurance Assistance Program (SHIP), Senior Medicare Patrol (SMP), and Medicare Improvements for Patients and Providers Act (MIPPA). SHIP was authorized by the Omnibus Budget Reconciliation Act of 1990, SMP by the Omnibus Consolidated Appropriation Act of 1997, and MIPPA was passed in 2008. For grantees in each program, there is a specific set of reporting requirements and associated reporting schedules outlined in the OHIC Program Reporting Guidelines. These include monthly, semiannual, and final reports that are either general or financial reports. Although reporting requirements are effective in obtaining specific grantees’ data, there is neither consistency nor uniformity in how individual grantees submit the data elements beyond a high-level agency overview. Profiles, or standardized documents summarizing grantee efforts, have been used by ACL for monitoring purposes and information sharing between grantees. However, profiles were only developed for SHIP grantees and were last updated in 2016. The goal of this data collection effort is to obtain consistent data elements for SHIP, SMP, and MIPPA program grantees. Data will allow ACL to develop an updated set of grantee profiles that are accessible, visually appealing, and uniform across programs. The profiles will be internal to ACL and will only be shared with OHIC grantees.

Abstract

The Tom Lantos Block Burmese Jade Junta’s Anti-Democratic Efforts (JADE) Act of 2008, Public Law 110-286, renders certain individuals involved in specified Burmese organizations or activities ineligible for U.S. visas, including: leaders of the State Peace and Development Council (SPDC), the Burmese military, or the Union Solidarity Development Association (USDA); officials of the SPDC, the Burmese military, or the USDA involved in human rights violations and impeding democracy in Burma; and Burmese persons who provided substantial economic or political support to the SPDC, Burmese military, or USDA. Immediate family members of these individuals are also ineligible for United States visas. Department of State consular officers will use the information provided to evaluate and adjudicate the individual applicant’s eligibility for a visa consistent with these requirements.

Abstract

The purpose of the federal Traumatic Brain Injury (TBI) State Partnership Program is to create and strengthen a system of services and supports that maximizes the independence, well-being, and health of people with TBIs across the lifespan and all other demographics, their family members, and support networks. The TBI State Partnership Program funds the development and implementation of statewide systems that ensure access to TBI related services, including transitional services, rehabilitation, education and employment, and long-term community support. To best monitor, guide, and support TBI State Partnership Program grantees, ACL needs regular information about the grantees’ activities and outcomes. The simplest, least burdensome, and most useful way to accomplish this goal is to require grantees to submit information as part of their required semiannual reports via an electronic data submission instrument.

Annual Report on Progress - University Centers for Excellence in Developmental Disabilities Education, Research, and Service

