What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

The NASA Ames Research Center, Human Systems Integration Division, manages voluntary safety reporting systems (VSRS) to collect and share safety information including, but not limited to, the NASA Aviation Safety Reporting System (ASRS) and the Confidential Close Call Reporting System (C3RS). Both systems are voluntary reporting systems for the reporting of safety incidents, events, or situations. Respondents include, but are not limited to, any participant involved in safety-critical domains such as aviation or railway operations including commercial and general aviation pilots, rotorcraft pilots, drone operators, air traffic controllers, flight attendants, ground crews, maintenance technicians, dispatchers, train engineers, conductors, and other members of the public. The collected safety data are used by NASA, Federal Aviation Administration (FAA), Federal Railroad Administration (FRA), and other organizations that are engaged in research and the promotion of safety. The data are used to (1) Identify deficiencies and discrepancies so that these can be remedied by appropriate authorities, (2) Support policy formulation and planning for improvements and, (3) Strengthen the foundation of human factors safety research.

Abstract

This request is for a revision to a previously approved overarching generic clearance. The title of the collection has been updated from ACF Mandatory Grant Programs to ACF Non-Discretionary Grant Programs. The proposed types of information collections and purpose and use of the collected information has not changed. This request includes the extension of multiple collections approved under this generic (GenICs) and requests updates to three of these GenICs through this revision process. The Child Care and Development Fund (CCDF) ACF-696 Financial Report for States and Territories and the CCDF ACF-696T Financial Report for Tribal Grantees have been updated to eliminate unused columns and subaccount codes as well as lines that were not funded or were funded once and will be closed. Additionally, the instructions for these forms were updated to reflect changes to the reports and current Office of Child Care and Office of Grants Management policies and program instruction to recipients. Abstract

This survey is intended to obtain taxpayer opinions and preferences regarding a taxpayer-funded, government-run service or a free service provided by the private sector as required by Section 70607 Task Force on the Replacement of Direct File which was enacted as part of the One Big Beautiful Bill Act. The feedback collected from taxpayers in this survey is a critical input to the Report and will shape the recommendations that are made to Congress.Emergency Justfication:The Department of the Treasury and the Internal Revenue Service (IRS) are requesting approval for OMB Control Number 1545-NEW, Form 15706, Free Online Tax Preparation Feedback Survey under emergency procedures in connection with provisions of sections 70607 of Public Law 119-21, 139 Stat. 72 (July 4, 2025), commonly known as the One Big Beautifull Bill Act. Section 70607 created a Task Force on the Replacement of Direct File that is directed to issue a report to Congress by October 2, 2025. One of the requirements of the task force is to collect taxpayer opinions and preferences regarding a taxpayer-funded, government-run service or a free service provided by the private sector. To meet this requirement, we will be inviting the public to participate in a brief, anonymous feedback survey. The survey will focus on obtaining taxpayer opinions and preferences about government and private sector free tax filing options. The IRS needs to publish the Free Online Tax Preparation Feedback Survey to obtain taxpayer opinions and preferences regarding a taxpayer-funded, government-run service or a free service provided by the private sector on the Replacement of Direct File. Under this provision The Department of the Treasury, who has delegated the responsibility to the IRS, is required to obtain this feedback from taxpayers for the report that is due to Congress on Oct. 2, 2025. The feedback collected from taxpayers in this survey is a critical input to the Report and will shape the recommendations that are made to Congress. Following normal Paperwork Reduction Act clearance procedures would not allowed IRS to meet the Statutory requirement as intended. Therefore, due to the extraordinary circumstances and statutory deadlines for implementing the sections 70607, the Treasury and IRS request emergency processing of this information collection request by September 12, 2025. Given the inability to seek public comment during such a short timeframe, IRS also respectfully requests a waiver from the requirement to publish a notice in the Federal Register seeking public comment during the period of Office of Management and Budget review.

