Information Collection Request (ICR) Tracker

ICR Definition

An Information Collection Request (ICR) is a federal agency’s request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

Federal agencies are required to submit an ICR whenever they create, renew, modify, or discontinue an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

ICRs are publicly available on RegInfo.gov, and additional guidance can be found in the FAQs.

Abstract

The Carrier's Report of Issuance of Policy (Form LS-570) is used by authorized insurance carriers to report the policy of insurance issued for each insured employer. This form is to be sent to the Deputy Commissioner in the compensation district indicated by the employer's address. Section 32(a) of the LHWCA (33 USC 932(a)), requires every employer to secure the payment of such compensation with any insurance company authorized by the Secretary, to insure payment of compensation under this Act or receiving an authorization from the Secretary to pay such compensation directly.

Abstract

he information collected serves to identify and develop safety countermeasures that will reduce the severity of injury and property damage caused by motor vehicle crashes. These Investigation-Based Crash Data Studies -- Crash Investigation Sampling System (CISS), Special Crash Investigations (SCI), and Special Studies -- involve voluntary information collections through which NHTSA collects detailed data on real world motor vehicle crashes. Specifically, these systems collect data, on vehicle safety system performance, occupant injury information including their kinematic interaction with interior components and scene geometry, marking and traffic controls. Respondents are police agencies that collection information on police-reported motor vehicle crashes, employees of tow yards where crashed vehicles are stored, people involved in these crashes, and hospitals with medical records for the people injured in the crash. For the standard investigation-based crash data studies acquisition process, once a crash has been selected for investigation, crash technicians or investigators locate, visit, measure, and photograph the crash scene; locate, inspect, and photograph vehicles; conduct a telephone or personal interview with the involved individuals or surrogate (another person who can provide occupant or crash information, such as parents for minor, or a parent or spouse for decreased individual); and obtain and record crash injury information received from various medical data sources. These information collections support NHTSA’s mission to save lives and prevent injuries due to traffic crashes. The data collected from these systems are used to describe and analyze circumstances, mechanisms, and consequences of serious motor vehicle crashes in the United States. Additionally, these data are used by NHTSA to identify the primary factors related to the source of crashes and their injury outcomes, develop and evaluate effective safety countermeasures, the establishment and enforcement of motor vehicle regulations, that reduce the severity of injury and property damage caused by motor vehicle crashes. On November 15, 2021, the Infrastructure Investment and Jobs Act (Pub. L. 117-58), also referred to as the Bipartisan Infrastructure Law (BIL), was signed into law. The Crash Data section (section 24108) of the BIL authorizes the Secretary of Transportation (NHTSA by delegation) to use funds to enhance the collection of data under CISS by, among other things, including additional data collection sites. In the 60-day notice NHTSA published on January 26, 2022 (87 FR 4099), NHTSA estimated that there would be 32 data collection sites in each of the next three years. As a result of the additional funding provided by the BIL, NHTSA now plans to phase in 24 additional data collection sites in CISS over the next 3 years. NHTSA is now accounting for the increases in burden hours for interviewees, Police, Tow Yards and Medical Facilities for an additional 24 data collection sites. The total data collection sites will incrementally increase from 32 to 56 over the next three years. The increase in burden hours and cost for these additional data collection sites are reflected in the Burden to Respondent section of this document. The previous request for CISS (2017) indicated 5,605 burden hours, this request increases the burden to 12,063. The request for the collection of information is revised due to a) Increasing the number of crashes investigated by Crash Technicians for 2021 and future years, b) adding Special Study crashes into this package, and c) adding Special Crash Investigation (SCI) crashes into this package. The combined impact is an increase of 6,458 burden hours to NHTSA’s overall total.

