Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

The HV-REACH project team received approval to conduct case studies in October 2024 with up to seven sites. Updates were approved in June 2025 to implement changes in response to priorities of the current administration. Currently, we have completed interviews with four sites and are in the process of recruiting the remaining three sites. Based on current data collection, we are requesting to increase the number of respondents for some instruments and including additional programs from specific sites. These changes are based on feedback from sites and for data quality.

Abstract

Section 372 of the Public Health Service Act requires that the Secretary of HHS, by awards, provide for the establishment and operation of the Organ Procurement and Transplantation Network (OPTN), which, under oversight of the Health Resources and Services Administration (HRSA), operates the U.S. organ procurement and transplantation system. The Secretary/HRSA may direct the collection of data in accordance with the regulatory authority in 42 CFR 121.11 of the OPTN Final Rule. HRSA, in alignment with the Paperwork Reduction Act of 1995, submits data elements for collection to OMB for official federal approval. HRSA is submitting this new data collection, separate from OMB No. 0915-0157, since it includes new forms developed in response to an HHS Secretarial Data Directive that are not in use by OPTN. HRSA believes that separating these data collections will minimize confusion, increase clarity among OPTN members and stakeholders, and enable more direct feedback on the new forms. Both data collections include time-sensitive, life-critical data on transplant candidates and potential organ donor patients, the organ matching process, histocompatibility results, organ labeling and packaging, as well as pre- and post-transplantation data on recipients and donors. The OPTN collects these specific data elements from transplant centers.

ACF’s Generic Clearance for Reviewer Recruitment Forms

Key Information

Abstract

This request by the Administration for Children and Families (ACF) is to extend approval of the generic clearance for Reviewer Recruitment Forms with no changes (note that burden estimates have been updated to reflect ongoing collections and estimated future burden, but the purpose and use of this generic have not changed). ACF may recruit reviewers for a variety of different activities and each program office within ACF has a slightly different needs for information about reviewer applicants for different activities. This overarching generic clearance allows ACF to request slightly different information from potential reviewers, yet the individual forms serve the same general function. The abbreviated clearance process of the generic clearance allows program offices to gather a suitable pool of candidates within the varied time periods available for reviewer recruitment. The forms submitted under this umbrella generic are and will be voluntary, low-burden and uncontroversial. Information will be collected electronically unless specified otherwise in an individual generic information collection (GenIC) request. These updates are in response to recent Presidential Actions related to diversity, equity, and inclusion (DEI), such as those covered under the EO Initial Recissions of Harmful Executive Orders and Actions.

These providers are also required to submit identifying information as well as information regarding their efforts to mitigate illegal robocalls to the FCC’s Robocall Mitigation Database.

The Federal Energy Regulatory Commission (FERC or Commission) requests that the Office of Management and Budget (OMB) review and renew the information collection requirements in FERC-725K under OMB Control No. 1902-0260. Regional Reliability Standard PRC-006-SERC-03 was developed to be consistent with the NERC Automatic Underfrequency Load Shedding (UFLS) Reliability Standard PRC-006-3. Regional Reliability Standard PRC-006-SERC-03 was designed to ensure that automatic UFLS protection schemes designed by planning coordinators and implemented by applicable distribution providers and transmission owners in the SERC Region are coordinated to effectively mitigate the consequences of an underfrequency event. The regional Reliability Standard PRC-006-SERC-02 added specificity not contained in the NERC UFLS Reliability Standard for UFLS schemes in the SERC Region. Regional Reliability Standard PRC-006-SERC-02 effectively mitigates (in conjunction with NERC Reliability Standard PRC-006-3) the consequences of an underfrequency event while accommodating differences in system transmission and distribution topology among SERC planning coordinators resulting from historical design criteria, makeup of load demands, and generation resources. Under the regional Reliability Standard, the information is used to ensure compliance with requirements associated with underfrequency load shedding plans. Without this information, it would be difficult to enforce compliance with the regional standard. A lack of compliance with this regional standard may lead to uncontrolled failure of the Interconnection.