Abstract

The National Network of University Centers for Excellence in Developmental Disabilities Education, Research, and Service (UCEDDs) is authorized under Part D of the Developmental Disabilities Assistance and Bill of Rights Act of 2000 (DD Act of 2000) The DD Act directs the Secretary of Health and Human Services to develop and implement a system of program accountability to monitor the grantees funded under the DD Act of 2000 [Section 104 (a) (42 USC 15004)], including the UCEDDs. Section 154 (e) (42 USC 15064) of the DD Act of 2000 includes requirements for a UCEDD Annual Report. Section 104 (a) (42 USC 15004) of the Developmental Disabilities Assistance and Bill of Rights Act of 2000 (DD Act) directs the Secretary of Health and Human Services to develop and implement a system of program accountability to monitor the grantees funded under the DD Act of 2000. The program accountability system must include UCEDDs authorized under Part D of the DD Act of 2000. Section 154 (e) (42 USC 15064) of the DD Act of 2000 includes requirements for a UCEDD Annual Report. The UCEDD Annual Report should contain information on progress made in achieving the projected goals of the Center for the previous year, including: (1) The extent to which the goals were achieved; (2) A description of the strategies that contributed to achieving the goals; (3) The extent goals were not achieved, a description of factors that impeded the achievement; and (4) An accounting of the manner in which funds paid to the Center under this subtitle for a fiscal year were expended. In addition, the DD Act requires information on proposed revisions to the goals; and a description of successful efforts to leverage funds, other than funds made available under the DD Act. The DD Act also states grantees must report on: (1) Consumer satisfaction with the advocacy, capacity building, and systemic change activities of the UCEDD; (2) The extent to which the UCEDD’s advocacy, capacity building, and systemic change activities resulted in improvements; and (3) The extent to which collaboration was achieved in the areas of advocacy, capacity building, and systemic change. Currently, UCEDDs engage in four broad tasks: conducting interdisciplinary training, promoting exemplary community service programs and providing technical assistance at all levels from local service delivery to community and state governments, conducting research, and disseminating information to the field. There are 67 UCEDDs throughout the United States with at least one or more in every State and Territory, as mandated. This is a revision of a currently approved information collection. While the IC remains the same and is consistent with currently approved data elements, the revisions are specifically designed to include a reporting mechanism to provide information to ACL on their use of funds to support people with intellectual and developmental disabilities through Centers for Disease Control (CDC) funds to expand vaccine access and the Public Health Workforce funds (PHWF) to strengthen the public health workforce.

U.S. Army Corps of Engineers (USACE) Navigation Improvement Surveys

Key Information

Abstract

USACE operates, maintains, and improves much of the nation’s navigation infrastructure. This includes inland navigation infrastructure and coastal infrastructure. USACE conducts periodic navigation improvement studies to ensure continuity of operations now and into the future. To fully evaluate these studies, USACE needs data on the use of the Nation’s waterways, the extent of navigation inefficiencies, and anticipated changes in vessel operations and sizes. This information is used in planning studies to formulate and evaluate the projected benefits and impacts of alternatives. Navigation improvement studies conducted by USACE typically use empirical data provided by the USACE Waterborne Commerce Statistics Center; however, the impacts on waterway traffic of alternative capital and operations and maintenance investment strategies collected by these surveys will complement the empirical data.

Abstract

The Children’s Bureau (CB) within the HHS Administration for Children and Families (ACF) seeks approval to continue to collect information for the Regional Partnership Grants to Increase the Well-being of and to Improve Permanency Outcomes for Children Affected by Substance Abuse (known as the Regional Partnership Grants Program or “RPG”) Cross-Site Evaluation and Evaluation-Related Technical Assistance project. The Child and Family Services Improvement and Innovation Act (Pub. L. 109-288) includes a targeted grants program (section 437(f) of the Social Security Act) that directs the Secretary of HHS to reserve a specified portion of the appropriation for these RPGs, to be used to improve the well-being of children affected by substance abuse. Under four prior rounds of RPG, CB has issued 91 grants to organizations such as child welfare or substance abuse treatment providers or family court systems to develop interagency collaborations and integration of programs, activities, and services designed to increase well-being, improve permanency, and enhance the safety of children who are in an out-of-home placement or at risk of being placed in out-of-home care as a result of a parent’s or caretaker’s substance abuse. In 2018 CB awarded 10 grants to a fifth cohort and in 2019 CB awarded 8 grants to a sixth cohort. The current information collection request (ICR) is for continued data collection activities associated with these 18 grantees. The first three cohorts were included in previous ICRs (OMB Control Numbers 0970-0353 and 0970-0444) and data collection for the fourth cohort were covered in the previous three-year clearance under this OMB number (0970-0527). The ongoing RPG cross-site evaluation will extend our understanding of the types of programs and services grantees provided to participants, how grantees leveraged their partnerships to coordinate services for children and families, and the outcomes for children and families enrolled in RPG programs. First, the cross-site evaluation will assess the coordination of partners’ service systems to better understand how partners’ collaborative effort affected the services offered to families. The cross-site evaluation will also focus on the partnership between the child welfare and SUD treatment agencies, to add to the research base about how these agencies can collaborate to address the needs of children and families affected by SUD. Second, the evaluation will describe the characteristics of participants served by RPG programs, the types of services provided to families, the dosage of each type of service received by families, and the level of participant engagement with the services provided. Finally, the evaluation will assess the outcomes of children and adults served through the RPG program, such as child behavioral problems, adult depressive symptoms, or adult substance use issues. This ICR is to continue the previously approved data collection activities that are currently ongoing and add a sustainability survey as a new data collection activity. Approval of this request will allow CB to complete data collection, as needed, through the sixth cohort of RPG grantees. These data collection activities include the following: (1) site visits with grantees, (2) a web-based survey about grantee partnerships, (3) a web-based survey about sustainability planning, (4) semiannual progress reports, (5) enrollment and services data provided by grantees, and (6) outcomes and impacts data provided by grantees.

