Information Collection Request (ICR) Tracker
ICR Definition
An Information Collection Request (ICR) is a federal agency’s request for approval from the Office of Management and Budget (OMB) to collect information from the public.
Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.
Federal agencies are required to submit an ICR whenever they create, renew, modify, or discontinue an information collection. Each ICR includes a description of the collection,
supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.
The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after
which the collection must be renewed through a new ICR submission.
ICRs are publicly available on RegInfo.gov, and additional guidance can be found in the FAQs.

These forms were directed by the Secretary of Defense for transparency and standardization of the mortuary procedures as part of the Final Report of the Dover Port Mortuary Independent Review Subcommittee Implementation Plan and 180-day study. The applicable form(s) is included in the individual case file of the decedent. The respondents are the PADD or PAED of the decedent and the witness to that selection. The PADD or PAED document their election, sign the applicable form to formalize this process and document the election of the PADD or PAED as applicable. These forms become a part of the Official Individual Deceased Personnel File. If the PADD or PAED do not sign these forms, then the Department cannot provide mortuary and transportation services as requested by the PADD or PAED. Currently, there is a lack of standardization across the Military Services as each Service currently utilizes different forms for these elections and they do not all capture the same information even on similar forms. Standardizing the information collected is essential in maintaining the transparency and integrity of the mortuary affairs process. |

AbstractThe U.S. Environmental Protection Agency's (EPA) Chesapeake Bay Program (the Program) is interested in tracking its progress at attaining its goals under the 2014 Chesapeake Bay Watershed Agreement (the Agreement). To do this, the Program plans to implement three surveys: the Citizen Stewardship Survey, the Diversity Profile Survey, and the Local Leadership Survey. The Citizen Stewardship Survey will be implemented as a multi-mode survey that includes phone, web, and mail components and will target approximately 5,200 residents living in the Chesapeake Bay area, stratified by jurisdiction (states and the District of Columbia). The Diversity Profile Survey will be implemented among people who work on partnership efforts within the Bay area as a web-based survey. The Local Leadership Survey will be implemented among state and local elected officials involved in policy making in the Bay area also as a web-based survey. The Diversity Profile Survey and the Local Leadership Survey will be implemented by having the survey link distributed via email by partner organizations and through list-servs. The Diversity Profile Survey will be distributed to approximately 900 people while the Local Leadership Survey will be distributed to approximately 15,000 people. The Program will be using the data from these three surveys to track it progress under the Stewardship goal of the 2014 Agreement. The Stewardship goal includes three outcomes: (1) Citizen Stewardship, (2) Local Leadership, and (3) Diversity. The three surveys under this ICR each address one of the outcomes and contribute to EPA's Government Performance and Results Act (GPRA) goals (EPA Goal 1, A Cleaner, Healthier Environment; Objective 1.2: Provide for Clean and Safe Water). Collecting these data and publishing them for public review will allow the public to track how well the Agreement is working to preserve and protect the Chesapeake Bay region from the standpoint of the Stewardship goal outlined in the Agreement. Overall, the Agreement contains 10 goals and their associated outcomes; data for the other nine goals are collected through other means. Combining the data for Stewardship goal outcomes from these surveys with the data for the other nine goals will provide the public with a comprehensive picture of the progress being made to preserve and protect the Chesapeake Bay. |

Apr. 22, 2021). The Commission updated its existing sponsorship identification requirements to address the issue of undisclosed foreign government provided programming. Specifically, the Commission adopted new foreign government sponsorship identification requirements to require broadcast television and radio stations to make a specific disclosure at the time of broadcast if material aired pursuant to the lease of time on the station has been sponsored, paid for, or furnished by a foreign governmental entity that indicates the specific entity and country involved. Licensees of each broadcast station also are required to exercise reasonable diligence to ascertain whether the foreign sponsorship identification requirements apply at the time of the lease agreement and at any renewal thereof. These obligations expressly apply to any programming broadcast pursuant to a section 325(c) permit. This submission is being made to the Office of Management and Budget (OMB) for approval of the modified disclosure requirements for this Information Collection, as adopted in the 2021 Report and Order. The Affordable Care Act created State-based health insurance Exchanges (Marketplaces), competitive marketplaces where consumers and small businesses can purchase private health insurance. Consumers who access health insurance coverage through a Marketplace can receive direct assistance from Navigators authorized to help consumers through the registration, eligibility determination, and plan selection process as they enroll in coverage. Section 1311(i) requires that a Marketplace establish a Navigator program under which it awards grants to individuals or entities who satisfy the requirements to be Exchange Navigators. Navigators will assist consumers by providing education about and facilitating selection of qualified health plans (QHPs) within Exchanges, as well as other required duties. For Federally-facilitated Marketplaces (FFMs) and State Partnership Marketplaces (SPMs), CMS will be awarding these grants. |