Abstract

This request is to establish a new umbrella generic clearance for the Administration for Children and Families (ACF) to request information from potential participants at ACF gatherings, such as meetings or conferences. This may include large scale conferences, meetings for grantees or contractors, workshops, trainings, poster sessions, and other in-person and virtual gatherings for individuals with interest in ACF programs (clients, researchers, policymakers, etc.), among others. To ensure ACF has adequate information to plan these activities, the Agency must often collect information from potential participants such as basic contact information, preferences for attendance (mode, special requests, etc.), organizational affiliation, feedback about meeting content, etc. Additionally, some activities require ACF to have additional information to have the means to select the most appropriate participants for attendance according to the type or purpose of a given activity, or to group participants into the most appropriate category or activity during an event. This may include information about poster presentations, speaking panels, training courses, professional perspectives or experiences, etc. In addition, attendees may be asked to submit an application or abstract for prescreening to be selected for attendance. The planning for meetings is most often on a quick timeline and the standard timeline to comply with a full request under the Paperwork Reduction Act would inhibit the ability to collect information to inform these activities. Therefore, an umbrella generic is necessary to allow for quick turnaround requests for similar information collections related to these activities. As part of this generic, ACF requests OMB provide a response on individual generic information collections within 5 business days. If no response is received and the request meets the purposes, uses and scope outlined in this document, ACF may move forward with the proposed information collection.

Significant changes to the collection include: 1) Adding data collection forms for candidates listed in the OPTN organ transplant waiting list to the existing OMB-approved information collection. These forms allow a transplant center to add, change, or remove candidates from the OPTN waiting list after a transplant center completes the patient evaluation. These forms contain information that the OPTN electronic organ matching system uses to match potential organ recipients with available deceased donor organs. There are 83 new data collection forms: candidate listing registration forms of all organs, candidate status justification forms of all applicable organs, Model for End-Stage Liver Disease or Pediatric End-Stage Liver Disease score exception and extension forms, and other forms. The SHIPS study is a randomized controlled trial (RCT) designed to determine whether participation in Individual Placement and Support (IPS) improves the employment, income, health, and self-sufficiency of people who are recently homeless and living in supportive housing. The SHIPS study will mark the first study testing the effectiveness of implementing IPS in a supportive housing program. SSA hypothesizes that combining the two most successful evidence-based practices that separately address homelessness and supported employment will yield a single intervention that effectively addresses both. The intent of the SHIPS study is to measure the effectiveness of evidence-based IPS compared to the services provided by local WorkSource Centers broadly available to jobseekers in the Los Angeles area, The housing case managers will refer PATH clients interested in finding employment and will randomly assign participants to one of two groups: 1. IPS: The Individual Placement and Support (IPS) service team will offer a range of structured services customized to participants’ personal needs, preferences, and challenges related to disabilities and/or mental health conditions. IPS was specifically designed as a supported employment model for individuals with serious mental illness and includes standardized training and fidelity requirements. Components of IPS that differ from those offered by WorkSource Services include integrated treatment that incorporates vocational and mental health services; benefits planning; and focus on rapid job search without extensive training. 2. WorkSource Centers: Under PATH’s current housing model, housing case managers refer PATH clients who express interest in finding employment to local American Job Centers, known as WorkSource Centers in Los Angeles. The WorkSource Centers are operated by the City of Los Angeles Economic and Workforce Development Department, and follow an employment services model that varies by WorkSource Center, is not evidence-based or subject to fidelity monitoring and is not necessarily responsive to the individual needs of jobseekers with disabilities.

Rule 204-3 under the Investment Advisers Act of 1940 (17 CFR 275.204-3) requires SEC-registered investment advisers to deliver written disclosure statements ("brochures and brochure supplements") to clients and prospective clients containing specified information about the adviser's background, business practices, services, fees, and key supervised personnel. The SEC needs this information collection to ensure investors receive information necessary to make informed decisions about retaining or continuing to employ investment advisers, while the SEC uses the disclosed information in its enforcement, regulatory, and examination programs to monitor adviser compliance and protect investors. Abstract

