Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

On March 23, 2010, the President signed into law H.R. 3590, the Patient Protection and Affordable Care Act, Public Law 111-148, as amended by the Health Care and Education Reconciliation Act of 2010, Pub. L. 111-152, collectively referred to as "The Affordable Care Act." The Affordable Care Act expands access to health insurance coverage through improvements to the Medicaid and Children's Health Insurance (CHIP) programs, the establishment of Affordable Insurance Exchanges (Exchanges), and the assurance of coordination between Medicaid, CHIP, and Exchanges. Through the Small Business Health Options Program (SHOP), the new Exchanges will assist qualified small employers in facilitating the enrollment of their employees in Qualified Health Plans (QHPs) offered in the small group market. The Exchanges, which will offer these QHPs, will become operational on January 1, 2014. Employers will be able to begin applying for coverage through the SHOP October 1, 2013, for coverage beginning as early as January 1, 2014. The employer's application for the SHOP will be a single, streamlined form that will be used to determine employer eligibility for the SHOP. The submission seeks OMB approval of the data collection elements needed to support eligibility determinations for small businesses in the SHOP.

Abstract

The purpose of this collection is to require foreign governments of countries that import meat, poultry, or egg products into the United States to submit more information with the certificates they are already required to submit, and require import inspection establishments to maintain written Sanitation Standard Operating Procedures. Additionally, when FSIS inspected and passed product is exported and then returned to this country, the owner, broker, or agent of the product (the applicant) arranges for the product’s entry and notifies FSIS. Abstract

The information requested will be used to support entry of the name of a minor (an unmarried person under 18) into the Children's Passport Issuance Alert Program (CPIAP). CPIAP provides a mechanism for parents or other persons with legal custody of a minor to obtain information regarding whether the Department has received a passport application for the minor. This program was developed as a means to prevent international abduction of a minor or to help prevent other travel of a minor without the consent of a parent or legal guardian. If a minor's name and other identifying information has been entered into the CPIAP, when the Department receives an application for a new, replacement, or renewed passport for the minor, the application will be placed on hold for up to 60 days and the Office of Children's Issues will attempt to notify the requestor of receipt of the application. Form DS-3077 will be primarily submitted by a parent or legal guardian of a minor.

FERC-725N, Mandatory Reliability Standards: TPL Reliability Standards

Key Information

Abstract

FERC-725N: Reliability Standard TPL-007-4 requires owners and operators of the Bulk-Power System (BPS) to conduct initial and on-going vulnerability assessments of the potential impact of defined geomagnetic disturbance (GMD) events on BPS equipment and the BPS as a whole. The modifications in the proposed standard address the Commission’s directives in Order No. 851 related to requirements for Corrective Action Plans. Final Rule in Docket RM19-10. Pursuant to section 215(d)(2) of the Federal Power Act (FPA), the Commission approves Reliability Standard TPL-001-5 (Transmission System Planning Performance Requirements). The North American Electric Reliability Corporation (NERC), the Commission-certified Electric Reliability Organization (ERO), submitted Reliability Standard TPL-001-5 for Commission approval in response to directives in Order No. 786. As discussed in this final rule, we determine that Reliability Standard TPL-001-5 improves upon currently-effective Reliability Standard TPL-001-4 by addressing: (1) the study of single points of failure of protection systems; and (2) planned maintenance outages and stability analysis for spare equipment strategies. The improvements in Reliability Standard TPL-001-5 are responsive to the directives in Order No. 786 regarding planned maintenance outages and stability analysis for spare equipment strategies. Reliability Standard TPL-001-5 is responsive in that it requires each planning coordinator and transmission planner to perform an annual planning assessment of its portion of the bulk electric system considering a number of system conditions and contingencies with a risk-based approach. The improvements in Reliability Standard TPL-001-5 are also responsive to the concerns identified in Order No. 754 regarding the study of a single point of failure on protection systems. Reliability Standard TPL-001-5 contains revisions to the planning events (Category P5) and extreme events (Stability 2.a-h) identified in Table 1 (Steady State and Stability Performance Planning Events and Steady State and Stability Performance Extreme Events), as well as the associated footnote 13, to provide for a more comprehensive study of the potential impacts of protection system single points of failure. For more common scenarios (i.e., planning events), the planning entity must develop a corrective action plan if it determines through studies that its system would experience performance issues. For less common scenarios that could result in potentially severe impacts such as cascading (i.e., extreme events), the planning entity must conduct a comprehensive analysis to understand both the potential impacts on its system and the types of actions that could reduce or mitigate those impacts. Reliability Standard TPL-001-5 is also responsive to Order No. 786 by modifying the requirements for stability analysis to require an entity to assess the impact of the possible unavailability of long lead time equipment, consistent with the entity’s spare equipment strategy. Accordingly, pursuant to section 215(d)(2) of the FPA, the Commission approves Reliability Standard TPL-001-5 as just, reasonable, not unduly discriminatory or preferential, and in the public interest.