Abstract

President Obama signed Executive Order 13496 (E.O. 13496) on January 30, 2009, requiring certain Government contractors and subcontractors to post notices informing their employees of their rights as employees under Federal labor laws. Regulations 29 CFR § 471.11 provides for the Department's acceptance of written complaints alleging that a contractor doing business with the Federal government has failed to post the notice required by E.O. 13496. The section establishes that no special complaint form is required, but that complaints must be in writing. In addition, written complaints must contain certain information, including the name, address and telephone number of the person submitting the complaint, and the name and address of the Federal contractor alleged to have violated this rule. This section also establishes that written complaints may be submitted either to OFCCP or OLMS.

The type of information collected from potential donors to any of the funds includes name, address, gift amount, payment method and payment information, dedication message, donor intent, status as a returned Peace Corps Volunteer, consent to be listed in the agency’s donor recognition page, consent to share name with Peace Corps Volunteer, and consent to receive information about agency activities and opportunities to donate to or partner with the Peace Corps. Personal information, such as credit card information, is necessary to: a) process donations to the agency, b) respond to donor intent regarding the donation (i.e. dedication message or in honor or in memory of or a restricted gift to support a specific activity or Volunteer community project), c) send donor recognition items, where applicable, and/or, d) send the requisite tax acknowledgement letter.

Abstract

Section 412(a)(2) of the Social Security Act (42 U.S.C. 612(a)(2)), as amended by the Personal Responsibility and Work Opportunity Reconciliation Act of 1996 and the Balanced Budget Act of 1997, created a new tribal work activities program – the Native Employment Works (NEW) program. The NEW program began July 1, 1997. The NEW program plan guidance for Tribes that do not include their NEW programs in Public Law 102-477 projects, and the NEW program report document contain requirements for information needed by the Department of Health and Human Services (HHS) to evaluate and approve plans for funding and to monitor and measure program performance. Eligible Indian tribes and Alaska Native entities must submit program plans (generally once every three years) in order to receive NEW program grants. The plan describes how the grantee will administer and operate its NEW program. NEW grantees must submit program reports annually, providing information on activities and services provided, characteristics of NEW program clients, and program outcomes achieved. The NEW program final regulations (45 CFR Part 287- Attachment A) contain requirements for program plans and program reports. In these regulations, sections 287.70 through 287.110 apply to NEW plans, and sections 287.150 through 287.170 apply to NEW reports. These requirements are reflected in the program plan guidance documents and program report document. The version of these documents for which we now seek approval replaces previous versions. There are minor changes in both documents: (1) the Annual report includes some clarifications on instructions; (2) the Program Plan Guidance includes a change in the directions for submitting a NEW Plan.