Section 16 of the Shipping Act of 1984, 46 U.S.C. § 40103, authorizes the Federal Maritime Commission (“Commission”) to exempt by order or regulation “any class of agreements between persons subject to this [Act] or any specified activity of those persons from any requirement of this [Act] if the Commission finds that the exemption will not result in substantial reduction in competition or be detrimental to commerce. The Commission may attach conditions to any exemption and may, by order, revoke an exemption.” The Commission added 46 CFR 532 to exempt non-vessel-operating common carriers (NVOCCs) from the more stringent tariff rate publication requirements of the Shipping Act of 1984 and related provisions of the Commission’s regulations to permit them to enter into negotiated rate arrangements (NRAs) with shippers.76 FR 11351 (March 2, 2011). The Commission subsequently extended the tariff rate publication exemption contained in 46 CFR part 532 to foreign based unlicensed NVOCCs as well. 78 FR 42886 (July 10, 2013). The exemption is conditioned upon the NVOCC including a prominent notice invoking the exemption in its electronically published rules tariff and indicating their intention to the Commission. In addition, NVOCCs must maintain original NRAs and associated records for 5 years in a format easily produced to the Commission and furnish those records promptly upon request from the Commission. An NVOCC invoking the exemption must: Include its rates in a tariff open to public inspection (46 CFR 532.3(a)(1); Comply with 46 USC 40501(d) and (e) (46 CFR 532.3(b) and (c)); Comply with 46 USC 40503 (46 CFR 532.3(d); Adhere to the prohibitions in 46 USC 41104(a)(2)(A) (46 CFR 532.3(e); Include its rates in a tariff open for public inspection in an automated tariff system (46 CFR 532.3(f); Comply with 46 CFR 520.4(a)(4), 520.4(f), 520.6(e), 520.7(c) and (d), 520.8(a), 520.12, and 520.14 (46 CFR 523..3(g); Ensure their NRA meets the requirements of 46 CFR 532.5; Indicate their intention to the Commission and the public to invoke the exemption by a prominent notice in its rules tariff (46 CFR 532.6); Maintain original NRAs in a readily accessible and retrievable manner for 5 years from the completion date of performance of the NRA (46 CFR 532.7; and Promptly respond to requests by the Commission for those records (46 CFR 532.7).

Abstract

Abstract (2000 characters maximum) The Skilled Nursing Facility Value-Based Purchasing (SNF VBP) Program is authorized by section 1888(h) of the Social Security Act (the Act), and it applies to freestanding SNFs, SNFs affiliated with acute care facilities, and all non-Critical Access Hospital (CAH) swing-bed rural hospitals. Beginning with payment for services furnished on October 1, 2018, section 1888(h) of the Act requires the Secretary to reduce the adjusted Federal per diem rate determined under section 1888(e)(4)(G) of the Act otherwise applicable to a SNF for services furnished during a fiscal year (FY) by 2 percent, and to adjust the resulting rate for a SNF by the value-based incentive payment amount earned by the SNF based on the SNF’s Performance Score for that FY under the SNF VBP Program. Through the SNF VBP Program, incentive payments are awarded to SNFs to encourage improvements in the quality of care provided to Medicare beneficiaries. The Skilled Nursing Facility Quality Reporting Program (SNF QRP) is authorized by section 1888(e)(6) of the Act, and it applies to freestanding SNFs, SNFs affiliated with acute care facilities, and all non-CAH swing-bed rural hospitals. Section 1888(e)(6)(A)(i) of the Act requires the Secretary to reduce by 2 percentage points the otherwise applicable Annual Payment Update (APU) to a SNF for a FY if the SNF does not comply with the requirements of the SNF QRP for that FY. Section 1888(e)(6)(B)(i)(II) of the Act requires that each SNF submit data on quality measures specified under section 1899B(c)(1) of the Act and data on resource use and other measures specified under section 1899B(d)(1) of the Act in a manner and within the timeframes specified by the Secretary. In addition, section 1888(e)(6)(B)(i)(III) of the Act requires, for FYs beginning on or after October 1, 2018, that each SNF submit standardized patient assessment data required under section 1899B(b)(1) of the Act in a manner and within the timeframes specified by the Secretary. Section 1888(h)(12)(A) of the Act (as added by section 111(a)(4) of Division CC of the Consolidated Appropriations Act, 2021 (Pub. L. 116–260)), requires the Secretary to apply a process to validate data submitted under the SNF VBP Program and the SNF QRP.