AbstractThis is a request for revision and extension of an existing information collection. The existing OMB control number is being revised to include additional information collection instruments that collect similar kinds of geospatial metadata as Send2NCEI, but that have different community-based practices or standards that provide for more or less details in the metadata requested. Additionally, the title of this collection of information is being revised from National Centers for Environmental Information (NCEI) Send2NCEI Web Application to NOAA Geospatial Metadata to more accurately reflect the revised collection of information. The National Oceanic and Atmospheric Administration (NOAA) provides several digital forms-based tools enabling data producers to create geospatial metadata conforming to International Standards Organization (ISO) standards. These tools collect very similar information but provide specific assistance for different scientific communities across the diverse spectrum of earth, ocean, atmospheric, and fisheries resources. In 1994, Executive Order 12906 established the National Spatial Data Infrastructure and directed each agency to “document all new geospatial data it collects or produces… using the standard under development by the FGDC...”. In 2010, the FGDC adopted the ISO 19115/19139 suite of standards for use by Federal agencies to document geospatial data. The voluntary information collection using these tools facilitates NOAA and others in Federal, State, or local governments, academic institutions, and others to provide clear descriptive information (metadata) about geospatial data. Metadata requested in the tools identifies the data provider, data collector(s),data collection dates, and other information that make the described data useful to other researchers and future users of the described data. Each tool reorganizes descriptive information into an XML file that conforms to the ISO19115-2 Geospatial Metadata standard and the ISO19139 Geographic information Metadata XML schema implementation. A significant advantage for a data provider using one of these tools is that the information is collected through a series of short form-based pages that provide suggestions for entries from established keyword lists and other standard responses. Data providers may opt to use suggested entries or to enter free text into the forms. |

The Act also instructed each OMWI Director to develop standards for assessing the diversity policies and practices of entities regulated by the Agency. The Agencies worked together to develop joint standards (Joint Standards) and, on June 10, 2015, they jointly published in the Federal Register the “Final Interagency Policy Statement Establishing Joint Standards for Assessing the Diversity Policies and Practices of Entities Regulated by the Agencies” (Policy Statement). The Policy Statement includes Joint Standards that cover “Practices to Promote Transparency of Organizational Diversity and Inclusion.” These Joint Standards contemplate that a regulated entity is transparent about its diversity and inclusion activities by making certain information available to the public annually on its Web sites or through other appropriate communications methods, in a manner reflective of the entity’s size and other characteristics. The specific information referenced in these standards is (a) the entity’s diversity and inclusion strategic plan; (b) its policy on its commitment to diversity and inclusion; (c) its progress toward achieving diversity and inclusion in its workforce and procurement activities; and (d) opportunities available at the entity that promote diversity. In addition, the Policy Statement includes Joint Standards that address “Entities’ Self-Assessment.” The Joint Standards for Entities’ Self-Assessment envision that a regulated entity, in a manner reflective of its size and other characteristics, (a) conducts annually a voluntary self-assessment of its diversity policies and practices; (b) monitors and evaluates its performance under its diversity policies and practices on an ongoing basis; (c) provides information pertaining to its self-assessment to the OMWI Director of its primary federal financial regulator; and (d) publishes information pertaining to its efforts with respect to the Joint Standards. |

The Patient Protection and Affordable Care Act, Pub. L. 111-148, was enacted on March 23, 2010, and the Health Care and Education Reconciliation Act of 2010, Pub. L. 111-152, was enacted on March 30, 2010. These statutes are collectively known as the “Affordable Care Act.” The Affordable Care Act extended MHPAEA to apply to the individual health insurance market. The Consolidated Appropriations Act of 2021 (the Appropriations Act) was enacted on December 27, 2020. The Appropriations Act amended MHPAEA, in part, by expressly requiring group health plans and health insurance issuers offering group or individual health insurance coverage that offer both medical/surgical benefits and MH/SUD benefits and that impose non-quantitative treatment limitations (NQTLs) on MH/SUD benefits to perform and document their comparative analyses of the design and application of NQTLs. Further, beginning 45 days after the date of enactment of the Appropriations Act, group health plans and health insurance issuers offering group or individual health insurance coverage must make their comparative analyses available to the Departments of Labor, Health and Human Services (HHS), and the Treasury or applicable state authorities, upon request. The Appropriations Act also requires the Secretary of HHS to submit to Congress, and make publicly available, an annual report on the conclusions of the reviews. |

Why They Are Important
ICRs play a vital role in ensuring transparency and accountability in federal data collection. When federal agencies collect information from 10 or more "persons" (which includes individuals,
businesses, and state, local, and tribal governments), they must submit an ICR to ensure that it fulfills their statutory missions, avoids unnecessary or duplicative requests, and
minimizes burden on the American public. Additionally, Federal Register Notices (FRNs) and the opportunity for public comments provide a formal way for the public to be informed of
proposed ICRs and participate in the process.
ICRs also serve as a key resource for tracking changes to federal data collections. The availability of detailed documentation, such as data collection instruments and methodologies,
allows the general public to identify revisions in a timely manner. These may include revisions prompted by Executive Orders or statistical policies like
OMB's Statistical Policy Directive No. 15 (SPD 15), which are often submitted as "nonsubstantive" or "nonmaterial" changes to a
currently approved collection.[1] Furthermore, ICRs can help determine if a data collection has expired without renewal or has been intentionally
discontinued. By reviewing ICRs, the public can better understand what data is being collected, how it evolves over time, and whether data collections have become inactive–often in response
to shifting priorities and updated standards.
How To Use The Tool
The ICR tracking tool offers a user-friendly view of ICRs that have been recently submitted, reviewed, or are nearing expiration. By aggregating key data from individual ICRs,
the tool allows users to view the current status of each request, including submission, conclusion, and expiration dates, details on whether any changes were made, authorizing statutes,
and more. Users can search for specific information and filter results based on various criteria. If seeking additional information, various text fields are hyperlinked to the full ICR
on RegInfo.gov and associated resources. The tool is updated on a daily basis to reflect the most current information available.