Abstract

With respect to all the forms referenced below, the use and reporting of the information is mandatory to operate a non-Federally owned facility in the NAS. Non-Federal sponsors are required to submit the forms to their assigned FAA Inspector for retention. These forms are record keeping documents. Collection frequency varies depending on the form and the situation. Form 6000-10A is filled out once at the commissioning of the facility. Forms 6000-8A and 6030-1 are filled out at each maintenance visit, as needed. Documenting the initial parameters during commissioning is necessary to have a baseline to reference during future inspections. Another requirement is recording maintenance tasks, removal from service, and any other repairs performed on these facilities in on-site logs to have an accurate history on the performance of the facility. In addition, at each periodic inspection, recording the facilities' current parameters provides performance information for the life of the facility. This collection provides the FAA with the necessary data on non-Federally owned facilities to maintain safety in the NAS. There is no plan to publicly disseminate the collected information, nor use it to support publicly disseminated information. The information collected is for internal FAA use only, and not public consumption. The FAA's Non-Federal Program will retain control over the information and safeguard it from improper access, modification, and destruction, consistent with FAA standards for confidentiality, privacy, electronic information and document retention guidance. (See response to Question 10 of this Supporting Statement for more information on confidentiality and privacy.) The information collection is designed to yield data that meet all applicable information quality guidelines. Technical Reference Data Record (TRDR), FAA Form 6000-10A. (Called Record of Meter Readings and Adjustments, Form FAA-198, in 14 CFR Part 171.) TRDRs contain the facility's parameters, including equipment adjustments and all meter readings, and the sponsor must fill it out at the end of commissioning the facility. The sponsor must retain this information on-site. During each periodic inspection, review of this document verifies that the facility remains within initial tolerance. Technical Performance Record (TPR), FAA Form 6000-8A. (Called Radio Equipment Operation Records, Form 418, in 14 CFR Part 171.) TPRs contain a record of system parameters recorded in each scheduled routine maintenance visit to the facility. The sponsor or the sponsor's representatives must keep the original of each record at the facility and send a copy of the form to the FAA. This form provides proof and/or validation that the facility's parameters are within tolerance during each periodic inspection. Facility Maintenance Logs, FAA Form 6030-1. (Called Facility Maintenance Log, Form 406C, in 14 CFR Part 171.) Facility Maintenance Logs are a permanent record of all the activities required to maintain a non-Federal facility. Logging is necessary in accordance with FAA policy. The entries must include all malfunctions encountered, if any, information on adjustments, equipment failures, causes, and corrective action taken. In addition, the entries must include any periodic maintenance required to operate the facility, and issuance of any facility NOTAMs. The FAA must receive copies of the logs. Generally, completion and close out of logs are at the end of each month. However, if maintenance or a scheduled outage carries over into the next month, the relevant log must be closed out at the conclusion of that maintenance or outage. In the case of an unscheduled outage that lasts longer than 60 seconds, the log must be terminated in time to ensure that the FAA will receive a copy of the log within 20 days of the outage's occurrence. This form documents a complete history of the operation and maintenance of the facility for the facility's life cycle until decommissioning.

Abstract

In alignment with the Weather Forecasting and Innovation Act of 2017 (Pub. L. 115-25), two data collections are proposed under this request. There are no other collections for which these can be merged. The first proposed information collection request is sponsored by DOC/NOAA/National Weather Service (NWS)/Office of Science and Technology Integration (OSTI). Currently, NOAA lacks data and data collection instruments that articulate and explicate how individuals receive, interpret, and respond to NOAA information, forecasts, and warnings for severe, winter, and tropical weather hazards. Furthermore, NOAA lacks this type of data longitudinally (i.e., collected over time). The second proposed collection is sponsored through NOAA's FY2021 Weather Program Office's Social Science Program, and addresses the Social, Behavioral, and Economic Sciences (SBES) component of meeting NOAA's Research and Development (R&D) Vision Areas (2020-2026) to integrate SBES into products, tools, and services that improve weather and air quality forecasting and societal outcomes.

Abstract

The response to the collection is voluntary. Persons (current U.S.- flag vessel operators and builders) desiring waiver of the coastwise trade laws affecting small passenger vessels may file a written application and justification for waiver to the Maritime Administration (MARAD). Respondents can submit an application on Form MA-1023 (which is available on MARAD’s website at www.marad.dot.gov under Programs, Small Vessel Waiver Program) or they may submit information in any format they desire. The information that would be reported and maintained in records are Small Vessel Application and Official Letter. These are publicly available in the docket maintained at regulations.gov. The collection frequency is on an as need basis. The information will be used by the Maritime Administration to determine if the applicant is entitled to a waiver to ensure the foreign vessel will not have an undue adverse effect on a current U.S. vessel operator or builder. Since the last approval, the title of the collection has recently changed to 'Application for Coastwise Endorsement Eligibility Determinations for Foreign-built Small Passenger Vessels'.

Abstract

The Bureau of Land Management (BLM) seeks to renew the control number (1004-0121) pertaining to the leasing of solid minerals other than coal and oil shale other than coal and oil shale on Federal land, and the development of those lease. Respondents affected by this information collection request are those who desire to obtain lease for Federal minerals other than coal and oil shale, and operators of such leases. The regulations at 43 CFR part 3590 apply to operations for discovery, testing, development, mining, reclamation, and processing. The BLM request that OMB renew this OMB Control Number of an additional three years.