Abstract

The Balanced Budget Act of 1997 created Section 1934 of the Social Security Act that established the Program for the All-Inclusive Care for the Elderly (PACE). The legislation established the PACE program as a Medicaid State plan option serving the frail and elderly in the home and community. Pursuant to the notice given in 64 FR 66271 (November 24, 199), if a State elects to offer PACE as an optional Medicaid benefit, it must complete a State Plan Amendment described as Enclosures 3, 4, 5, 6 and 7. The information, collected by CMS on a one-time basis, is used by CMS to affirm that the State elects to offer PACE as an optional State plan service and the specifications of eligibility, payment and enrollment for the program.

Abstract

Terms of Credit Card Plans Survey The Terms of Credit Card Plans (TCCP) Survey was implemented in February 1990 as required by Section 5 of the Fair Credit and Charge Card Disclosure Act (FCCCA) of 1988. Each respondent provides information about its credit card plan with the largest outstanding number of cards. The FCCCA required the Federal Reserve to collect this information semiannually from the largest 25 issuers of credit cards and at least 125 additional institutions in a manner that ensured both an equitable geographic distribution within the sample and representation of a wide spectrum of institutions. The Federal Reserve was further directed to make the credit card price information for each institution available to the public upon request and to report the information semiannually to Congress. Consumer Credit Card Agreement Quarterly Submission On July 21, 2011, the Bureau assumed responsibility under the CARD Act for the quarterly collecting of credit card agreements in a publicly available database. Pursuant to 12 CFR 1026.58 and 15 U.S.C. 1632(d)(2)–(3), collection is mandated for all issuers (subject to de minimis exceptions), and the scope of the mandate may require submission of multiple different agreements for each issuer. See 15 U.S.C. 1632(d)(2) (“Each creditor shall provide to the Bureau, in electronic format, the consumer credit card agreements that it publishes . . . .”) (emphasis added); id. at 1632(d)(3) (“The Bureau shall establish and maintain on its publicly available Internet site a central repository of the consumer credit card agreements received from creditors pursuant to this subsection, and such agreements shall be easily accessible and retrievable by the public.”) (emphasis added). College Credit Card Agreement Annual Submission This collection is made pursuant to the CARD Act requirement, codified at 12 CFR 1026.57(d); 15 U.S.C. 1637(r). Every year, the CARD Act requires the Bureau to collect agreements in place between credit card issuers and institutions of higher education, as well as certain summary data about the credit cards offered pursuant to these agreements. See 15 U.S.C. 1637(r)(2)(A) (“Each creditor shall submit an annual report to the Bureau containing the terms and conditions of all business, marketing, and promotional agreements and college affinity card agreements with an institution of higher education, or an alumni organization or foundation affiliated with or related to such institution, with respect to any college student credit card issued to a college student at such institution.”). The CARD Act requires the Bureau to submit to Congress, and to make available to the public, an annual report that lists information submitted to the Bureau concerning agreements between credit card issuers and institutions of higher education or certain affiliated organizations. See id. 1637(r)(3) (“The Bureau shall submit to the Congress, and make available to the public, an annual report . . . .”). Affiliated organizations include fraternities, sororities, alumni associations, or foundations affiliated with or related to an institution of higher education. The Bureau provides this centralized repository on its website and also submits to Congress and publishes an annual report each year based upon the submitted data.

