An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

Federal agencies are required to submit an ICR whenever they create, renew, modify, or discontinue an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

ICRs are publicly available on RegInfo.gov, and additional guidance can be found in the FAQs.

Under 7 USC §6934, the Office of Partnerships and Public Engagement (OPPE) is directed to administer section 2501 of the Food, Agriculture, Conservation, and Trade Act of 1990 (7 USC §2279, The Act). Section 2501 directs the Secretary of Agriculture to carry out an outreach program to encourage and assist socially disadvantaged farmers and ranchers, veteran farmers and ranchers, and beginning farmers and ranchers in owning and operating farms and ranches, and in participating equitably in the full range of agricultural programs offered by the Department. In order to do so, the Secretary may make grants to eligible entities that have demonstrated an ability to provide such outreach and technical assistance. Part A of this ICR addresses the application process for potential grantees to submit applications for funding to the Secretary for the purposes of fulfilling the requirements of section 2501 of The Act. Additionally, the Secretary is directed by section 2501 of The Act to report certain data to Congress on an annual basis. Part B of this request addresses the creation of a web-based system to collect information from grantees that will partially fulfill this requirement. Part C of this request addresses the creation of a survey tool that will allow OPPE to engage directly with program participants from grantee programs in order to collect the data needed to respond fully to the Act’s requirements for Congressional Reporting.

This is a request for clearance for the NEA to conduct a pilot test of the Participant Outcomes Survey to assess individual-level outcomes associated with the Creative Forces®: NEA Military Healing Arts Network Community Engagement Grant Program (http://www.maaa.org/creativeforces/). This is a new data collection request, and the data to be collected are not available elsewhere unless collected through this information collection. The pilot study will enable the NEA to test and refine the survey’s methodology for assessing outcomes and the administration process. The Creative Forces®: NEA Military Healing Arts Network seeks to improve the health, well-being, and quality of life for military and veteran populations exposed to trauma, and for their families and caregivers through clinical and non-clinical programs (https://www.arts.gov/initiatives/creative-forces). Creative Forces is funded through Congressional appropriation. The Congressional Committee on Appropriation “supports the NEA’s continued efforts to expand upon this successful program to embed Creative Arts Therapies at the core of integrative care efforts in clinical settings, advance collaboration among clinical and community arts providers to support wellness and reintegration efforts for affected families, and advance research to improve our understanding of impacts of these interventions in both clinical and community settings.” Beginning in 2022, Creative Forces will award Community Engagement Grants to support non-clinical arts engagement programming for military-connected populations through matching grants of $10,000 to $50,000 for emerging (“Emerging”) and established (“Advanced”) community-based arts engagement projects to serve military-connected populations. The NEA anticipates awarding approximately 30 awards annually, with the first round of grant-funded projects taking place after July 1, 2022. The grant program will support a range of program models (e.g., ongoing class, drop-in studio, single event) designed to meet local needs. The grant program will be the largest coordinated effort in the U.S. to provide community arts engagement programming for military and veteran populations exposed to trauma, and for their families and caregivers. The Creative Forces Community Engagement Grant Program is conducted in partnership with Mid-America Arts Alliance (M-AAA). The purpose of the Participant Outcomes Survey is to determine the impact of the Community Engagement Grant Program by measuring the extent of change over time (pre-to-post) in the four participant outcomes and to inform ongoing program improvement. Through this pilot test of the survey, the NEA will evaluate the effectiveness of the survey instrument and the administration process during the first cycle of the grant program. The data collection activities are planned for January through June 2023. A regular Paperwork Reduction Act clearance package will be submitted in Fiscal Year 2023 for a Creative Forces Community Engagement Grant Program evaluation study that utilizes this survey instrument.