Abstract

CBP Form 7523, Entry and Manifest of Merchandise Free of Duty, Carrier’s Certificate of Release, is used by carriers and importers as a manifest for the entry of merchandise free of duty under certain conditions, such as when a shipment is valued at $2,500 or less. CBP Form 7523 is also used by carriers to show that articles being imported are to be released to the importer or consignee, and as an inward foreign manifest for vehicles or vessels, weighing less than five tons, arriving from Canada or Mexico, otherwise than by sea, with merchandise conditionally free of duty. CBP uses this form to authorize the entry of such merchandise. CBP Form 7523 is authorized by 19 USC 1433, 1484 and 1498. It is provided for by 19 CFR 123.4 and 19 CFR 143.23. Abstract

10 CFR 50.55a incorporates by reference Division 1 rules of Section III, "Rules for Construction of Nuclear Power Plant Components," and Section XI, "Rules for Inservice Inspection of Nuclear Power Plant Components," of the American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code (B&PV Code); and the rules of the ASME “Code for Operation and Maintenance of Nuclear Power Plants” (OM Code). These rules of the ASME B&PV and OM Codes set forth the requirements to which nuclear power plant components are designed, constructed, tested, repaired, and inspected. The ability of licensed facilities to comply with these requirements may be negatively impacted by the Coronavirus Disease 2019 (COVID-19) Public Health Emergency (PHE) in order to not conflict with practices recommended by the Centers for Disease Control and Prevention (CDC) to limit the spread of COVID-19 (e.g., social distancing) while performing in-service testing and inspections.To facilitate licensees’ requests for alternatives to the requirements in the above regulations, the NRC is providing an online form to submit the required information for a specific alternative request.

Abstract

The Harbor Maintenance Fee (HMF) and Trust Fund is used for the operation and maintenance of certain U.S. channels and harbors by the Army Corps of Engineers. U.S. Customs and Border Protection (CBP) is required to collect the HMF from importers, domestic shippers, and passenger vessel operators using federal navigation projects. Commercial cargo loaded on or unloaded from a commercial vessel is subject to a port use fee of 0.125 percent of its value if the loading or unloading occurs at a port that has been designated by the Army Corps of Engineers. The HMF also applies to the total ticket value of embarking and disembarking passengers and on cargo admissions into a Foreign Trade Zone (FTZ). CBP Form 349, Harbor Maintenance Fee Quarterly Summary Report, and CBP Form 350, Harbor Maintenance Fee Amended Quarterly Summary Report are completed by domestic shippers, foreign trade zone applicants, and passenger vessel operators and submitted with payment to CBP. CBP uses the information collected on CBP Forms 349 and 350 to verify that the fee collected is timely and accurately submitted. These forms are authorized by the Water Resources Development Act of 1986 (26 U.S.C. 4461, et seq.) and provided for by 19 CFR 24.24, which also includes the list of designated ports. CBP Forms 349 and 350 are accessible at http://www.cbp.gov/newsroom/publications/forms or they may be completed and filed electronically at www.pay.gov.

Abstract

34 CFR Part 601 - Institution and Lender Requirements Relating to Education Loans is a section of the regulations governing private education loans offered at covered institutions. These regulations assure the Secretary that the integrity of the program is protected from fraud and misuse of program funds and places requirements on institutions and lenders to ensure that borrowers receive additional disclosures about Title IV, HEA program assistance prior to obtaining a private education loan. The Department is submitting the unchanged Private Education Loan Applicant Self-Certification for OMB's continued approval. While information about the applicant's cost of attendance and estimated financial assistance must be provided to the student, if available, the student will provide the data to the private loan lender who must collect and maintain the self-certification form prior to disbursement of a Private Education Loan. The Department will not receive the Private Education Loan Applicant Self-Certification form and therefore will not be collecting and maintaining the form or its data.