Reemployment Services and Eligibility Assessment (RESEA) Evidence Building Portfolio Project

Key Information

Abstract

DOL funds the Reemployment Services and Eligibility Assessment (RESEA) Evidence Building Portfolio Project programs in 49 states, the District of Columbia, Puerto Rico, and the Virgin Islands. States and territories use these funds to address the reemployment services needs of Unemployment Insurance (UI) claimants and to prevent and detect UI improper payments. DOL’s Chief Evaluation Office contracted with Abt Global and its partners, the W.E. Upjohn Institute, the Behavioural Insights Team (BIT), American Institutes for Research (AIR), and the National Association of State Workforce Agencies to develop strategies to support the evidence requirements for the RESEA program as enacted in 2018 amendments to the Social Security Act that permanently authorized RESEA. This Evidence Building Portfolio Project will help DOL establish rigorous and informative evidence standards and support states in conducting evaluations to expand the evidence base on effective RESEA interventions. The evidence standards and new evidence will each help states develop and implement RESEA programs that more quickly and cost-effectively return more claimants from UI back to work. In 2018, Congress and the President enacted the Statute, which included a provision to permanently authorized the Reemployment Services and Eligibility Assessment (RESEA) program, which aims to help Unemployment Insurance (UI) claimants return to work more quickly. Building on a core set of services, each state designs and implements its own RESEA program model, drawing on rigorous evidence of effective strategies. The Statute describes the need for states to implement evidence-based strategies or to evaluate their current programs to build evidence. To support states’ ongoing RESEA program improvements and evidence building, the US Department of Labor’s Chief Evaluation Office (CEO), in collaboration with the Office of Unemployment Insurance (OUI) in the Employment and Training Administration (ETA), seeks to provide evaluation technical assistance and conduct evidence-building activities to support and complement states’ evaluations. Specifically, this Evidence Building Portfolio Project will continue to provide evaluation technical assistance and build on evidence review activities started under DOL’s previous RESEA Evidence Building and Implementation Study (2018-2023). The current project also includes two new studies aimed to support and supplement state-led RESEA evaluations: the RESEA Impact Study (comprising the Component Study and the Behavioral Study) and the RESEA Cost Study.

Abstract

CMS' regulatory authority to require data validation is described in 42 CFR 422.516(g) and 423.514(g). Abstract

Section 1848(q) of the Social Security Act, as amended by section 101 of the Medicare Access and CHIP Reauthorization Act of 2015, authorizes the establishment of a Merit-based Incentive Payment System (MIPS) for eligible clinicians. Beginning in CY 2017, eligible clinicians are required to collect and submit data on four performance categories to CMS (quality, cost, advancing care information and improvement activities). This program replaces and consolidates portions of the PQRS, Value-based Modifier, and the Medicare EHR Incentive Program. Domestic Finance Company Report of Consolidated Assets and Liabilities

Key Information

Abstract

The voluntary FR 2248 is collected monthly as of the last calendar day of the month from a stratified sample of finance companies. Each monthly report collects balance sheet data on major categories of consumer and business credit receivables and on major short-term liabilities. For quarter-end months (March, June, September, and December), additional asset and liability items are collected to provide a full balance sheet. A supplemental section collects data on securitized assets. Board staff may ask either quantitative or qualitative questions through the use of a special addendum section no more than twice per year. The data are used to construct universe estimates of finance company holdings, which are published in the monthly statistical releases Finance Companies (G.20) and Consumer Credit (G.19), and in the quarterly statistical release Financial Accounts of the United States (Z.1).