Abstract

14 CFR § 65.11 requires that applications for a certificate or rating under this part be reported on a form and in a manner prescribed by the FAA Administrator. The applications forms are submitted by the applicant, to the FAA, by way of the local Flight Standards Office. The information collected on the forms is, and has been, used by the FAA to determine eligibility for a mechanic, repairman, or parachute rigger certificate and/or rating, or a mechanic certificate inspection authorization. Application and certification is necessary to ensure qualifications of the applicant. A new FAA form, 8610-3, is being requested to collect the repairman applicant information previously collected on FAA Form 8610-2. Additionally, Mechanic and parachute rigger applicants must pass FAA knowledge and practical tests, with a minimum passing grade of 70%, to be eligible for certificate issuance. Applicants may retest 30 days after failing a test, however if an applicant desires to test prior to 30 days from the test failure, they must present a signed statement from an airman holding the certificate and rating sought by the applicant, certifying that the airman has given the applicant additional instruction in each of the subjects failed and that the airman considers the applicant ready for retesting. Certificated parachute riggers must also keep records, as required by §65.131. This is a mandatory requirement of the certificate holder. The record describes the parachute being packed, maintained, or altered. The record must include a description of the work done to the parachute, and the date and place where the work was performed. The records must be kept for at least 2 years after the date each was made. Section 65.93(a)(4) specifies that, mechanics with an IA can complete a refresher course(s) acceptable to the Administrator, of not less than 8 hours of instruction for each year of the two-year renewal requirement. Respondents who are requesting FAA acceptance of an IA renewal course are required to submit FAA Form 8610-6, IA Refresher Course Acceptance Request to the FAA.

NARA uses the information to ensure that only authorized persons have access. All individuals requiring recurring access to non-public areas of NARA’s facilities and IT network must obtain NARA Facility Access Media (FAM). To obtain FAM, they must complete and submit the Facility Access Media (FAM) Request, NA Form 6006 (formerly called Identification Card Request, but changed in this renewal to comply with Government-wide terminology changes). Such individuals include NARA employees, contractors, volunteers, NARA-related foundation employees, foundation volunteers, interns, and other non-NARA Federal employees, such as Federal agency reviewers. People requesting FAM are referred to as “applicants.” If we approve the request, the applicant receives a FAM, which provides access to the non-public areas of NARA’s facilities and IT network. Abstract

The High-Frequency Surveys Program was established as a natural progression from the creation of the Household Pulse Survey. The Census Bureau developed the Household Pulse Survey to produce near real-time data in a time of urgent and acute need to inform federal and state action in response to the Covid-19 pandemic. Changes in the measures over time provided insight into individuals’ experiences on social and economic dimensions during the period of the pandemic. It has evolved to include content on other emergent social and economic issues facing households and is designed to supplement the federal statistical system’s traditional benchmark data products with a new data source that provides relevant and timely information based on a high-quality sample frame, data integration, and cooperative expertise. In January 2025, the HPS and CHP will be relaunched as the Household Trends and Outlook Pulse Survey (HTOPS). The HTOPS will continue the strong tradition of the HPS by rapidly providing insight into national events that have social and economic impacts on U.S. households. It will complement more traditional federal surveys by producing data much closer to real time as the events develop and with the new longitudinal design, have the ability to track changes over time. Along with HPS content, the survey will enable the Census Bureau to address research and content development needs for its census and survey programs. The HTOPS will be one more tool among the Census Bureau's data collection efforts to provide the nation with quality, up-to-date information that informs our future.

Section 343(a) of the Immigration and Nationality Act (INA) (8 U.S.C. 1454) provides that if any certificate of naturalization or citizenship issued to any citizen or any declaration of intention furnished to any declarant is lost, mutilated, or destroyed, the citizen or declarant may make application to the Secretary of Homeland Security for a new certificate or declaration. Additionally, INA 343(b) provides for the issuance of special certificates of naturalization for the purpose of obtaining recognition as a citizen of the United States by a foreign state, and INA 343(c) provides for the issuance of a new certificate of naturalization in cases where a naturalized citizen has changed his or her name after naturalization. Finally, 8 C.F.R. 338.5 provides for the issuance of a corrected Certificate of Naturalization where the certificate does not conform to the facts shown on the application for naturalization or a clerical error was made in preparing the certificate. The Form N-565 is used to apply for a replacement of a Declaration of Intention, Naturalization Certificate, Certificate of Citizenship or Repatriation Certificate, or to apply for a special certificate of naturalization as a U.S. citizen to be recognized by a foreign country.