Abstract

In 2008, TSA initiated a process to amend and supersede the primary Federal guidance for pipeline security, the 2002 Pipeline Security Information Circular, with forthcoming Pipeline Security Guidelines. The new document will include recommendations for the voluntary submission of pipeline operator security manager contact information to TSA and the reporting of security incident data to the Transportation Security Operation Center (TSOC). Security manager contact information will enable TSA to provide security-related information to the appropriate personnel and/or the security operations or control center for natural gas and hazardous liquid pipeline operators. Incident information will be used for vulnerability identification and analysis, trend analysis, and for reporting by TSA’s Office of Intelligence.

Abstract

Pursuant to delegated authority under the Defense Production Act (50 App. U.S.C. §§2061-2170), EO 13603, EO 12656, and 46 CFR 340; MARAD issues pre-emergency, standby Port Planning Orders (PPO) to the DoD-designated strategic commercial seaports (port) for providing port facilities to support military deployments during national emergencies. At DoD request (since the 1980's), MARAD has been collecting a Monthly Readiness Report from each port to validate the port’s ability to provide the PPO delineated facilities to DoD within the PPO delineated time frame. GAO Report GAO-14-184SU, dated February 02, 2014, recommended that MARAD collect additional data from the strategic commercial ports. DoD now requires this enhanced data on a quarterly basis for DoD contingency planning and readiness evaluation purposes. The collected information is required to obtain and/or retain benefits. All data collected from the strategic commercial seaports is maintained by the Office of the Associate Administrator for Strategic Sealift as For Official Use Only (FOUO) and is not intended for release to the public.

Abstract

The U.S. Merchant Marine Academy Candidate Application (KP-2-65) is required to obtain admission to the U.S. Merchant Marine Academy. The application consists of three parts. Part I of the application is completed by individuals wishing to be admitted as students to the U.S. Merchant Marine Academy. It is used to pre-screen applicants for basic eligibility and to enter them into the Department of Defense medical system. Part I is usually submitted to the USMMA Admissions Office approximately one month before Parts II and III. The information on Parts II and III of the application is used by the USMMA admissions staff and its Candidate Evaluation Board to select the best qualified candidates for the Academy. Part II is completed by the applicant and Part III is completed by an official at the secondary school where the applicant attends or has attended. Selection of applicants for appointment is made in the order of merit. The order of merit is determined by: the score on the required college entrance examination; an assessment of the applicant’s academic background; and by other factors considered by the USMMA as effective indicators of motivation and the probability that the applicant will complete the Academy’s course of instruction.

Why They Are Important

ICRs play a vital role in ensuring transparency and accountability in federal data collection. When federal agencies collect information from 10 or more "persons" (which includes individuals, businesses, and state, local, and tribal governments), they must submit an ICR to ensure that it fulfills their statutory missions, avoids unnecessary or duplicative requests, and minimizes burden on the American public. Additionally, Federal Register Notices (FRNs) and the opportunity for public comments provide a formal way for the public to be informed of proposed ICRs and participate in the process.

ICRs also serve as a key resource for tracking changes to federal data collections. The availability of detailed documentation, such as data collection instruments and methodologies, allows the general public to identify revisions in a timely manner. These may include revisions prompted by Executive Orders or statistical policies like OMB's Statistical Policy Directive No. 15 (SPD 15), which are often submitted as "nonsubstantive" or "nonmaterial" changes to a currently approved collection.[1] Furthermore, ICRs can help determine if a data collection has expired without renewal or has been intentionally discontinued. By reviewing ICRs, the public can better understand what data is being collected, how it evolves over time, and whether data collections have become inactive–often in response to shifting priorities and updated standards.

How To Use The Tool

The ICR tracking tool offers a user-friendly view of ICRs that have been recently submitted, reviewed, or are nearing expiration. By aggregating key data from individual ICRs, the tool allows users to view the current status of each request, including submission, conclusion, and expiration dates, details on whether any changes were made, authorizing statutes, and more. Users can search for specific information and filter results based on various criteria. If seeking additional information, various text fields are hyperlinked to the full ICR on RegInfo.gov and associated resources.