Abstract

FDA seeks to engage in research to analyze and better understand perspectives on challenges and opportunities faced by outsourcing facilities as they attempt to supply safe, high-quality drug products needed by patients. The Agency currently lacks this information and cannot obtain it from other sources. An electronic survey will be administered to approximately 80 outsourcing facilities to gather information regarding the challenges an opportunities these entities face. The results of this research will be used by FDA to develop a comprehensive understanding of the outsourcing facility sector, its challenges, and opportunities for advancement. The information will be essential to help identify knowledge and information gaps, operational barriers, and views on interactions with FDA. The research results will inform FDA's future approaches to communication, education, training, and other engagement with outsourcing facilities to address challenges and support advancement of the industry in its ability to produce high-quality drug products that are safe for patients.

Abstract

The Service enters into agreements and partnerships with nonprofit groups to facilitate and formalize collaboration between the parties in support of mutual goals and objectives as authorized by: • The Fish and Wildlife Act of 1956 (16 U.S.C. 742a-742j); • The National Wildlife Refuge System Administration Act of 1966 (16 U.S.C. 668dd-ee), as amended; • The Refuge Recreation Act of 1962 (16 U.S.C. 460k et seq.), as amended; • The Fish and Wildlife Coordination Act of 1934 (16 U.S.C. 661-667e), as amended; and • The National Wildlife Refuge System Volunteer and Community Partnership Enhancement Act of 1998 (16 U.S.C. 742f), as amended. Since 2014, the Lenape National Wildlife Refuge Complex, in partnership with the New Jersey Chapters of the National Wild Turkey Foundation (NWTF), jointly administers mentored hunts on an annual basis. The mentored hunts occur on refuge property, and many of the mentors come from NWTF. The partnership provides would-be hunters with an opportunity to experience hunting by learning from Service staff and our partners through a mentored hunt program. The program not only provides the necessary hunting experience, but it also teaches the hunters about a variety of hunting topics, from ethics to shooting proficiency to wildlife ecology. The program provides a great introduction to the rules and regulations that govern access, which can be a little overwhelming to new hunters. The Service’s partnership with the NWTF helps new hunters to better understand access, rules, regulations, and setting up on public land. The program provides applicants with an opportunity to start a new family tradition, harvest their own food in a sustainable manner, or enjoy the outdoors in a new manner which is a safe, inclusive, and fun way to enjoy their public lands.

Abstract

The information collection is necessary to manage the general health status of patrons participating in Child and Youth Programs (CYP) and to register patrons for CYP. The information collected in the Health Assessment is used by USMC CYP and Inclusion Action Team (IAT) personnel to determine the general health status of patrons participating in CYP activities and, as required, the appropriate accommodations for the patron to ensure full enjoyment of the CYP services. Abstract

The Clean Water Act Section 305(b) reports contain information on whether waters assessed by a state meet the states water quality standards, and, when waters are impaired, the pollutants and potential sources affecting water quality. This information helps states and the public track progress in addressing water pollution. Section 303(d) of the Clean Water Act requires states to identify and rank waters that cannot meet water quality standards (WQS) following the implementation of technology-based controls. Under Section 303(d), states are also required to establish total maximum daily loads (TMDLs) for listed waters not meeting standards because of pollutant discharges. In developing the Section 303(d) lists, states are required to consider various sources of water quality related data and information, including the Section 305(b) state water quality reports. Section 106(e) requires that states annually update monitoring data and use it in their Section 305(b) report. Section 314(a) requires states to report on the condition of their publicly owned lakes within the Section 305(b) report.