Abstract

Under Section 15B(a) of the Exchange Act (15 U.S.C. 78o-4(a)), municipal securities dealers which are banks, or separately identifiable departments or divisions of banks (collectively, “bank municipal securities dealers”) are required to be registered with the Commission in accordance with such rules as the Commission may prescribe as necessary or appropriate in the public interest or for the protection of investors. Under Section 15B(c) of the Exchange Act (15 U.S.C. 78o-4(c)), any registered municipal securities dealer may, upon such terms and conditions as the Commission may deem necessary in the public interest or for the protection of investors, withdraw from registration by filing a written notice of withdrawal with the Commission. Rule 15Ba2-1 (17 CFR 240.15Ba2-1) provides that an application for registration by a bank municipal securities dealer must be filed on Form MSD (17 CFR 249.1100). Rule 15Ba2-1 further provides that if the information contained in any application for registration on Form MSD, or in any amendment to such application, is or becomes inaccurate for any reason, the applicant must promptly file an amendment on Form MSD correcting such information. Rule 15Ba2-4 (17 CFR 240.15Ba2-4) permits the successor to a registered municipal securities dealer to assume immediate responsibility for the operation of the predecessor’s business provided certain requirements are met, including the filing of a successor application on Form MSD. Rule 15Bc3-1 (17 CFR 240.15Bc3-1) provides that bank municipal securities dealers that wish to withdraw from registration must file a notice of withdrawal from registration on Form MSDW (17 CFR 249.1110). Rule 15Bc3-1 further provides that, prior to filing a notice of withdrawal from registration on Form MSDW, a bank municipal securities dealer must amend Form MSD in accordance with Rule 15Ba2-1(b) to update any inaccurate information. Rule 15Ba2-5 (17 CFR 240.15Ba2-5) permits a duly appointed (or duly qualified) fiduciary to assume immediate responsibility for the operation of a registered municipal securities dealer’s business. Under the rule, the registration of a municipal securities dealer is deemed to be the registration of any fiduciary, appointed or qualified by order, judgment, or decree of a court of competent jurisdiction to continue the business of such municipal securities dealer, provided that such fiduciary files with the Commission, within 30 days after entering upon the performance of his duties, a statement setting forth as to such fiduciary substantially the same information required by Form MSD or Form BD (“Rule 15Ba2-5 Statement”). The Commission uses the information obtained from Form MSD, Form MSDW, and Rule 15Ba2-5 Statement filings to, among other things, determine whether bank municipal securities dealers, their Rule 15Ba2-4 successors, and their Rule 15Ba2-5 fiduciaries meet the standards for registration set forth in the Exchange Act, and whether it is in the public interest to permit a bank municipal securities dealer to withdraw its registration. Certain provisions of Rule 15Ba2-4 and Rule 15Bc3-1 apply to non-bank municipal securities dealers which make registration filings on the standard forms for brokers and dealers (Form BD instead of Form MSD, and Form BDW instead of Form MSDW). Burden estimates for such non-bank municipal securities dealers are accounted for in the Supporting Statements for Form BD (OMB Control No. 3235-0012) and Form BDW (OMB Control No. 3235-0018) and are not included herein.

Abstract

The Bank Holding Company Act of 1956 (BHC Act) and Board’s Regulation Y - Bank Holding Companies and Change in Bank Control (12 CFR 225) require a bank holding company (BHC) to seek the prior approval of the Board before purchasing or redeeming its equity securities in certain circumstances. Due to the limited information that a BHC must provide in connection with any such request, there is no required reporting form, and each request for prior approval is generally filed 30 days before the proposed stock purchase or redemption as a notification with the Reserve Bank that has direct supervisory responsibility for the requesting BHC. The Federal Reserve uses the information provided in the redemption notice to supervise BHCs.

Abstract

The Federal Reserve Act (FRA) requires a state member bank to seek prior approval of the Board before making an investment in bank premises or the securities of a corporation holding its bank premises in certain circumstances. The Board has implemented this requirement in its Regulation H - Membership of State Banking Institutions in the Federal Reserve System (12 CFR Part 208), which requires a state member bank seeking to make such an investment to provide prior notice to the appropriate Federal Reserve Bank. The Federal Reserve uses the information provided in the notice to determine whether to object to the proposed investment.