Abstract

Defense Logistics Agency, Disposition Services disposes of excess property received from the military services. The inventory changes daily and includes thousands of items from air conditioners to vehicles, clothing to computers, and more. DLA Disposition Services manages the disposal of hazardous property for DoD activities, maximizing the use of each item and minimizing environmental risks and costs through monitored compliant disposal. Part of that monitored compliant disposal is the Qualified Facility List where facilities are screened to ensure they are operating within the limits of the law. The information collected via the DLA Form 2507, “Application for Qualified Facility List (QFL),” will be used to validate hazardous waste (HW) disposal facilities around the world. Prior to the U.S. Government sending hazardous waste to a disposal facility, the facility must undergo a vetting process to ensure they are properly permitted, insured, and operating within local, state, and/or national regulations.

FERC-551, Reporting of Flow Volume and Capacity by Interstate Natural Gas Pipelines

Key Information

Abstract

The Commission has a statutory requirement to facilitate price transparency in markets for the sale or transportation of physical natural gas in interstate commerce, having due regard for the public interest, the integrity of those markets, fair competition, and the protection of consumers. FERC-551 uses the information provided by pipelines as part of its overall implementation of the statutory provisions of sections 23(a) (1) of the Natural Gas Act, 16 USC sections 717t-2(a)(1); section 220 of the Federal Power Act; Section 316 of EPAct 2005; section 23 to the Natural Gas Act; section 1281 of EPAct 2005. More specifically, the Commission uses the pipelines’ FERC-551 postings as part of fulfilling the transparency provisions of section 23 (a) (1) of the Natural Gas Act as mandated by Congress. FERC relies is part on section 23(a) (1) of the Natural Gas Act, for authority to collect this information. The data requirements for pipelines are listed in the Code of Federal Regulations (CFR) under 18 CFR Part 284.13(b), reporting requirements for interstate pipelines.: The Commission has directed the data requirements under FERC-551 are to be posted on interstate pipeline websites and not filed on formatted/printed forms.

The authority for collecting this information is contained in the Higher Education Act of 1965, as amended § 496 (HEA), and implementing regulations at 34 CFR § 602. The data is required in order for recognized accrediting agencies to demonstrate compliance with 34 CFR § 602. The Secretary will use these criteria in determining whether an accrediting agency is a reliable authority as to the quality of education or training provided by institutions of higher education they accredit. The data is required for State Agencies for the approval of Vocational Education to demonstrate compliance with 34 CFR § 603. The Secretary will use these criterial to determine whether a State Agency for the Approval of Vocational Education is a reliable authority as to the quality of education or training provided by the vocational institutions of higher education they accredit. The data is also required in order for State approval Agencies for Nurse Education to demonstrate compliance with the criteria and procedures for recognition of State Agencies for Approval of Nurse Education published in the January 16, 1969 Federal Register. The Secretary will use these criteria in determining whether a state agency is a reliable authority as to the quality of training offered by schools of nursing. In addition, and in accordance with 34 CFR §600.55, the Secretary is also required to collect information, review, and determine whether the accreditation standards used by foreign countries to accredit medical education programs are comparable to the standards used to accredit medical education programs in the U.S.

Abstract

This is a revision request for the previously approved ICR to continue collecting program monitoring and reporting information from jurisdictions (which include states, Washington, D.C., U.S. Territories, cities, and counties) currently funded under the Overdose Data to Action (CDC-RFA-CE19-1904) notice of funding opportunity (OD2A NOFO). Reporting requirements have been carefully designed to align with and support the specific goals and outcomes outlined in the OD2A NOFO. This revision request is to initiate data collection activities for two new Overdose Data to Action NOFOs, the Overdose Data to Action in States (OD2A-S) will collect program monitoring and reporting information from states and Washington, D.C. The Overdose Data to Action-Limiting Overdose through Collaborative Actions in Localities (OD2A-LOCAL) will collect program monitoring and reporting information from U.S. Territories, cities, and counties.
